Our Team loves the opportunity to talk with families and keep them updated on our their loved one's day.  We utilize phone, SKYPE, ZOOM and a variety of communication tools to keep them informed and connected.

Our home is located just off North Port Washington Road and blocks from shopping and restaurants.

We pride ourselves in having a high caregiver to resident ratio that is planned based on the needs of the residents within the home.  In addition, to our on-the-floor caregivers we also have a variety of other support team trained to care and engage with our residents too.  This is why you might see our Chef in his hat dancing the polka or our Housekeeper helping a resident in the garden.

Our Azura Team receives over 115 hours of training within their first year of joining the Azura family.  This includes learning the specialized techniques of our MOSAIC Philosophy and how we have used the Household Model to enhance the home and care for all.

Is Azura Assisted Living and Memory Care of Fox Point affiliated with a hospital if more care is needed?

Accepted Answer

Azura is not affiliated with a hospital, but does have a Medical Director to oversee our residents care. In addition, we work a visiting physicians service, podiatrist, dentist, optometrist, home health, hospice and offer on-site physical, occupational and speech therapy.

All of our menus are overseen by Dieticians to ensure they are nutritious and delicious! We also work to incorporate our residents favorite foods, beverages and treats!

We offer a robust activity program that is tailored based on what each resident likes and dislikes are.  As a result, our offerings are constantly changing, but some of our staple programs include Harley's Heroes for the Harley-Davidson enthusiast, Green Thumbs for our resident gardeners and Songbirds for those residents who love to sing the day away.

This includes learning the specialized techniques of our MOSAIC Philosophy and how we have used the Household Model to enhance the home and care for all.  In addition, we use the internationally known Virtual Dementia Tour to ensure our Team truly understands what it feels like to have dementia and gain empathy and understanding of the disease and how they can best assist our residents.

Who assesses residents' health and cognitive functioning? How often is that assessment repeated?

Accepted Answer

Our residents are assessed by our Executive Director, Registered Nurse, House Manager and Team on a daily basis with changes or concerns documented in our electronic charting system.  More comprehensive assessments are done on a quarterly basis, unless a significant change of condition occurs.

At Azura we engage our residents in Chore Therapy and encourage them to be a nurturing part of our home.  As a result, residents may help with baking dessert, setting the table or simply enjoying the meal.  We use our MOSAIC Philosophy to enlighten our Team on ways to engage residents to eat, converse and enjoy themselves just as they would at home. In addition, based on a study done by Boston University, we use red plates and bowls, which have been proven to increase appetite and intake for those with dementia.

Our residents at Azura Assisted Living homes are 55 years of age or older and enjoy all of the normal activities of life.  Their goal is to live in safe, supported and engaged home that sparks their smiles of joy every day!

Residents at Azura participate in a variety of personalized activities and outings that are tailored based on their personal history and their likes and dislikes.  We have different Clubs or groups that exercise, garden, bake and laugh together daily.  In addition, we also provide individual supports to those who prefer more individualized or solitary time.  Our goal to ensure our residents have the supports they need to feel empowered to live their lives in the highest quality that they desire.

One of the enhanced features of our Assisted Living homes are the safety features planned throughout.  Each home is equipped with the state-of-the-art RCare Safety and Alarm system, Age-in-Place Bathrooms, and more!

Our internal community is further enhanced by our on-site Community Gardens, produce stand, 100  year-old landmark barn, bocce ball court and putting green. Plus our residents love to get out into the community taking trips to museums, theaters, casino's and more!

Azura has a wheelchair accessible van that is free for residents to use to attend schedule social or medical appointments.

if a resident in our Assisted Living home begins to wander our RCare Safety and Alarm system will help support them. However, we also provide transition service for residents who need more supports, which can be easily obtained in our EXCEPTIONAL Memory Care homes located right next door.

Our Team welcomes the opportunity to talk with residents and their families.  One tool we use is the Sagely Engagement Program, which tracks the activities each resident is participating.
