About CenterLight Healthcare Pace Center in Bronx, New York
Welcome to CenterLight Healthcare Pace Center! At CenterLight Healthcare, our goal is to provide individualized care that enriches the lives of those we serve and helps them live safely in their homes. One of the things that makes our program so special is that our care is delivered by a team of doctors, nurses, physical therapists, nutritionists, and other professionals dedicated to our participants' health and well-being. With this in mind, we are proud to introduce a new name for our Program of All-Inclusive Care for the Elderly (PACE) plan: CenterLight TeamCare. CenterLight TeamCare remains the exact same PACE plan that our participants have always enjoyed, but now with a reinvigorated and refocused theme: to provide "care as unique as you are." This new plan name symbolizes our renewed commitment to work as a team, with each other, our providers, community partners, our participants and their families to be able to help those we serve live their best lives.
