I am/was a resident of this facility

this place is really old and decrepit with non-educated people working there. If I could, I would rate this place a zero! I will not recommend this place to anyone who won’t quality healthcare. If anyone recommend this place, they do not care for their family members.

I am a friend or relative of a current/past resident

On 10/30/2023 Banner Hospital discharged my dad to Sabino Canyon Rehabilitation Center. When he left via an ambulance due to what we found out was a colon blockage which backed his whole GI system up which in turn aspirated into over filled into his lungs also making him turn into Sepsis. Details below. When my step mom and I arrived the next morning of his arrival we were greater by many staff members who appeared to be on top of things. Well not long after the arrival things started to go down hill. From the very beginning (day 1) we told the center that my dad has a severe colon issue & must have a BM every day or at the very least every other day. Everyday either myself or my stepmom would question if he had a movement and or been given something to make him have a BM. Of coarse we were told no or I don't know. So we took matters into our own hands & started bringing prunes & other high fiber movement aides. Not only were the BM's a issue but the lack of hygiene (showers) a issue but lack of staff coverage. My dad went 10 days without a shower (my brother & I) gave him his first shower and I gave him the last 2 during his stay. He had 3 showers & none of them were given by the staff on the "days" that were assignedto him and or his bed number. I got assistance from 1 CNA on his 10th (1st shower) day. This facility is need of a update. The diner section is so dark it is depressing. There is a TV in there but it doesn't work & the staff don't know how to use it. They use the middle nursing station as the babysitter for "problem" patients. I get that some patients need a "babysitter" then someone needs to stay at that location to insure they are warm, comfortable & SAFE! They lower the beds almost to the floor for safety. Great idea because like in our case my dad "fell" out of bed 3 or 4 times so no problems were detected at that time however, when they put patients at the nurses station (again someone needs to watch them) because my dad tried to stand up & fell. We were called about 2 of his falls. 1 out of bed & 1 from the wheelchair. No others were reported. Now for Pysical Therapy. Oh I can't say enough good things about it. You can tell they really care & go over & above to help the patient & the family. I give 5 stars for them. The patio another 5 stars - what a nice place to hang out. My step mom & I came everyday for a minimum of 4.5 hours a day. To which we spent most of it outside. My dad was very content out there. Therapy will bring patients out there to do their therapy session there. We believe that fresh air & sunshine is 1 key to recovery. The doctor assigned to my dad didn't heed our warning & plees about his pain & lack of BM movement. I would take videos & show the doctor as well as the RN & ask them to give him SOMETHING for the pain as well as things to move his bowels. They would give him Tylenol and either an enema or miralax concoction. They would only give the Tylenol when we requested it & not on a schedule. MY DAD is now on comfort care hospice due to the downfall of his condition. Though we were there during the day for hours the lack of this facilities actions has exasperated my dad's condition as well as additional health challenges. Would I ever allow my family members to go there that answer is no & if I had no other choice I would stay there longer and or come and go morning, day & night.

WORST PLACE EVER . RUDE NURSES . DIRTY, GRIMY . UNEDUCATED PEOPLE WITH NO COMPASSION AT ALL . WHILE VISITING MY MOTHER THERE, I SAW MANY PATIENTS BEGGING FOR HELP AND ATTENTION. WHEN I WENT TO THE DESK TO ASK A NURSE FOR A CUP OF WATER FOR MY MOTHER, I OVERHEARD A NURSE TALKING ABOUT MY MOTHER AS IF SHE WAS IN THE STREETS HAVING A DISPUTE WITH SOMEONE. SO UNPROFESSIONAL . I TRULY BELIEVE THEY JUST HIRE ANYBODY OFF OF THE STREET. I ALSO BLAME THIS HORRIBLE PLACE FOR THE DECLINE OF MY MOTHER. I FEEL SO SORRY FOR ANYONE WHO HAS TO STAY THERE FOR ANY TYPE OF CARE. IF I COULD I WOULD GIVE THIS DISGUSTING PLACE NO STARS. I TRULY HOPE THIS PLACE GETS SHUT DOWN. I AM A DISPATCHER FOR A CAB COMPANY, AND HAD A CALL FROM A DESPERATE PATIENT THERE BEGGING ME TO SEND A CAB TO PICK HIM UP AND GET HIM OUT OF THERE. I CANNOT EVEN STAND TO LOOK AT THIS PLACE WHEN DRIVING BY.

I would give this place a zero if I could. The place is dark and old. They have left my father to decline. All the while saying he was doing rehab. Not true. Staff won't give a clear answer to questions. Absolutely the worst facility I've ever seen.

