My wife is currently in Room 407-B, and she is not getting the best health care compared with being in the hospital. She has a bed sore, it hurts, but do the nurses give her pain pills and/or cream when needed? No, figure every 6 hours when the pain hits every 4. At least once, while she paged the nurses and waited in pain, she could hear the nurses discussing non-nursing stuff (aka lazy). Does she get a drink with her meals or when needed? Occasionally. Since she got there a week ago, she is not allowed visitors for another week (14 day quarantine due to Coronavirus prevention), which is not a complaint, but when I bring stuff to her, they first tell her there is nothing for her, then bring the box to her a few hours later. Three times a week, she is scheduled to go to DaVita for Dialysis (call it a monopoly and my insurance calls Davita out-of-network), they don't get her ready to go until maybe a half hour before the bus goes, meaning they're hurrying to get her ready. When she returns from Dialysis, her food is cold. She also gets PT, which is fine, except one of her therapists called her "evil" and yelled at her, the nurses said "oh, she's 7 months pregnant" (bad excuse for harassment). If the management wants to fix things, I will not talk to you, you shall go talk to her and FIX IT! If we could go to a better rehab hospital, we would, but Aetna (Group Insurance) said this is the only one they would pay for. This past week, those good-for-nothing staff lost her blanket, which is a family heirloom made by her late mother. Tonight? THEY DID NOT GIVE HER DINNER! So, do I recommend? Not one single bit!

I am unable to visit my mom so I tried to call her and was unable to talk to her because “she doesn’t have a phone” and “we are working on the phone in her room”. After three days of this I have called and left several messages for the executive director who has yet to call me back. I have spoken to a front office person who has assured me she isn’t on vacation and “she gets to you when she gets to you”. They pay this person 120,000 I believe she has circulated and yet she can’t answer a phone call. I am hoping she will leave and they will get the leadership they need per the four different nurses I have spoke with she doesn’t help them she is never there and she doesn’t care about the place. I’m mad because I have also been told by nurses when I call my mother “is one of those difficult patients and we have to force her to take medications”. If you force my mom to take a med I will lose it I told the nurse. She is only there for short term but I hope and pray she gets out soon. She hates it there. I wouldn’t write this but I’m very upset with the fact the executive director has not acknowledged a phone call and I have never met this Trisha person. Oh well after reading other negative comments on her it just seems to be swept under the door like the rest of the comments.

Because of the lock down caused by the Covid19 I was not able to visit my husband during his two week stay. When trying to get any type of information from his assigned phone number all the way to who distributes his laundry to the reception area so I could pick it up was a terrible experience. The receptionist could not give me an answer and would say, "I'm just doing my job." When I finally got to talk to a social worker I told her my husband wasn't answering the extension he was given. She checked and he didn't have a phone in the room and would get someone to put it in. When I called later I was sarcastically told that ALL ROOMS HAVE PHONES, DID YOU KNOW THAT? When I said that when I talked to the social worker she said he didn't have a phone, did you know that? He was given a shower the second day of his arrival and never got another one and was there for two weeks. He had been given wash clothes up to a point and had to use paper towels to wash. In order for me to get his dirty laundry without going through all the sarcastic remarks these people hand out he would get in his wheel chair and bring it up to the front. He was overlooked on many requests and was only given the small pitcher of water once a day. I could go on but I am running out of room. The therapists and RNs were great and they are the only ones. Terrible PR.

The doctors barely visited or informed myself or my family. The medication nurse admitted giving me less of an important medication to improve my liver, because "she didn't think it was necessary". Kept there until my insurance ran out and sent home without oxygen I was on 24hrs a day wasn't ordered. Got 2 showers in 2 months. Missed meals because they ran out of food. Anyone was able to come or leave the site without signing. They didn't even know I was gone. Physical and occupational therapist were great. Food was ok when it was there. Had no TV (broken). No phone either I was told to move the bed and jiggle the phone Jack to make it work. Human services and the director or manager were a joke and quite rude. Overall met some nice residents, nurses aides, but being that I died of septic shock with all organ failure thought I should have gotten a little more help than I did learning to walk, talk, eat, etc.

I have a relative there right now. They don’t even deserve 1 star, but you can’t put zero. I understand their no visiting policy due to corona virus right now but they have been totally unresponsive to my relatives needs. On the phone (If you can get anyone) they give you stock answers and even basic dietary needs are totally ignored. If I had money to pay - my brother would be taken out of there this very minute. I hope God can keep him alive in there!!

