You have your own room, the staff are very friendly caring people, they all do the best they can to take care of the people in their care, very nice and clean nursing home they were very supportive and sweet to my sister and I,, my sister was a hospice patient here,all I can say is come check the place out it's wonderful, no where is perfect but they do the best they can,

Had a recent skilled stay here and everyone was wonderful. From my charge nurse, to daily staff. Everyone was very caring and sincere. Their DON took the time to walk me through questions I have had a long time about my illness and conditions. I am thankful I went and would return if needed to.

Both of my grandparents live here together. They get to share a suite and still continue to live together. The staff cares for them and has really made a difference with my grandpas depression. They do a ton of activities and try to keep life as normal as possible. The rooms are actually quite big and they had to choice to live in suite in separate beds or move them together, I like that they had options. My grandparents love the food too. It’s like menu choice.

I visited this facility

This has to be one of the worst places in the world! The D.O.N. "Polly" is complet joke! They let go of really good aids probably the only ones that truly cared on the floor 11 to 7 my mother will never go back again!

My husband was at Sienna Extended Care & Rehab. That was during COVID, and so I could not go up there and take care of him. They only had one nurse on duty covering two halls and no CNAs, but before that it was fairly decent. It was nice, clean, and beautiful. He had a good aide that gave him a shave and bath times. They kept him pretty good. The food was lousy, though, but they had bingo and music every day. At one time, they had a nice director of activities too.

They do not answer the phone in the evenings. Tell you what they think you want to hear but not always truthful. Nice building. Need better trained help.

Do not send your loved one here- The people you deal with are friendly- but you can not speak to an administrator- no way to get help with anything- nights, week ends or holidays- your loved one is in extremely understaffed- avoid this place at all cost.

I visited this facility

Please do not bring your family members to this hellhole. I witnessed patients lying in bed, naked, uncovered, and wailing in pain. They are giving end-of-life patients Tylenol for their pain instead of morphine. It happened to our family member, a U. S. Air Force veteran, and it was nothing short of traumatizing to have to see him suffering like this. We were lucky enough to have been able to get him out of there as soon as we noticed the horrible neglect. He died peacefully and pain free at home with the help of home health care and a hospice nurse. Now Sienna Extended Care is dragging their feet to refund us the money for almost three months of pre-paid care fees ($18,000) which they insisted on having paid to them before they would even see the patient. We are grieving over the fact that we wish we had known earlier and didn’t rely on “professionals” who told us that this was a reputable facility. Please do your research before entrusting your vulnerable loved ones to just anyone.

Basic problem I have with the facility is nobody wants to answer the phone!!! this facility is a 24/7 operation and when the phone is ringing, someone should answer it, NOT TOMORROW but now. I don't care where or who you are. THERE MIGHT BE AN EMERGENCY!!!!!! Even if no emergency, the 24/7 operation should be answering the phone as if it were an emergency - you are a business and a "care giving" facility. At $225.00 a day private pay, plus whatever else you charge for, the phone should be instantly answered!!

If you place your loved one here please stay active with their care. The overall facility looks very nice however most of the nursing staff is rude, non caring, and inconsiderate. When you request assistance for your loved one... the staff behaves as if you are inconveniencing them. Owners should have courses to educate and inform staff about patient care and care with dignity. State needs to come in and see why they don’t have enough staff. They are so short staffed they didn’t give my LO 2 meals for that day. I Was extremely upset. How do you forget to feed your patients? Aren’t there charting requirements to indicate the care the patient receives that day? What happens to the patients who don’t have family to check on them daily? Do they get fed? Do they get their required medications? I would say These events occur on a daily basis. Melissa aka Director of Nursing is completely over her head in this facility. She has no control of her team and the care they provide to residents or it wouldn’t be so intolerant. I would recommend anyone who is considering this facility to just sit and observe for a little bit. The issues are clearly evident in the attitudes of the staff. Also make sure your loved one is getting their medications. They forget to do that as well.

It took a few days to get situated, but they were very good to my father. It wasn't perfect, but it was better than most. Stay clear of "Tiffany" she is a patient advocate/social service. She was aweful, mean and cruel as well as lazy. The facility was always clean, my father was clean and he loved the nursing staff. Overall they took excellent care of him, he was in tears when he had to leave. I wish they would get rid of Tiffany and hire someone caring and kind in her place. That would have made it a much better experience. Now that my father is fully Medicaid, I would love to take him back to Sienna, but I don't know if I can deal with working with Tiffany. How sad is that? I am rating 5 stars because it wouldn't be fair to everyone else to give less than that. Tiffany is a bad seed and a cancer to Sienna. GET RID OF HER!!!!