The staff is extremely caring and always made the time to care for the patient needs. The facility is a little older but the care that was provided, well made up for it. The therapy department is wonderful and helps patients get back on their feet so they can go back home. They have an activities department that runs games, social times, and various other activities to help pass the time and get patient's out of the room. Beautiful dining room and courtyard. Doctors come to the facility daily. 10/10 would recommend. Nice 5 star facility.

The rooms are depressing looking, dark and gloomy. Some nurses are helpful until its time for them to go home. Alot of the staff do not know how to handle patients, do not look or ask why they are there and expect them to get up or be able to turn over when due to their injuries they are not able to. They do not do patient verification when giving medication, and try to give my Mother someone else meds, and because it was time for the nurse to go home refused to take my mother to the restroom

If I could rate this rehab facility a zero, I would. My father came home with Covid after they refused to test him. He ended up right back in the hospital where he spent the next 3 months! He could not perform any of the functions on his P/T report. His home nurse said he should have been sent to the hospital. My phone calls were not returned. Visitors were not allowed and it seemed like they took advantage of that to neglect the patients. After they called to report Covid was present in the facility, no one called back with answers to my questions. We received a $2000 bill from a doctor that we never heard of. T.E.R.R.I.B.L.E.

We have my mother at Sabino Canyon Rehabilitation & Care Center. It's a combination of a rehab center and a nursing home, and they do physical therapy and occupational therapy. The staff is bilingual, so there's always someone who can talk to her because she's Spanish speaking only. The grounds are awesome. The facility is huge, laid out real nice, and it has a nice courtyard. The therapy room has a lot of equipment to help folks learn to stand or walk. They have a dining room. They have a full staff, and I'm really impressed. The therapist is professional. They always have at least four or five nurses on hand. The value for the money is excellent.

Sabino Canyon Rehab is not really a dementia or Alzheimer's facility, so my mother didn't get the best care while she was there for two weeks. The facility was nice. It's not the worst I've seen, but it's not the best either. The place was clean, but old.

I am/was a resident of this facility

I recently got discharged from Sabino Canyon. When I arrived I couldn't walk couldn't talk had absolute no use of my left arm whatsoever from being mauled by a vicious dog. I was so impressed with the pair the love that compassion the kindness the unity of all the employees. But most of all the dedication that was shown to me and the uplifting happy positive energy I received from housekeeping all the way up to the administrator. The doctors and surgeons informed me that I would probably be there for about 3 months doing physical therapy before I would even be close to being able to walk or use my arm. I was there two weeks and I walked out using a cane talking just fine and having the knowledge of body mechanics that they taught me that I will use the rest of my life. I went from 0 use of my arm to 50% use of my arm thank you Sabino Canyon. In conclusion I accumulated a wonderful little family there with other residents as other residents families and employees. Nurse CLIFF, Cna-Yvonne, Occupational Therapist Marsha thank you all for believing in me when I didn't believe in myself. The positive uplifting happy energy there was my medicine thank you Canyon I will never forget any of you and thank you Ernesto from the kitchen for your kindness and compassion and respect.

I can not speak for the care, so will say it was average. My mom was there and I live out of state. All I know is it was next to impossible to get anyone to return my calls. That tells me a lot. I do not expect all calls to be returned same day. But by end of following day, for sure. One person returned my call after my mom was released. I was just in rehab myself, and my husbands calls were always returned same day.

My friend was moved here after a brief horrible stay at a nursing home behind St. Joseph's Hospital. Big, big upgrade in all phases of her care. We are very happy with Sabino Canyon!

I got really good care from everyone at Sabino Canyon. Very friendly and caring people. The place is kept very clean and the nurses and nurse aides were so caring. They worked very hard to take good care of me and I am forever grateful.

I visited this facility

It was a pretty nice facility, however the building was somewhat older. It had no central air conditioning in the rooms. Portable units were installed. The nurses seemed kind and professional. The particular facility also had a dementia section for residents with dementia. It was a locked portion of the facility to prevent wandering. I felt the facility was well maintained and in good working order. The staff, aside from the nurses, was kind and professional. They seemed to be generally concerned with the patients' well being. All employees seemed to know resident names and held meaningful conversations with them in the halls etc. The resident rooms had 2 beds per room. The bathrooms seemed somewhat small for 2 residents to share. The activity room seemed to be active with the activity coordinator directing the residents in bingo or some other fun activity. The facility seemed to be an overall nice place.

At this facility, the staff members were very caring and nice, though the actual facility itself was kind of old and run down. There are different types of activities that go on here, including Bingo, movie nights, transport to outside retail shops and an onsite beauty salon, so there is a lot for people to do. But if you are confined to the bed, then you don't have a lot of options. The rooms are very basic and there are not a lot of frills included, but meals are provided three times a day and nurses do come check on your quite often. Overall, this facility was adequate but did not offer anything over and above some of the other rehab/nursing facilities in the area.
