I may pay for this, but folks should know that there is no one at this facility who is there and in charge. There is only one contact person, who is charge of 6 facilities around the state. She is there only on a rotating schedule and as said, no full time admisitrator on site. Can be reached only by text, voicemail, or email. She does answer her phone at times, and does reply to texts. There are other petty issues but once again, no one to speak to directly at the facility who can take action without contacting the above person. Could be that the rates are more affordable than surrounding locations, so they can't afford a full time administrator. My spouse seems content with the way things are, even though she does have some severe memory problems. I moved from Palm Coast to Holly Hill to be closer to her, so she will stay unless something serious happens to her. I do not know if workers are RNs, LPNs, CNAs or if any are actually legally able to provide care, though I have asked. I have in the past year seen only one person with a name tag, which I feel should be required. I've said enough; but one more thing comes to mind, No one will answer my questions about residents are vaccinated against all the likely diseases floating around. Still trying to get reply to that one.

I wish there was a way to not only leave a zero star review, but a red flag to the families who are considering trusting this facility with the care of their loved one. The conditions are not only unsanitary, but unsafe, this facility has become nothing more than a holding pen for these poor souls, just marking the moments until their death. We found there to be no compassion, no care, very little interaction, even the essentials such as dressing and bathing are neglected routinely. The food is not only nutritionally suspect, it is vile. The roaches are the only creatures who seem not to mind the mess. Medication was missing, and even reordered after his departure, making it impossible for the new facility to have filled, and although they said it was returned to sender, it was never seen again. The individual that was reported to be the "nurse" ended up being someone who was not qualified to hold the position. The director Paul appeared to never leave the comfort of his office by the front door and the rest of the staff congregated in the conference room, two locked doors away from the ward where these lost individuals were warehoused. Shame on the directors, ownership and the employees who willfully neglect these helpless patients.

It’s not a 5-star glamorous establishment. It is run by a staff that cares deeply about the health and welfare of the residents living there. The Administrator is a person that knows and greets each of the residents by name. He manages a Not-for-profit facility with many years of experience and focus on the needs of his staff, the residents and their family. He physically helps with tasks that improve the operation and environment of the facility. During Hurricane Ian he staid inside the building to insure the safety and quickest possible recovery from utility outages. The Director of Nursing, wears many hats. From managing resident care, to transporting them to Dr. visits, to assisting his staff personally, to make sure the proper medications are provided in a timely manner to residents. His communication skills are extremely positive and effective and show a great deal of care and understanding for all he come in contact with. He takes on many roles in addition to his Nursing job description. Marketing to the state inspections, to food server, he is competent and dependable. The Activities coordinator has more than 20 years of experience in assisted living event planning and activities. She spends her days in direct contact with the mobile residents, providing hands on support to their needs in this area. The Cleanliness coordinator is unparalleled in his diligence to maintaining a sanitary environment. He is in constant movement, zipping around the building get things done with urgency. Yes, I’m bias. After 5 years of close observation to make sure my wife who has dementia, is well taken care of, I do recommend Indigo Palms for families that need help with the some times overwhelming care of a loved one that is not able to function independently.

2 people died there, one resident escaped and was found deceased behind a house less than a mile from the facility. Another had a fall and died 6 days later. Additionally, my relative recently experienced severe malnutrition, bruises, bone on bone bed sores, and a brain bleed that’s being covered up. My relative is now in a coma and may have to be taken off life suppport. This all happened 01/17/2021 I would never recommend anyone use them your loved one May end up dead, Facts

Director of nursing is skilled, caring and very responsive. Staff provides good assistance to residents. Activities include karaoke led by caring volunteers, musical performers, and physical activities for residents. Freedom to move about in the outside enclosed areas is very accessible and well maintained

My significant other has been here a year and a half. It’s a good place for him he needs care that I cannot give him at home.

It was fine. The apartments were a little bit bigger than the other place that I visited. The staff was friendly and accommodating.

Indigo Palms reeked of urine. It smelled horrible. Everybody in there was clearly having more memory issues than my brother. The staff showed me a new area that they had just opened up, but I don’t remember much about it. I couldn’t get out of there fast enough. The residents were in more serious condition than my brother, so it would not be a suitable location. The amenities would not have mattered once I walked in the door.
