We have existed since 2015. Martha's CBRF is a wheelchair-accessible home-like 15-bed Community Based Residential Facility in lovely Princeton, WI. We have 13 private rooms with bathrooms and 2 couple rooms with half baths. Care services are available on a continuous, 24-hour basis by resident care aides who will provide assistance with activities of daily living (ADLs), which may include: dressing, meal time, hygiene and grooming, mobility, bathing, toileting, incontinence care, using the telephone and shopping. "It was a very positive tour experience. The people there were very warm and friendly. The staff looked like they were doing an excellent job of interacting with the residents. It is strictly memory care, so there aren't people that don't have memory issues interacting with people who have dementia. It seemed to be a better fit for my sister. There are only 20 rooms. They have three available right now, so it is a small community. It just seems very pleasant and it's residential. It's not in a busy part of town. We left there yesterday with a very positive feeling. The rooms were relatively small. They only have a half bath attached, but they have two rooms in which residents can shower and they help with that, which is good. The room itself is about 15x15. It has a decent-sized closet. They can provide furniture or you can provide your own, and I think we're going to provide furniture. It has a nice view. There are birdfeeders right outside her window, which she will love. The staff was excellent. We did not see a menu, but they have a kitchen there. They prepare all the meals there, from fresh food not shipped in things. We were there shortly after lunch, so people had already eaten. They have several rooms, large rooms that are set up almost like living rooms. One has a fireplace in it and actually an organ in it. Another room has a number of tables set up. They've got some puzzles that people are working on. There were people just sitting and visiting and doing some work with colored pencils. They had a lot of nice activities available that people could participate in if they chose to. They don't force anybody to do anything, but we're hoping that she will adapt and be a little more sociable when she's there. The dining area was small, but there were only 20 residents, so there are maybe four or five tables set up. They will serve meals in the room if that's what the resident wants. They don't force people to gather, which is good. I was impressed with the facility when we toured it, but I don't know what our experience will be like once we actually have her moved in. The staff during the tour was very, very nice. She's not there all the time. They have three facilities in the area and she spends time at each of them. They're going to be doing some remodeling, but they need some updating and things like that. They're working on it. They said they have new furniture ordered for the lounge area, and they're trying to replace the floors in various rooms."

"This facility is one of the best that my family has experienced for my dad and he has been to many facilities in the past few years. The staff communicates with me on everything regarding my father. He has been there for over a year and he calls this place home. The new administrator is passionate at what she does and the residents love her like family. She goes above and beyond for their needs and never turns her head. She is one of the best assets this facility has. She has a huge heart and she leads by example for the rest of her staff. Speaking of the staff, they all communicate well and care about the residents so much! This facility feels welcoming, the food is delicious with many options and if you didn’t want what was being served, they would make you a sandwich or soup. They do activities with all the residents, games to watching a movie and eating popcorn. This facility is clean and smells great! Always greeted with a smile, and always doing improvements to make you feel comfortable. I highly recommend this facility. Thank you, Residence’s on Forest Lane! "

"Faith Gardens was fantastic, I would recommend them any time, any place. The staff was very warm and welcoming. The clients were smiling and all seemed happy. They have different size rooms, and each one had their own bathroom, a nice closet, and some shelves. The rooms were a decent size, and overall, we were highly impressed. "

"Remembrance Home is a place that I was looking at for my friend. Actually she was on their waitlist. It's small, so my friend would not get lost. Everything is in a circle; she would always end up back at the same place she started. They have a non-denominational pastor coming in. There is a mass service and a Sunday morning service. They have a salon area, too. I also saw some of the people reading. I would give this place a 5 because if they had an opening when my friend had to go some place, that's where she would have went."

"My mother has moved into Prairie Ridge Assisted Living. They have been awesome and helping me place my mom. It's very, very clean. The rooms are very clean. They supply toiletries and paper towel dispensers. The staff is very caring. They make sure that the residents are down for their meals, and if they can't make it down, they bring their meals to them. They have activities for them to do which they encourage them to participate in. The staff interacts very well with the residents. I'm very pleased with them. The food is good. There are good portions. My mom has not had any complaints about the food. Yesterday, they had a field trip to Fond du Lac, Hobby Lobby, and Gilles. They have bingo, and today they had KerPlunk. One thing I like is every day they have exercise, two sessions, one at 10:00 and one at 2:00. They have card games, they have a puzzle in the main area for all the residents to work on, and they had an ice cream social last Friday for the residents. They have a coffee social. They have a church that the residents could participate in. They have bowling, beach volleyball, arts and crafts, and they did painting, and Halloween coloring. Every day, they have stuff for them to participate in."

