Our Geriatric Case Management provides your elderly family complete with providing an in-depth assessment, link advocate to put services in place. Geriatric Care Managers not only help coordinate moves, but they can also help with planning the downsizing, and moving seniors into their new homes. Geriatric Care Management services provide families who live at a distance or locally and want an advocate for their loved one. In-home assessments are most often requested to determine short and long-term care goals, identify resources to maximize independence and improve quality of life. This includes addressing activities of daily living, financial matters and environmental concerns. Angels Among Us Services Inc will act as a liaison with family members to ensure safety, alert them to any potential problems and take a proactive approach towards implementing any changes for the benefit of your loved one. Advocacy, caregiver support, counseling for individuals and families. Along with education and ongoing monitoring, we can also implement coordination of moving into an assisted living or memory care.
