What options are there for the transition into a care facility?

When a loved one has need for full time care facility, what options are there for making the transition? I hear stories that facilities dictate what I call a 'dump and run' policy, leaving the patient often agitated and confused at being left (constantly asking when they will go home etc.) what are the benefits to leaving them for an 'adjustment' period and what are other alternatives to advocate for if this is not a fit for you?
