We have existed since 2015. Martha's CBRF is a wheelchair-accessible home-like 15-bed Community Based Residential Facility in lovely Princeton, WI. We have 13 private rooms with bathrooms and 2 couple rooms with half baths. Care services are available on a continuous, 24-hour basis by resident care aides who will provide assistance with activities of daily living (ADLs), which may include: dressing, meal time, hygiene and grooming, mobility, bathing, toileting, incontinence care, using the telephone and shopping. "My father has moved in. The people have been great. The administrator is great, and the staff has done a good job with him. He only has one bedroom because it's memory care. He's more with it than most of the people there. They usually would have people come in and play the piano. The facility is good."

"I found a really nice place for my brother. The Residences on Forest Lane has such a nice atmosphere, it's like you're at home. The ones that I met seemed to be happy. The rooms are nicely sized. They have their own bathroom with a shower, and they've got a little refrigerator. The Montello School is right across the street, and a nursing home is right next door. It's got a nice yard, a pond, a library, and a fireplace. "

"My mother has been here for almost three years. Our experience there has been very good. There have been two or three corporate changes, and the quality of the food varies, but overall, it has been very good. The residents are happy to be there, and it is very pleasant. She is living in a two-bedroom apartment and my brother lives in the other bedroom because he is her caregiver and they have a cat. My mother has the things that she needs. She cannot go to a place that does not allow pets. It is a very nice and lovely spot. "

"We toured A Place for Us. It was delightful. I liked the owner-manager, and I got to meet the residents. The only thing I would say is they didn't have any outdoor space, and my sister likes to be outdoors and sit outside. She is a smoker, and she would have had to find a place where she could smoke. So that was a concern because there was no outdoor sort of sitting space or garden or anything. The owner was new. She had just bought the place in February. So it's upgrading. The rooms were very large, bright, and well-designed. I thought that was one of the reasons I was considering it. The whole place was very clean and it smelled good, which is always important."

"I visited Remembrance Home. It’s a giant open concept and each person gets a room with an individual bathroom, but you cannot bring your furniture. The place was very nice, just not fit for my grandmother. "

"Prairie Ridge Assisted Living seemed like a nice place. It was large and all on one level. It was a small apartment that they showed me. There was a very nice meeting place or a lounge when you walked in, and that's where they ate. There were several tables and chairs in there, and it looked nice. They would have supplied the care my mom needed. I didn't see anything going on when I was there, although they said they had activities. There was an activity area and I think that takes place where they eat, in that same room. It was like a big lobby when you walked in. The staff who assisted me was somewhat pretty informative. It was a nurse's aide, and she seemed alright."

"Christian Home & Rehabilitation Center is a new facility. They can't take my relative in right now, because of the nursing shortage and COVID. We are thinking of choosing this place because he gets more square footage for a room. So it's kind of an actual living area that's acceptable. They have everything available right there. They have haircuts, if he has to go to rehab it's right there, and the nursing home is right there if he ever has to move over. The staff who gave the tour was very nice, and she showed us all the benefits of living there. They have snacks throughout the day and things like that. The facility is very nice. They keep all the assisted living in one section, the nursing home in another section, and rehab in a separate section. If they can go through the little store that is in there, they can go to any of the little shops that are in there. They have some little shops there. It's only a couple of years old, so it's very clean. It feels more like a home for my relative, and I would recommend them to anybody."

"After spine surgery I went to Hillside Manor to recuperate. The aids, nurses and other staff there were just wonderful. I especially liked my physical therapists. We had a lot of fun together! The building and the grounds were just so very nice. Outside was a gazebo and paths to walk along and look at the flowers and plants. Inside they have aviaries. It's so calming to sit and watch the birds. AND, they have lap-top computers that are internet accessible. The place was always clean and the staff so friendly. I nearly cried when it came time for me to come home! And the food? Well, it really wasn't bad at all. And if you didn't want to eat what was posted on the menu for that day you could go down to the cafeteria and get what you wanted and the put the cost on your bill. It's part of the Beaver Dam Community Hospital so a person has a lot of walkiing they can do if they'd like. If ever I have to have that type of caree again it will be there. And, I've told friends and family about how nice it is too. "

"We visited Eagles Wings while in the area looking at assisted living facilities to consider for our uncle. The outside had lovely grounds with nicely landscaped shrubs, trees and lawns as well as ample seating areas.We could see that they can accommodate a variety of ambulation levels and as needs change, the care team adjusts accordingly. There were ample community rooms for a variety of stimulating activities such as reading, games, socials, physical exercise and hosted events The fitness center had regular exercise classes, such as stretch and yoga and aerobics classes. There are also personal trainers who come regularly to instruct the residents. The meal menus all had healthy food options for all kinds of diets, as well as healthy snacks regularly offered whenever residents request them.The dining room was nicely designed with comfortable furnishings. The meals we saw looked and smelled appetizing. The outside activities feature many interesting cultural events in the wonderful Beaver Dam and surrounding areas, like films, art and other interesting cultural gatherings. The private spaces had nice, personalized decorations and easy access furnishings with privacy, fresh air and light. We were impressed with how nicely designed all of the living spaces were, and how comfortable the seniors appeared. The staff was also very tender, thoughtful and accommodating. "

"My dad moved into Beaver Dam Health Care Center. They feed him well, but they don't take care of him very well at all. They don't do anything. They get him up, put him in a chair, and leave him. They don't do any therapy or anything like that like they're supposed to be, and that's just awful. The only good thing I think is that they feed him well. Some of the staff are good and some of them are bad. I haven't tried the food there, but they do feed them well, a healthy, well-balanced diet. They don't do any activities. My dad has no towels in his room. We had to bring our own and they're supposed to supply all this stuff and they let him sleep in a chair instead of getting him in bed. They don't clean all that much. I don't think we're getting value for our money. Medicaid covers most of that. But we pay so much."

"My father has moved into Prairie Ridge Assisted Living - Waupun. The rooms are a very nice size. They have a lot of open spaces where you can go do activities. It's just a nice and friendly atmosphere when you walk in. I think they are very on top of things. I had to take my dad to the doctor for something. When I came back, I said, "I'm not sure he's going to do this. Would you please make sure you keep an eye on him?" They wrote it down on the chart, and they've been very good about it. They follow through. The staff is very friendly. When you walk through the door, they'll ask if they can help you. They always greet you and say, "Hi, how are you doing?" They're very, very nice. I have not had any issues so far. My dad was there also about six years ago after he had some surgery, and he couldn't be on his own. He got better and left. I couldn't tell the difference inside and outside of the building. It's very well kept. The shrubbery outside is all trimmed nice, and the lawn was cut every week. The carpets looked like they were vacuumed quite frequently. They clean his apartment, and they do a very nice job. It looks nice there. They always have puzzles out. They have a TV in the main area that people can go to, and once a month people can sign up, like last month they went to Walmart and Hobby Lobby, and then they went out to eat. I think that's nice that they're able to get out and have a nice day. They always have something every day and it's always written on the whiteboard what they're going to be doing. They go down into the rooms and tell the people who don't come, "Hey, this is going to start. You want me to take you down there?" They are encouraging them. My dad doesn't need a lot of care right now and I know the more care, the higher the price is, but it's still pretty pricy. I would have no problem recommending them."
