Food is attractive. plentiful and tasteful. Care appears to be mid-range. Not enough chairs for visitation AND NICE CLOTHING OR ITEMS BROUGHT IN OFTEN DISAPPEAR OR ARE "NO SHOWS" This is serious as plenty of thought and expense goes into our choices to please our friends and relatives who are so dependent upon acts of love and kIndness. AFFINITY, YOU HAVE TO DO BETTER TO TRACK THESE LOSSES AND FIND THE CAUSE AND TAKE MEASURES TO PROTECT THE RESIDENTS PERSONAL PROPERTY!!!

My mother, Joan, entered Affinity In December 2022. She was admitted on floor two for rehabilitation. She ended up getting Covid and had a setback and went up to floor three where she will remain as a forever patient. The nurses and the aides on the second floor were absolutely wonderful. They were friendly, they remember who you were and they were more than willing to help. The PT’s, OT’s and speech therapists were absolutely fabulous. They all knew who my mother was and who we were. We were a little upset when she went up to floor three until we met the aids and the nurses who were absolutely fabulous. Renee has control over everything that’s going on on third floor. She make sure everything is taken care of everyone is taken care of. She answers questions for you and if she doesn’t have the answer she finds it. Everyone on the third floor makes me feel comfortable and the rest of my family when we go to visit our mother. They all greet you because they know you. It’s like one big happy family. I would definitely recommend this facility to anyone who has a parent, elder family member or for themselves if rehabilitation or long-term care is needed. The facility is constantly getting cleaned. I see people cleaning every time I go there which is at least five times a week. The food is not bad. My mother enjoys some food items more than others, and my mother always gets a sandwich with her lunch in case she’s still hungry. And mid day between lunch and dinner a snack is always served. My family is very happy my mother is so well taken care of.

The facility is woefully understaffed, with one nurse sometimes covering an entire floor. Aides receive no supervision or training regarding the importance of interacting with residents. One aide can sit in a room with residents and focus on her iPhone. They don't seem to understand that patients need to be involved, even if only a brief comment, to feel they are still alive. Too often the residents are left in the hallway with absolutely no social interaction, as if the residents were more "things" than human. Food is average, schedules regarding showering, feeding are oftentimes ignored. Unfortunately, this facility is not the only facility functioning on this level. Management should be mandating ongoing staff development focusing on the psychological impact of missing social interaction with others.

I spent two and a half weeks there and found the staff and medical personnel to be nothing but courteous, always willing to help (even in the middle of the night) and prompt to give assistance whenever I needed it. Their concern was obviously genuine and they often went the extra mile for me, as well as responding to the many questions and concerns of my son, who was my caregiver during my stay. The physical therapy was by far the greatest feature ... everyone challenged me to get me back on my feet but never forced my therapy. And the cleanliness of my room was noted by my visiting friends and family members.

Not all that glitters is gold! Even though the facility appears to be eye appealing, the care leaves a lot to be desired. There is only a few staff that truly care. The rest do not treat the residents and family members as people with feelings. Times may be tough, but that does not give anyone an excuse to disregard humanity. They may say what you want to hear but don't believe anything until you see it with your own eyes! Nothing but empty words and unfulfilled promises. They consistently lie and try to intimidate anyone who asks a question. Everyone in this facility must have an strong advocate or you will not see any progress. In fact, you will see your loved one going downhill. I have not seen any activities for the residents at all. Many of the staff are extremely rude. They all complain of being overworked and do not have enough time to take care of the residents. They never return phone calls. They make it impossible to visit with your loved ones. They are annoyed when you visit. Then they are held accountable. They blame everything on Covid. Their excuse for no transparency. They do not like their jobs. And it surely shows. Residents lay in soiled diapers hours on end. Sadly, the residents suffer. I would not recommend this facility at all.

My father was a patient on the vent floor for 2 days in November 2020. My father fell out of bed had his feeding tube dislodged and replaced by them which caused massive internal bleeding. He was left in his bed for 2 days bleeding and not sent to the hospital even after he asked to go. After 2 days the night nurse contacted me and asked if I wanted him to go and I demanded he did. He had two major surgery’s to stop the bleeding. If you honestly care about your loved one I would never send my family member to this place. They do not care about your loved ones they only care about money

