SeniorLinks provides Geriatric Care Management with the goal of safety, wellbeing and the highest level of independence possible for our clients. Our experienced professional staff offers companionate high quality care to our clients. We thoroughly assess our clients to identify the best plan for them. Our team approach to care insures that there will always be someone available to assist you or your loved one with any issue that may arise. - Advocating for clients needs in any setting

- Coordinating/management of services in the home or in a facility

- Counseling
- Placement- coordination of all aspects from selection to moving
- Monitoring of health and well-being
- Serving as POA/Guardian
