Welcome to Hill Haven! Hill Haven Transitional Care Center offers a comprehensive range of medical and assisted living senior services, clinically coordinated with Rochester Regional Health.Our goal at Hill Haven Transitional Care Center is to help our residents reach their greatest level of physical functioning and emotional well-being. Residents at Hill Haven Transitional Care Center are provided with help with daily living activities of daily living (dressing, bathing, eating). Registered dieticians plan nutritious, appetizing meals and snacks. Recreational therapists encourage residents to participate in as many activities and events as interest them. Our senior services staff help them stay as connected to the community as possible with special programs and an open door for family and friends. We assess each resident’s needs and abilities and ask about their goals so we can tailor an assisted living care plan just for them. Consistent senior care staff assignments mean we really get to know and establish real relationships with our residents and their families.
