Best Facility for Parkinson's Patient

It seems assisted living with memory care does not offer the kind of everyday physical support mom needs and skilled nursing homes are the only option. She requires assistance with all transfers from bed to wheelchair and toilet, help with all dressing and grooming. She is not incontinent if some one comes to assist her when she needs it but I am concerned that she will become so if she has to wait long. We have in home care for her but I wonder if a facility would provide more service, activities and rehab. She is 89 and has had Parkinson's for about 2 years, but seems to have late stage symptoms including hallucinations, freezing etc. She is very depressed. How do I find the best place for her?
