Thank you for your review and we agree that families need to visit multiple homes to ensure that the fit for their loved one and they work. In addition, families need to be upfront about the care needed for their loved one and then utilize the expertise of that home to help them. Did you know that in the State of Wisconsin to be classified as a memory care home you only need to provide “client group” training with no direction as to topic or length? At Azura Memory Care we believe our residents and their families deserve better, which is why we have developed and provide an advanced dementia training program, called MOSAIC, to all of our Team Members. In fact, this training includes over 90 hours of hands-on, computer based and classroom training on the disease process, proper ways to approach, communicate, engage and holistically care for those with memory loss. Please note, this is triple the amount of training found in any other Memory Care home in our area and focuses specifically on helping those experiencing behavioral expressions. In fact, Azura Memory Care is known as dementia experts, not only in the Eau Claire community, but across the State. We are continually asked by other assisted living, memory care and skilled nursing facilities to provide this knowledge with their staff through consultations and trainings. Each person who walks this journey of dementia does so differently and while we tailor our approach to each individual, the expectations and ability to accept the realities of the disease by those who know and love them can be very hard to hear, see, understand and believe. We apologize, if in this instance, you felt your experience with our care was not what you expected. When our more than 20 years of expertise in dementia and advanced training is utilized the outcome is typically very good for that individual and their family. However, when this knowledge is completely disregarded even normal expectations would be hard to meet given these constraints.

Questions About Azura Memory Care of Eau Claire
What ongoing training does the staff receive in dementia care?
At Azura Memory Care we recognize that those within our care deserve the highest quality of service, which is why we have invested in creating an exclusive training and engagement program called MOSAIC. MOSAIC is used to ensure that every team member working at Azura understands what is dementia and the symptoms of the different types of this disease process such as Alzheimer’s disease, Lewy Body, Frontotemporal, and other related dementias. In addition, MOSAIC provides hands-on training that provides insight into the proper approach and communication techniques that will allow our Team to be most successful in engaging those within our family of care to continue living life to the fullest. In the first year our Team receives over 90 hours of training and has over 30 hours of ongoing education each year. Azura was also the first provider to require the Virtual Dementia Tour, a sensory experience that helps one walk in the shoes of those with memory loss.
How many staffers are on duty in the daytime and overnight?
We pride ourselves in offering a higher than average staffing ratio, which enables our Team to engage with residents and spend time making their personalized joys a reality. In addition, at Azura Memory Care we monitor the acuity of each resident and of the overall home to ensure that as the needs of our residents change we are equipping them with the supports they need to maintain quality of life and quality of care.
What programs or activities do you offer to keep residents engaged? How do you engage residents' long-term memories?
Thanks to Azura’s exclusive MOSAIC training and engagement program we offer an extensive array of activities to engage our residents socially, mentally, emotionally, physically and spiritually. Our goal is to help our residents maintain a high quality of life. One way we do this is by collecting an extensive Quality of Life Profile for each resident from their loved ones. This profile is then used to personalize their care plan and other tools that our Team uses to stimulate residents in conversations or activities that personally bring them joy. We also take our residents out into the community to local restaurants, concerts, farmers markets or any place that will make them smile! We also tap into their personal desires through our MOSAIC Dreams program, which asks what our residents best day would be and then works to make that day come true. This program has enabled residents to go on helicopter rides, fishing trips, ice skating, Brewers games and to even meet an elephant!
Who assesses residents' health and cognitive functioning? How often is that assessment repeated?
At Azura Memory Care we look at each resident as an individual and deserving of a very personalized approach, plan of care and daily smile. Thanks to the extensive knowledge of our three-pronged interdisplinary team, all assessments required are done by our team at Azura. These assessments are done per code, but also as needed to ensure that all aspects of our resident’s health are monitored in real-time and addressed in efficient and effective manner.
What techniques do you use to support memory care residents during mealtimes?
Our MOSAIC Dining experience ensures that those within our care are receiving delicious and nutritious food that has been selected with their needs specifically in mind. We first tap into the normal likes of our residents and then using a team of dietitians created a menu that is both dementia-specific and mouthwatering! In addition, we utilize a special approach to dining that includes our caregivers sitting down and eating with our residents. Thereby utilizing their expertise to help cue, monitor and inspire conversations and socialization around the dinner table. We have also utilized research on best practices in dementia that include proper table settings to ensure space and tool recognition, adjustable heights in tables and red plates to increase appetite and help with visualization of food.
How do you keep residents’ families informed about their loved ones’ health?
Family interaction is at the heart of all that we do at Azura and our hope is that once you walk through our doors you feel like family! Therefore, we do our utmost best to have an open line of communication with all our families including regular phone calls, emails and photos of their loved one engaged in a personalized moment of joy. We also use social media, website blog, and enewsletters to keep those associated with Azura abreast of what is going on within our family of care. Plus, we have monthly gatherings and events that families are invited to participate in as well.
Describe your community’s neighborhood. Is it located in a residential neighborhood, a business district, a rural setting, or other type of locale?
Azura Memory Care is located in a residential setting and set up to feel very much like one’s own home.
Is the community affiliated with a hospital if more care is needed?
We are proud to work with all the local hospitals, clinics, home health and hospice agencies in the surrounding area. Our goal is to ensure that our residents have the ability to seek and utilize the care they chose at any time. We are honored to partner with in-house providers for some services such as physician, podiatry and beautician; but our residents and their families can also use outside providers too.

About Azura Memory Care of Eau Claire
At Azura Memory Care we recognize that what's on the walls isn't as important, as who is walking the floor. Held-up as a standard in dementia care, MOSAIC honors the personhood of each individual by understanding that dementia is a disability in only specific areas of the brain, not the whole person. When you visit Azura you will witness our everyday use of MOSAIC programming through creative expression, small group peer interactions, special interest clubs, one-on-one engagements with caregivers, outdoor activities, small group outings, daily themes, church services, supervised baking, music and much more! In addition, you will be visiting a home that was specifically designed to support and engage those with memory loss throughout their disease process.
