If you happen to go to the hospital for any reason, feel free to call us before you go or if you are unable to do so, please ask your discharge planner at the hospital to contact us and we will make sure to see you when you return home.

Visiting Home Health Aides
  • Assists with personal care hands-on

Visitng Occupational Therapist

Establishes a home program for patients with the following conditions:

  • Cerebrovascular accidents
  • Exacerbation of Chronic Obstructive
  • Pulmonary Disease (COPD)/ Arthritis
  • Upper body weakness or trauma

Affinity Home Care Visiting Occupational Therapists Assist and train patients with transfers, activities of daily living, and fine motor coordination

Visitng Medical Social Worker
  • Assess the social/emotional aspects pertinent to the patient diagnosis and environment
  • Assist with plans for long-term care and disease management
  • Counsel patients on adaptation to the disease limitations
  • Educate and provide information on community resources related to the patient's diagnosis
