What do I do in the following situation?

My friends dad recently had a stroke hospital says he needs a care taker she herself is disabled and is saying she takes care of him but he hasn't had a bath in 6 months hasn't had his meds regularly in two years only eats twice a day and they have 23 chihuahuas that play outside n go to the bathroom inside everywhere the bathroom sinks r not hooked up they have a bucket to catch dirty water house itself is falling a part and sewer for that house is up against house (actually touches it ) when they pump it out they pump it further into same yard were the have food growing and their water is under that same ground well water back yard is a junk pile of stuff also has over 30 chickens 6 ducks n 8 geese last counted I am very concerned for all three people living there they constantly r having health issues what do I do with out offending them