This place is Just Awful! My brother was admitted for Rehab. He chose Affinity because he had stayed there about 5 years ago and had a good experience. Seems this place has decided to cut costs. Their Social Workers Chantel and Dana are extremely rude. They are a poor choice for the face of Affinity. The Social Workers wanted us to terminate my brothers insurance and put him on Medicare so they didn’t have to recertification him weekly. They didn’t care that when he was released that this action would cause problems with his doctors and prescriptions. They would schedule family phone Care Meetings with us then change the day and or time and not notify us. When I questioned the Care Meeting change, Dana was screaming at me that Chantels family member was murdered the night before. I extended my condolences and asked about rescheduling to which she again screamed about the murder of Chantels family member. Later that day we were informed my brother was being discharged as was his his roommate. His family asked too many questions also. I asked to speak to Dana and she would not get on the phone because she was angry at me. This place is beyond terrible and very, very short staffed. They use Covid to their advantage, keeping family members out of the building. It take a long time to get an appointment to see your loved one. They put patients to bed at 6:30pm in diapers. Patients are told to use the diapers in place of going to the bathroom. Not enough aides to assist them. Food is terrible, vegetables puréed and not editable. Look else where if you love your family member.

First..the food is absolutely horrible!! Words can't describe it! (Slop) Food is ALWAYS cold! 2.takes FOREVER to get ur much needed meds!!! You have to ring the nurses bell AT LEAST 5. 6 XS for them to finally answer..too 2 hrs 15 mins to get my panic meds I thought I was going crazy. There's few I really like p/t Mike super caring ! Jennifer...norman very caring. I am Impatient WANT TO GO HOME!!. DONT RECOMMEND AFFINITY. Mrs good size. During covid so Nzo activites

DO NOT BRING YOUR Loved ones here. Nursing staff overwhelmed because of not enough staff to care for pts. One nurse on 3-11 with 30 pts..an 3 aides. This shift feeds baths does treatments etc and put pts to bed.SAFTEY IS MAJOR PROBLEM. PT FELL OUT OF WHEELCHAIR IN FRONT OF OUR EYES. NO one around they were ALL busy. Food they gave my aunt smelled horrible. Looked bad too For about 3 weeks this summer there was NO AIR CONDITIONING. NOONE COULD BREATH..consstruction work had ceilings torn apart. Pts AND nurses became ill. Mold an mildew in that ceiling. My aunt is home now with private cna . This place horrible

Not good at all! 2 aides for 30 patients is unacceptable. Too many mistakes made by nurses, wrong meds for wrong people. Not enough attention paid to patients. Not enough PT-1 hour does nothing for patients. My husband wound up with a very serious Urinary Tract Infection due to him being left to sit in diapers all day long. When asked to go to bathroom he was told "it is not poop time now"!! Unacceptable, unacceptable, unacceptable. I will be sending letters to the state and any other agency I feel should know about this.

My mother has been there for 8 years. When I made vist this past Monday, I found her sitting in the corner of her bathroom stripped of all of her clothing. She does not walk and cannot remove her clothes. The aide was no where to be found. After a search, she appears and says she had to "pee". She got a 2 day suspension. I don't buy it. I'm not sure what happened but I will never get the image of my mother sitting there naked, with her head hanging down. I was told this was a 1st incident..really, the 1st they know of because I found her.

My Dad who had to be transferred to Affinity from the V.A. was sent to Affinity. we were so scared after reading the reviews to that had been posted fooled us into believing the place was a dump is 100 percent wrong. The home in my opinion is the bestt place my father could have been placed in Long Island. I see very caring oersonnell; clean ; and I can,'t say enough about the people; premises; and respitory team that has actually been trying to wean my dad off the VENTILATOR. I will never base a review to form an opinion B4 actually judging for myself: so don't let reviews scare you until you can form your own opinion. Barry who is a respiratory therapist has gotten my dad slowly and safely to the point where we now have some hope when there was NONE....We we're fooled by the reviews! I am actually happy that a well trained; caring staff is looking after my DAD. We came in early; and to our surprise my father was dressed in a nice shirt and shorts. He was bathed and content. If you knew my Dad he is the most difficult man to please. We as a family were very frightened when he arrived as the reviews are NOT TRUE. Again; I am writing this review because others may be fooled as we were. I see a caring staff and clean environment. Know I am not an easy person to please and I will never listen to something I read before accessing the situation myself. Kudos to Affinity and there staff. May God bless them all at Affinity and do make your own opinions before coming to conclusions. W due respect I apologise for the way I acted when I arrived. I thank God he iwas placed in the facility. We as a family are content knowing my father is safe and alert. He is even getting physical therapy when last week we had no hope at all to hang onto. This is a terrible place to put your loved one. I wish I had never put my father in here. It's too late now.He fell four times in two weeks in this nursing home. If you love your family member do not put them in Affinity Nursing Home in Sayville.

