I visited this facility
My mother was in there close to a year . During that time she had fallen out of bed 3 times.. They found her laying on the floor at 2:00 am she had hit her head, and was taken to the hospital... Nurses station was only 2 doors down...went to visit her one day she was in the hall with vomit all over her...went to visit on another occasion her had was all swollen and wrapped up...but they couldn't tell me what happened to her...this is a woman who went in there. Could walk,talk,feed herself go to bathroom alone.... (My sister had her put there). Went to visit her on a mon... On Tues. Morning got a phone call she had passed away....they put Down heart failure, she had never had heart trouble! They said a nurse left her room and when she came back my mother was gone! To this day we don't know what really happened. We asked what her routine was from when I left her on Mon. Until she passed away on Tues... Paid $25.00 to see her records, but still no information to explain anything....

I am a friend or relative of a resident
My parents-in-law are in Crestview Rehab now. The reason we chose this facility has a lot of to do with convenience. It was in a familiar location in their hometown. For accommodation, we're not elated with the room in general. With regards to staffing, there are people there that we like but we always think they're understaffed. Also it is a rare occasion that the resident doctor would actually go and see their patients personally. It's just basically warehousing people as opposed to trying to find cures or really give them full medical attention. We're not enamored with this place. As for food, I've not eat there but I did observe the foods that are sometimes brought in for my parents-in-law. Sometimes food actually looked very good and recognizable, but there were times where you would kind of go like, "Is that a chicken? What exactly is that?" For activities, I did know for Christmas, they had singers there. They also had a guy walking by with a guitar. They have some presentation for the people there. I think the best thing about this facility is just the location. I think they could improve on the quickness in responding to the needs of their patients. They do have a pull cord but somebody would come in maybe within 10 minutes or something like that, which I don't think is quick enough.

I am a friend or relative of resident
My Dad was there in Nov of 2012 for less than 21 days before he passed away . He was there for rehab for a broken hip. He was there less than 5 hrs before I was called that he had fallen out of bed . When I arrived there the nursing staff was at the nursing station leaving my Dad unattended on his bed strap to a board and rails down . He was taken to Fairfield hosp . for x-rays. This was one of many falls that happen. There was never enough staff there to do the basic care. My Dad was not taken care of. I went every day for 5 to 6 hrs. and Dad was place in chair in hallway. Before my Dad broken hip my Dad walk 2 miles a day, was able to care for himself. He did get confused in the evening but was able to take care of his own needs. I feel like Crestview was the worst place I could put him and before I could find another rehab center he was taken to hosp. and 6 days later he passed away

I visited this facility
I visited Crestview to see my grandfather three times. Every time I went he seemed like he was well cared for. He did complain about the food a little. I thought the physical therapy people were doing a really good job and he seemed excited to go to therapy, which I thought was odd but nice. The place was pretty clean and the people were nice for the most part. There were a few rude nurses but I just ignored them. The only major complaint that I heard from my grandpa was that some of the nurses would get to rough with him when helping him take a bath. He said sometimes it just seemed like they did not want to help him. I thought this was a bit ridiculous which is why I gave 4 stars the staff could use some improving as far as I am concerned. He was there for 8 months and came home.
