Geriatric Care Management service provides assessment,consultation, and care planning with elders and their families to select, coordinate, and manage elder care solutions. Advocates for clients to maintain highest level of health, independence, and safety as possible, whether in the home or a facility. Works with all involved to educate, facilitate, and oversee programs and services as needed.
