Elysian has two Care & Wellness Assistants staffed around the clock.
Our residents are assessed by an RN every 90 days or with a change in condition. We have a rounding provider that comes on site every other week or by special request.
We adapt menu items to meet residents changing needs. Texture and color play a large role in what is served. We offer feeding assistance, altered diets, and an "always available" menu.
Families are contacted if there is a change in condition, new medication, or if the nurse identifies a need that is not being met. More formally, care conferences are offered every six months. Care conferences can also be held at the request of Elysian or the family in addition to scheduled care conferences.
Elysian Senior Homes of Northfield is located in a quiet, residential neighborhood.
We allow families to choose the hospital system they desire for their loved one to meet any acute care needs. Northfield Hospital is our closest hospital. However, our nurses are able to coordinate care for our residents with any organization.

We never fall into a pattern with our activities. Our home was designed to be accessible as well as aesthetically pleasing. Each resident has a private bedroom and the option for private or semi-private bath. Residents can enjoy common areas that promote inclusiveness as well as areas specially designed for quiet.