Concerned Health Care Worker

We have visited our family member regularly. In the beginning things seemed OK, but then we noticed that the staff were rarely getting our family member out of bed. We had to request that they get the person up and dresses each day. You would think this would be a standard. Then we began to notice how dirty and urine smelling our relative was. We asked about showers and were told that if the resident didn't want one then they didn't give them one. This is ludicrous....how can they not smell the urine all over the guest and/or see the greasy hair. What makes it even worse is that we have met with the "care team" on at least 3 occasions and not once have our voiced concerns been noted in the patient record. We know this because we asked the nurses to check. The patient is overweight, lethargic ( left sitting in a wheelchair with no attempts to have the person walk a little each day with a walker and the individual CAN walk, though if left in a chair this will soon change due to lack of exercise). It should not be left up to a patient with dementia to decide whether or not to eat, shower, or exercise....healthcare should be standard even for the elderly. Nurses and CNA's can do more than congregate at the central desk and chat. The people are there because they need attention and care. Giving it another month or so and then contacting AHCHA and Medicare to have them do an official investigation

this facility is horrible. I wouldn't recommend this place to anyone.The staff is unconcerned as well as the business office.Never a returned call no quality treatment. For over $10,000 per month you would think you would get better people. My mother in law died there a few weeks ago and was never treated for a disease she got while in this place.go somewhere else

The care provided by the nursing staff, Nursing Aides, and Administrative staff was excellent. There exists a truly caring concern and attitude toward the residents.

Grand Oaks was clean. There was plenty of staff around. I spoke with the people there. They were very nice, and they told me they will take them to the doctor's appointments. The place was clean. There were plenty of personnel around. I saw the activity room, and there was a lot of people in there playing games. It seemed very nice.

My husband spent 2 weeks here recovering his strength after an illness. I was very pleased with the care he was given & the staff was pleasant & caring. I encountered only one rude staff member & she was in the business area & not in contact with any of the patients. Also, I have to add that the food was good!

Grand Oaks under the new administration is wonderful. I was very pleasantly surprised at everything I saw and felt. There is a palpable positive feeling as soon as you enter through the front door. The staff is friendly and polite. The building is extremely clean and well maintained. The therapy staff is second to none. All in all I couldn't have been more pleased with everything. I would definitely give Grand Oaks another chance.

Coffee and TV are wonderful baby sitters. Coffee should be in an area for visitors to get for their loved ones at all times. To stop a nurse to get coffee is silly. TV; two TV's in a room. They are too far away from the head of the bed. No speakers in bed. Speakers are at the TV. Requiring the TV to be too loud. Drowning each other out. These two item jay seem trivial. They are not. They are the differenc between patients calling nurses and it adds to the depression of being in a facility.

I am so pleased with the new administration. The administrator and director of nursing are making great improvements to the center. It is nothing like it was in early to mid 2016. With these changes I have seen, and with those to come, this center will definitely be in a class by itself.

My family member is here now long term. And it's horrible. The staff yells to and fro down the hallways. Call lights go unanswered for Sooo long. The admin and director of nursing are a joke. Zero communication. Half the staff doesn't wear name tags. I'm trying my best to get my family out of here. Wouldn't bring a sick squirrel here. Zero stars.

My wife's mother was in Grand Oaks, and it was beautiful. It's the kind of place I want to be in. She had a two-bedroom that was kind of tight with the two beds. As far as the meals go, they were superb. They should have larger rooms.

my mom (85) has been at Grand Oaks for 4 months after a bad leg fracture. when she first arrived i would have to agree with some other reviewers, her roommate was loud, confused and prone to episodes of throwing things, the day nurse was very condescending, basically ignored patient and family and the aides didn't seem to care much at all (every request was "an effort" and one actually told my mom if she didn't like it here why doesn't she leave?" HOWEVER, after a room change, she had the best nurse ever (Deanna). ( i've been a nurse 40 yrs.. i know). this woman was on top of everything, healed up a lingering wound, addressed every concern promptly, was caring and supportive. The aides under her followed her lead and were very helpful and kind. The rehab PT (Marta) is OFF THE CHARTS good. she is just amazing!!!, always upbeat, encouraging..just goes that extra mile day after day. Mom had a couple physical set backs during her recovery and if it wasn't for the support she received at Grand Oaks things would not have gone as well as they did. Also have praise for house cleaning (floors shine, no odors), dietary that does their best and administration that addressed issues quickly and efficiently. i would highly recommend Grand Oaks.

