Elder Keepers, LLC

Elder Keepers, LLC is a private Geriatric Care Management Service located in Canton, MA. We serve select towns in the following counties: Essex, Middlesex, Norfolk, Plymouth, and Suffolk. Our mission is to support the unique needs of each senior. We pair nurse practitioners and registered nurses that are skilled in all aspects of care management with seniors and their caregivers. We monitor day to day health care needs of clients, and provide customized support to seniors and their loved ones. Our goal is that seniors are able to play a vital role in decision making, achieve good health, and remain part of the community, at home or in a setting of their choosing. We recognize that we all do best when we’ve had a say in our own health, and this extends to seniors. We provide the following supports: Appointments: We accompany seniors to appointments and coordinate scheduling and tracking of appointments. Care Coordination and Planning: We serve as a bridge between seniors and health care providers, conduct medical review, provide inpatient, transition, and discharge support. Client Advocacy: We help to ensure that seniors needs are heard and understood. We help to ensure that seniors and their loved ones understand the full depth of aging, healthy and otherwise, and support decision making throughout the lifespan.
