How Do You Know When to Start Making Plans For End of Life?

my mom has end stage COPD, she is in the hospital right now having exacerbation nearly everyday, she was on a Bipap machine for a day and a half but was able to come off. She has right sided heart failure and each time we check in the hospital they are worried about kidney function. She has two compression fractures in her back and we have no idea how. Her weight has gone down considerably and I notice that she get into these clammy sweats a lot. This time while at the hospital they have taken off 9lbs of fluid so far. are we about to loose this battle