DON'T SEND ANYONE TO GRAND OAKS! OH IT'S PRETTY BUT STAFF COULD CARE LESS AND WILL LIE ABOUT REQUESTS. FRIEND ASKED FOR HOT CHOC. - WAS TOLD NONE, I WALKD DOWN TO A DAY ROOM AND THERE WAS A LADY LOADING UP A CART WITH HOT CHOC. PACKAGES, ASKED FOR ONE BUT THESE WERE "ONLY" FOR THE COFFEE CLUB. NO LOUNGE CHAIR FOR THE PATIENT, HAD TO REQUEST ONE, TOOK FOREVER, NO CHAIR FOR THE WIFE TO SIT. WHEN ASKED TO CHANGE THE POSITION OF THE LOUNGE CHAIR WAS TOLD IT WAS TO HARD FOR ONE PERSON TO ADJUST, TOLIET WOULDN'T FLUSH IN ROOM, JUST TO MANY THINGS TO LIST!!!!!!

I am a relative of a former resident. STAY AWAY My mother dies 3 days after coming here, I got a TEXT with the facilities number no one had a clue, after 10 minutes of being transferred around I was told they found her bluish but didn't know how long she had been that way. And they called the paramedics, she was not supposed to have any liquids unless it was thickened. She died at the hospital,She aspirated as of a mistake from the staff. STAY AWY from this place looks nice but the esthetics are were it ends.

My mother died in this facility, she was pretty much ignored because she was unable to speak, but thankfully, I had family nearby to visit and to watch over her. Examples of issues included - no hand washing, my mom had food and dirt caked under her nails! the staff was using a scrub cleanser on her and tearing her skin, and she had a little tiny pillow and scratchy blankets. It was so sad. The only positive thing there was the events, bingo, music, etc. If your parent or loved one can fend for themselves, this place is fine, otherwise, look elsewhere.

I had a family member in the care of this facility and it's staff. They neglected the needs of my family member. She needed help, was in pain, needed fresh linen and many more things. Nothing was done. Yet all the staff seem to have nothing to do, but sit around talking. This is not the place to take a friend , family member, or even your loving pet. I have never witnessed such a lack of caring or such laziness at any other medical facility. I was very disappointed with the whole ordeal. They made a great appearance when I first went there, but there was absolutely no follow through. The complete staff needs to be retrained or maybe even trained to begin with! hey get no stars at all.

My mother was only there for 3 months, but if I could do it over again, I would never have put her in a nursing home. It is so sad that such a nice place offers so little individual care for the residents in the last stages of their lives. No one ever asked the family about special needs of their loved ones. We found her one morning with the air conditioner at 67 and she was dressed in her lightest garments even though she had warner clothing. I requested she be given water because the doctor said she was dehydrated, and the response was that water was put on her tray, but she was blind.

Right now my father in law is in a rehab/nursing home at Grand Oaks Health and Rehab Center. The physical therapy department is excellent. If someone needs to have a very good PT or occupational therapy, this is actually the best facility. My father-in-law is a very unsocial person and we've been trying to get him to socialize. We also hear him grumble all the time and complain about the food. It's not that great at all. I had been to their dining room which they also use as a place for their activities. They have a very good method of recourse like we have family meetings with all the head of the department very so often and that's good. I think Grand Oaks is good for long term care and also if you have someone who maybe have had a stroke and is trying rehab, but I think their LPNs need to be a little sharper.

My experience with Grand Oaks Health and Rehabilitation Center is appreciated and thankful but at the same time it was frustrating so I guess it is best to say that it is bitter sweet. The staff at times was helpful but not all of them were as helpful as you would want them to be. We tend to befriend three nurses that we completely loved that treated my grandmother like family. I think that is what you want out of a Nurse who is tended for a love one. These employees will constantly check on my grandmother to see if a change of sheets or undergarments were needed. They always made sure to acknowledge and celebrate her birthday and treated my grandfather with so much compassion who was hurting so much to see his wife. I liked how the facility had a monthly calendar that listed events such as bingo, piano night, comedy night, etc. These events were very cool in providing the patient excitement to help make them give somewhat of a notion that life isn't over for them. There is still time to live, laugh, and enjoy. I think that is important. The facility had nice and peaceful areas outside that you can take your love one for a short walk. It was frustrating in dealing with the doctors and nurses that treated the patients as if it was the end of the road for them. I felt like the doctors cared less more than the nurses which is why I gave Grand Oaks Health and Rehabilitation Center 4 stars out 5. To end on a positive note, when my Grandmother passed away it brought me to tears for my grandfather to see the support of the nurses that I attended the funeral. That support will never be forgotten.
