The sheets were never changed after 3 weeks and me asking them 3 times their food was the worst I have ever ate club sandwich was shredded cheese and shredded ham on a hot dog bun I was not taken off morphine properly ended back in hospital the morning her nurse ignored my telling her I was sick the night nurse checked my was very high I've been out for 2 weeks they still are getting my meds from my pharmacy so I'm unable to get them when they were called they said oh we thought she was still here

The rooms are very small and not as clean as you would like it to be but they are very understaffed. the nursing staff is very small and not what you would think that a good quality facility would have. TERRIBLE. You would not put a loved one in this inviromemt. It is just disgusting.There are no amenities at all to look forward to. On occasions they may play bingo and that's it Food is terrible and always cold. Not even what you would think it would be.

I have toured Elms Haven Care and Rehabilitation. It's a quite large community. The people were very friendly. The lady took me on a tour, and everything looked very clean. The atmosphere was very nice and very friendly. The rooms were very clean and very open. They're nice-looking rooms. They offered beds and dressers. You do have to bring your own TV, but they have closets, and it's hardwood floors. Each area has its own eating area. If you're in the rehab place, you have your own eating place; if you're in skilled nursing, you have your own eating place; and if you're in memory care, you have your own eating place. Everybody was extremely friendly. It was open and airy, and there was nothing dingy or dark or anything like that, so that was nice. The dining area was very open. It wasn't like in a room or anything like that. I could see it when I walked in the door. It looked very clean. They were eating at the time that I came. The person who gave me the tour was friendly and outgoing. I liked her.

I hate the fact that I have to give you a 1 star rating, you deserve a negative star rating. Due to the lying, negligence, and the horrible communication with staff, and administration. My mother didn't deserve this. She was admitted due to a stroke; she was paralyzed on one side of her body and blind in one eye. She was at Elms Haven for Rehab. The lying started when we called for information regarding admission. We were told Elms Haven had the Covid vaccination on hand and would be administered to my mom upon admission, and that we could visit her the next day. However, she was never vaccinated during her 11 day stay, and thus quarrantined for up to 14 days. But she didn't live that long. We had to beg for "compassion visits" through her window from the outside. Her many get well cards were left on the tray table unopened. On day 11 of her stay she was sent out to the hospital for what was called "no big deal" by the nurse because her labs were askew. When in reality she had suffered a second stroke, had a massive UTI and had gone septic. She died the next day. If you are looking for quality compassionate care for your loved one do not choose Elms Haven!!

This place is terrible. They neglect the people in their care. We had a loved one there for 11 days and she was still in the same dirty hospital gown she came in when she left. The staff is very unprofessional would hang up on us when we would call for an update. None of the employees know how their rules work. So expect lies from anyone you talk to. We are very sad for anyone who is currently there. If you read reviews choose somewhere else.

Unlike others who may comment, I actually live here at Elm's Haven. Despite my initial reluctance to make this life-saving choice and relinquish my former independence, I have been relieved (and very surprised) to find that I have gained much more than I have lost. The staff has become like my second family! Their compassion is genuine and heart-felt. No other human quality is so very important as this. I've also received skilled and thorough medical care that has improved my physical condition substantially. The food is much better than I expected and even during this CoVid time, there are engaging activities offered. I am fortunate to receive such skilled and caring help!

My sister has been a resident here since march..started out well food was great..last few months..she has missing clothes..food horrible..now with "covid" no visitors...left in their rooms to eat etc...patients very depressed..they tested and tested my sister and her room mate until finally both tested positive ..right a way they move/moving them to another facility..because they want NO covid here...so you ship them off to spread or actually catch it? You are not letting them come back are you? They basically leave with clothes on their backs...and many like my sister hung treasures on walls to make it more homey...now they have to leave those too...you are definitely in it for the money!! These peoples families need a investigation in to how you running this "joint"

I was forced to move my dad out of the hospital 2 days after surgery and only had a couple hours notice. I went to Elms Haven and spoke with the director who gave me a tour of the rehab unit where my dad would be and really sold me on everything. It was not the same when we arrived a few hours later. Very understaffed, did not have pain medication ready for several hours, did not have insulin, we had to bring from home. No running water in room, air conditioning didn’t work, very hot and stuffy. My dad hated it here. They never cleaned and checked his incision which in turn got badly infected here. The staff knew his white blood cell count was high on Wednesday and let him lay till Friday. When I arrived the nurse said he was very tired, it was a far cry from being tired he could not feed his self, answer the phone barely even talk. I ask for an ambulance to be called and have him moved to the hospital, they wanted to use there own transport we waited for 45 minutes, not once did someone come in to check on him. They finally called an ambulance, when he got to the hospital they told us he wouldn’t make it through the night, fortunately he did and had emergency surgery to clean out infection. When they did the bloodwork at the hospital they also found out he had MRSA. He had only one shower in the week he was there. He ultimately died a week after removed from this place. He was healthy lived on his own, took care of farm animals daily, and lost his life due to the negligence of this place.