"Christian Home & Rehabilitation Center is a new facility. They can't take my relative in right now, because of the nursing shortage and COVID. We are thinking of choosing this place because he gets more square footage for a room. So it's kind of an actual living area that's acceptable. They have everything available right there. They have haircuts, if he has to go to rehab it's right there, and the nursing home is right there if he ever has to move over. The staff who gave the tour was very nice, and she showed us all the benefits of living there. They have snacks throughout the day and things like that. The facility is very nice. They keep all the assisted living in one section, the nursing home in another section, and rehab in a separate section. If they can go through the little store that is in there, they can go to any of the little shops that are in there. They have some little shops there. It's only a couple of years old, so it's very clean. It feels more like a home for my relative, and I would recommend them to anybody."

"My husband is in rehab at Beaver Dam Community Hospitals Hillside Manor nursing facility. He's been there a long time, and it's very good. He's been getting great care. Overall, the staff is great, and his room is large and adequate. For activities, they have music that my husband goes to, and they play balloon volleyball, too. When the insurance paid for it, it was great, but we're paying out of pocket now, and it's very expensive."

"We visited Eagles Wings while in the area looking at assisted living facilities to consider for our uncle. The outside had lovely grounds with nicely landscaped shrubs, trees and lawns as well as ample seating areas.We could see that they can accommodate a variety of ambulation levels and as needs change, the care team adjusts accordingly. There were ample community rooms for a variety of stimulating activities such as reading, games, socials, physical exercise and hosted events The fitness center had regular exercise classes, such as stretch and yoga and aerobics classes. There are also personal trainers who come regularly to instruct the residents. The meal menus all had healthy food options for all kinds of diets, as well as healthy snacks regularly offered whenever residents request them.The dining room was nicely designed with comfortable furnishings. The meals we saw looked and smelled appetizing. The outside activities feature many interesting cultural events in the wonderful Beaver Dam and surrounding areas, like films, art and other interesting cultural gatherings. The private spaces had nice, personalized decorations and easy access furnishings with privacy, fresh air and light. We were impressed with how nicely designed all of the living spaces were, and how comfortable the seniors appeared. The staff was also very tender, thoughtful and accommodating. "

"My dad moved into Beaver Dam Health Care Center. They feed him well, but they don't take care of him very well at all. They don't do anything. They get him up, put him in a chair, and leave him. They don't do any therapy or anything like that like they're supposed to be, and that's just awful. The only good thing I think is that they feed him well. Some of the staff are good and some of them are bad. I haven't tried the food there, but they do feed them well, a healthy, well-balanced diet. They don't do any activities. My dad has no towels in his room. We had to bring our own and they're supposed to supply all this stuff and they let him sleep in a chair instead of getting him in bed. They don't clean all that much. I don't think we're getting value for our money. Medicaid covers most of that. But we pay so much."

"My mom moved to Prairie Ridge Assisted Living. The individual apartments are quite sizable. The staff has been very friendly and open throughout the process and accommodating. Everything's very easy to get to. The halls are nice and wide. I wish they had central air, other than that everything is very clean and easily accessible. I've only had one meal there. The dining area is pretty basic. They have tables and staff who bring the meal to you, but it's just your basic open space. They had a fair amount of activities for the residents to partake in. They have somebody who can come in and do the residents' hair for a fee. They have dietary staff. They have an exercise area and exercise equipment. They've got a chapel. It's pretty expensive. It's probably on the lower end of other facilities from a cost perspective, but a lot of that has to do with location."

"My sister stayed at Markesan Resident Home for rehab. It was excellent. The people there were very caring. They took extremely good care of her. The place was clean. It was very nice, and we can't say enough positive about it. It was wonderful. The rooms were nice. She had a private room with a window, and it was very clean. She had a bathroom she shared with someone else, but there was nobody in the other room. The food was good. They just took such good care of her. It didn't smell bad at all, it smelled really good. Everybody that's worked there has worked there for a long time. So, they don't have a big turnover which is always a good sign. My sister didn't take part in any of the activities, but one thing they had was a hair salon. Then they had music, activities, and art activities. They have a dining room and a courtyard. In terms of cost, it's pretty average. They give you everything other places might say they give you, but the others aren't probably as good at it."
