Should Mom have 24-hour monitoring after hip replacement surgery?

A fellow caregiver asked...

Mom broke her hip, had replacement surgery and a month later had two blood clots. She's released from the hospital back to a rehab facility. Here's the problem: she is severely confused when waking. She is a fall risk for the hip and the blood thinners. When she wakes, she thinks she is home and tries to get up to go to the bathroom or do things. She is not to walk unassisted. We are trying to arrange family sitters to keep an eye on her until she becomes oriented to her space again. Is there anything that can help this process? She has no cognitive impairment. After she is awake for a longer time period, she is sharp as a tack. Further, when she is discharged from rehab, she will still be on Coumadin. Should she still have 24 hour supervision? Thanks for your advice.
