I am a friend or relative of a current/past resident

If I can give zero stars I would. I don't know where to begin. My father was a patient at this facility for only a week. In a matter of a week. This facility did not give him his required meds, misplaced inhalers, bruising on his eye and a lump on the head (which may required stitches), arm bruised due to blood work (which was not authorized) dehydration and lack of exercise. The nursing staff failed to properly insert a catheter. Patients only leave their beds for dining purposes only, maybe if there is an activity going on. According to the staff and I swear you cannot make this up! Patients were in bed due to quarantine. Yet they were all eating in the dining area at the same time!! They did not change my father's clothes maybe twice in a week. They even sent another man's boxers in his suitcase!!!! Again you cannot make this up! I will be contacting the VA and put in a complaint through them. This facility is contracted through the VA so I will go through them. I also have pictures I will be submitting as well.

I would not recommend this facility. We brought our mother there for end stage dementia. We brought in her clothing and a handmade blanket that was made for her birthday. We have lost our dear mother and all we ask is that we be returned her personal items. We requested the blanket be returned but despite going there and asking for our item and calling daily, they will not give us an answer. It's the same runaround, that the persons that we are calling are unavailable. They never call back! When we went to pick up our mother's personal items, we were given a different resident's belongings. At least we were honest enough to return those items to the home. God only knows if they were returned to the rightful owner after we returned them. We paid cash for our mother's care and that's all they cared about. It was not a matter of her being on welfare. The very least we can ask for is a handmade blanket be returned and the lies stop. Please do not put your loved ones in this facility. They are dishonest and rude and it's all about the money. The fact that they can't even tell the truth about something as small as a blanket should tell you everything you need to know about the care they are receiving. They are rude and will lie at every turn. If you care about your loved ones, find a different location.

I am a friend or relative of a current/past resident

My mother is in Aperion Care Oak Lawn. I think it's the best place for her right now, she seems to be getting very good care, and she's still getting therapy a couple of times a week. They're gonna be reassessing her to see if hospice is necessary for right now. Since she's been there, it's been good, because we actually get a hold of the nurses when we wanted to get information on her, and actually we could even talk to her. They'll bring the phone to her, and we can talk to her, but we're not sure if she knows us or anything, but at least we can talk to her. I've been very pleased with the nurses there, they seemed to really be taking good care of her. She's a fall risk, so they make sure they take care of her well, they make sure she's strapped into the chair, they even placed mats around the bed because she tends to get up. They watch her a lot. It does seem clean, and as good as any nursing home. It's a smaller place, and I liked that. They're able to give a lot more one-on-one care.

Please people check places before placing a family member there. The place smells of urine when you walk in, they have bugs flying around in the room, they do not change the sheets on the bed, they put something over it, very unacceptable. I am the health care field, and I was totally taken back, I could not believe the conditions in there. PLEASE DO NOT GO TO THIS FACILITY.

I am a friend or relative of a resident

My dad was at Aperion Care Oak Lawn, and it was pretty good. The people are very, very nice and treated my father very well. The staff was very good. We have absolutely no complaints about this nursing, just a little under staffed. The physical department was really amazing. Their food was fine.

I visited this facility

I would not recommend this nursing home. Soon after passing the lobby into the hallway I was greeted by bad odors. In the large multi-purpose room that serves as an activity room, the bad smells continued and seemed to be enhanced by the heat and lack of ventilation. As if the bad odors were n't enough to turn me off, I was appalled by the residents' grooming. I know that this is an issue in many nursing homes, but still it was pretty bad. Many of the residents seemed to be wearing their breakfast on their faces, their clothes, their arms -- you get the idea. Many residents looked very disheveled, needing their hair combed. A couple of patients were asking to go home. It was not a happy sight. As for the facility itself, little thought seems to have been given to making this a cheery place. The floors and walls were clean, but mostly barren and the furniture old. The nicest area that I saw was an extension of the activity room which had modern windows and looked like an enclosed porch area. There was also a little patio for residents to go out. By and large, the staff seemed to be kind, caring people so I don't understand about the grooming. There is free parking outside the building. This is not a place where I would leave my parent or loved one.

I had gone to visit Concord Manor Health Care because of a family friend who had recently been placed into the home for future assistance. I was immediately surprised by how many residents were in the home and the conditions therefor were lacking. I had felt the space to be crowded, and in turn, a bit cluttered and dirty. It just goes to show that sometimes you do actually get what you pay for.