Parent was at Affinity for a couple of weeks, Parent told us staff scolded her out loud when she had a bathroom accident; staff complained to her; staff made parent feel worse. On visits, staff appeared pleasant and caring. Parent cried to come home, Staff was often no where to be found during visits. Laundry was lost or wrong laundry came back although it had patient's name. Other patient said laundry lost blankets. Place was not well-heated for patients. Staff was not available for patient to move around except to sit up, not walk. Need I say more?

The facility is clean and quite. That is the only good thing I would have to say about Affinity. My mother uses a walker for a condition with her feet, but she was there for rehab after a heart issue. In the morning she was plopped in a wheelchair and wheeled out to the lobby for the day and told that she wasn't allowed to return to her room or sit in the chair in her room. There she sat like one of the nursing home patients. When she asked if she could get a straw to drink some juice served with her meal she was simply told "no". The same went for asking that the raw english muffin be toasted, nope. No toasting. We took my mother home prior to her official discharge, and we were told that "the police would be called" if we took her out. Affinity was not a good fit for my mother and family. Remember, looks can be deceiving! Share

The best I can say is that this place is the anti rehab - All the patients sit in the hall way and do nothing all day. There are some nice nurses but that is all I can say about this place. My mother went there able to walk and stand and left with her legs bent and no longer able to walk. Seems to me that all they care about is getting the money from the insurance. Do yourself a favor and stay away from this place

I think Annie is a very low cost place to put your parent or loved one. I actually forgot to give my mother her medicine. They tried to give her someone else's medicine, I don't check Dan's to make sure that they're giving the medication read the correct person. They are so short staffed it's pathetic. My mother has weird over 45 minutes to go to the bathroom. Their service is now the reason of why my mother is being transferred. The only nice thing I can say about affinity rehabilitation is a nurse named Jen on one north. She is very nice and will always help patients. If you ask a nurse to heat up your cold food, they will tell you it's not their job.

I visited this facility

My 91 yr old mother-in-law, Margaret, was admitted o Affinity from Good Samaritan hospital for lung issues. The staff was fabulous! She was always clean and dressed, including hair and nails done. They have a state of the art physical therapy unit. Kristen, Carrie, Erin,Wanna, Jessica, Deidre, Dorothy,Ray, and Lee Anne the social worker were so kind and helpful. I could hear them talk to other patients with dignity and respect. I know that I could sleep well at night knowing that she was well cared for.

I went to visit this place for my aunt..i was greeted by a worker named teri a pleasant very friendly italian girl..she told me all about the nursing home..im impressed by the dedication. Of this skilled worker..im going to set up a interview for my aunt..thanks to teri i was pleased to have someone with such polite manners!!

