PACE: A Program of All-Inclusive Care for the Elderly is a community-based long-term care option that allows frail older adults to safely remain living in the community. PACE provides all services that would normally be covered by Medicaid and Medicare, uniquely customized to each participant’s circumstance. Who is Eligible? Participants can join PACE if they meet the following criteria. 55 years of age or older Live in Monmouth County or Ocean County Needs assistance with basic care needs Able to live safely in the community with the help of PACE services (Enrollment is effective at the first of each month. The participant may dis-enroll at any time) List of Services: Primary Care Services Social Services Recreational Services Adult Social Center Therapy Services Nutritional Services/Meals Prescription Drugs Medical Transportation, and more
