The average age of our resident population is 79. Some of their favorite things to do include, socializing with family and friends, playing bingo or getting together for a game of cards. Whether its enjoying a good book, bird watching or taking a stroll in our beautiful courtyard we encourage individuality.

We encourage our residents to invite family and friends to come join us. The turnout is great and a good time is had by all!

Upon admission your loved one will receive a comprehensive assessment which helps to create a individual service plan for person-centered care that is delivered by well trained staff. The ISP is a valuable tool that we use on a daily basis while monitoring for any change in condition. Our facility has also teamed up with Transitions at home which offer the support of a registered nurse that visits bi-weekly and is available 24 hours a day to ask questions and address concerns.

Residences On Forest Lane is a comfortable place with a home-like atmosphere where compassionate care is our promise, and independence is encouraged.
Built in 2010, this new and updated community is owned by Wisconsin Illinois Senior Housing Inc. with whom we share the mission of “serving elderly and disabled individuals by providing quality housing, health care and other services to enhance the lives of those we serve.”
Our beautiful 16-bed CBRF community is located on a wooded and spacious 5-acre campus in Montello, WI, which we share with our sister facility, Montello Care Center. The benefit of this full-service campus is that our residents can receive outpatient rehab therapy and nursing care as needed right next door.
All individual and double suites have a private bath and living area. We are pleased to offer a cozy atmosphere where residents and families feel comfortable and at home. Our community is best suited to seniors who appreciate an independent living environment with some assistance needed for daily tasks.