I MADE MY FIRST VISIT TODAY. MY BROTHER WAS BROUGHT THERE FROM THE HOSPITAL, WITH A BRAIN BLEED. WHEN I FOUND HIM SITTING BY HIMSELF IN THE LOBBY OF HIS FLOOR, IT TOOK MY BREATH AWAY, HE LOOKED SO BAD.HIS FIRST, LAST AND ALL IN BETWEEN WORDS TO ME WERE..."PLEASE GET ME OUT OF HERE. THE STAFF CONSTANTLY YELLS AT US...I HAVE BEEN ASKING FOR WATER FOR A LONG TIME....I DON'T SEE ANY DOCTORS HERE AND I THINK I AM GOING TO DIE HERE...PLEASE GET ME OUT AND TAKE ME TO ANOTHER HOSPITAL". THE FIRST THING I DID WAS TRY TO GET HIM A GLASS OF WATER. AFTER A 15 MINUTE "DISCUSSION" WITH ONE OF HIS "NURSES" I ASKED TO SEE A SUPVR. ONE CAME AND THE DISCUSSION CONTINUED. I PLEADED WITH THEM TO PLEASE GET MY BROTHER SOME WATER. ANOTHER 15 MINS PASSES AND FINALLY SOMEONE BROUGHT HIM A SMALL PLASTIC CUP OF WARM WATER. DURING THIS FIASCO, ANOTHER PATIENT CAME UP TO ME IN HIS WHEEL CHAIR AND VIOLENTLY ATTACKED ME WHILE SHOUTING..."GET OUT OF HERE". HE CONTINUED TO ATTACK ME AND FINALLY A "NURSE" TOOK HIM DOWN THE HALL (TO SIT IN THE HALL). I ASKED FOR A SECURITY PERSON AND WAS TOLD "THERE ARE NO SECURITY PEOPLE THAT WORK HERE". WHEN I ASKED TO SPEAK TO MY BROTHERS "NURSE" (WHO HAS THE RESPONSIBILITY FOR THE ENTIRE FLOOR) HE WAS NOT VERY FORTHCOMING WITH INFORMATION, SUCH AS: "WHAT IS THE NAME OF MY BROTHERS DOCTOR?" HE GAVE ME THE NAME OF SOMEONE WHO IS NOT EVEN ON STAFF. THEN I SAID"WHO IS THE REFERRING DOCTOR" HE SAID HE HAD NO IDEA. I ASKED HIM IF HE KNEW WHAT MEDS MY BROTHER WAS ON...AGAIN...HE HAD NO IDEA. I IDENTIFIED MYSELF AS HIS SISTER, SHOWED HIM MY ID AND TOLD HIM I WAS MY BROTHERS MEDICAL PROXY. STILL I RECEIVED NO ANSWERS. NO ONE IS ADDRESSING MY BROTHERS BRAIN BLEED, AND HE WAS TAKEN OFF OF HIS ANTIDEPRESSENTS....COLD TURKEY. MY BROTHER IS A MESS AND THIS PLACE IS JUST A WAREHOUSE FOR PEOPLE WITH MONEY. TOO BAD THEY WILL CONTINUE AS VERY FEW PEOPLE WILL TAKE THE TIME AND EFFORT TO PUT A STOP TO THE WAY THE PATIENTS ARE TREATED.

MY MOM IS A RESIDENT AT AFFINIITY AND I CAN TELL YOU I AM NOT HAPPY. THEY ARE UNDERSTAFFED AND THE WEEKEND IS WORSE. ALL MY MOM ASKED FOR IS SOME COLD WATER SEVERAL TIMES A DAY. FOR FIFTEEN THOUSAND DOLLARS A MONTH I THINK AT THE LEAST THEY CAN SUPPLY THAT. HER CLOTHER ARE ALWAYS MISSING EVEN THOUGH WE REQUESTED THAT THEY DON'T DO HER LAUNDRY, WE ARE THERE EVERY OTHER DAY AND TAKE IT HOME. IT SEEMS THE HELP IS TALKING ABOUT A STRIKE AND A LOT OF PATIENTS WERE VERY UPSET TODAY. IF I DON'T HEAR FROM THE ADMINISTRTOR I WILL BE CALLING THE DEPARTMENT OF STATE.

When I read the reviews ,I was worried that my Dad was going in for sub acute rehab. But I have to say the nurses have been wonderful. The pt department is great and is more than willing to discuss his progress. The aides have been attentive and social worker was very informative. I am there at least every other day for 3 weeks and really have not seen very little to complain about. I heard new management is has been trying to make improvements. And they have. It feels good to leave knowing my Dad has been in good hands.

My father-in-law was at Affinity Skilled Living. It was beautiful, with big, wide hallways and a nice courtyard. The people were nice there. They're very busy in a place like that. The rooms were very clean.

Because Affinity Nursing Home was only eight years old, we thought it was modern, so we placed my husband there. However, the family didn't like it very much. I wasn't impressed with this facility at all. They weren't that bad, they were just okay. It also wasn't that clean. The staff was unimpressive. They had problem with the organization and there wasn't enough staff.

the place is clean and new...But the staff are terrible. Very lazy staff..The get annoyed to get you a drink of water..Took my dad out with peneumonia..they didn't even tell us he was sick..They yell at the patients...please don't go there....

I visited this facility

Of all the facilities I toured, Affinity seemed to be the most hygenic and clean. The physical therapy department was impressive, and the nursing staff seemed very qualified and caring. We had a sit down meeting with the rehabilitation unit and they assured us that our loved one's needs would be met, if not surpassed. The recreation department also seemed very active and involved with the residents. Many of the activities they provided matched the interests of our loved one. All in all, Affinity skilled living was higher on the possibility list. We also sat down and spoke with the fiscal team. They were very clear and concise. The social worker informed us that she would advocate for our loved one and apply for medicaid/medicare, which would alleviate some of the expenses from our family. Many of the staff seemed genuinely concerned for the care of our loved one, and were open to our many questions and concerns.