Sienna is like a roller coaster...goes up and down with care. There is no consistency with care. A lot is expected of the aids with little or no supervision. There are so many areas that are not transparent that family members have to keep an eye on. The owners are all about the money going into their pockets. I understand making a profit but not at the expense of the residents. The monthly care cost continues to rise with no better care. The food is basically bad. There are residents who need help being fed and they are at the mercy of staff. If fed in the rooms, many times the food is cold and undesirable by the time they get around to assisting with feeding or the resident is too tired to eat. Something needs to be done to keep these type of facilities honest. You have to walk on egg shells with management or they might retaliate. I feel sorry for anyone having to deal with care giving facilities. Owners and management need to have real integrity...be present regularly supervising personnel properly, screen for good qualified staff and do not work the good staff to death. Sienna is a newer facility that is nice but what is going on internally needs a lot of improvement.

Mentioned to nurse anbout cutting my mothers finger nails and she said no problem but it took a week to get them to do this. I happened to notice her feet extremely swollen and when you took of her socks you could see deep impressions where they were making in her ankles this was due to them taking her off the water pill. As trained nurses and doctors they should have caught were swollen feet because of the danger of retaining water and I had to provide compression stocking because they did not have that size. They said she was able to walk unassisted, however when we came to watch they had a belt around her and they were holding the belt, that is not unassisted. We told them about her unable to have coffee and sugar numerous times but we kept seeing her with coffee and sugar. During a caregivers meeting we told them about the coffee/sugar, her finger nails not being cut, not getting baths on her days and not eating but they said they did not know about the sugar when we told them the day and the day after admission but no documentation until we told them in the caregivers meeting. We told them this because her PCM said no coffee and very limited amount of sugar. The only thing that was corrected was her finger nails finally got cut a week later. They could not explain anything about them not noticing the swollen feet. Overall the nurses were trying but there hands are tied because of management. Overall I would not recommend this facility for any of my family or friends family. Called the Oklahoma Health Department and I was told they could not due anything so therefore I decided to write this review and try to let people now how terrible their care is. By the way the day she was released I took her to her PCM and he put her back on a low dosed water pill and within 5 days were swelling was gone and they looked normal again.

Please do NOT ever put a LO in this place unless you plan on staying there 24/7 to observe what is going on with your LO. They treated my LO horrible. The assisted living center my LO had been living in basically pulled my LO out of that place because of the lack of care my LO was given. The staff was rude and non caring. While in that place, my LO went down hill physically and never recovered.

My dad has been in there since this afternoon and I do not like it. He has no heat in his room. He has told them numerous times the heat is not working. They do not listen and will not give him an explanation as to why the heat is not working. He can't be in a room that is cold due to the meds he is on. I give this place a -0.

I would have given Sienna Extended Care & Rehab a zero rating but it was not provided as an option. We chose to place my mother in this facility due to it's location and on first glance it looked like a nice facility. While looking at several places for my mom I visited Sienna Care. I pushed the button on the outside wall and walked in. I walked around this entire facility without one person greeting me or asking if I needed help. When leaving the building the code to get out the door is on the key pad so I just exited and left the facility. My mother received very poor care while there. While visiting with her one day she told me she had not been getting a medication that she had been on for years. She takes this medication 3 times a day. I asked the LPN on duty for a print out of my mom's medication schedule and was provided with it. The medication was not marked as being given to her along with her blood pressure medication that she had been taking for years. My mother had not received these medications since she arrived at Sienna Extended Care & Rehab from the hospital. This was brought to the attention of the staff who stated that she had been getting them, that what I had received was just a quick printout. Immediately following this incident my mother began to receive the medications she had not been receiving. On one occasion my aunt went to visit my mother and she heard the CNA through the door tell my mother that she "would leave her sitting in that wheelchair for a good long time." For several days I told the LPN and Wound Care Nurse that my mom had a rash on her feet and legs that appeared to be a fungal infection and needed treated. She also had a pressure wound to her heel that needed the dead skin removed. I finally purchased anti-fungal cream and powder to treat my mother myself. Finally, I was told by the Wound Care Nurse that my mother had a fungal infection and the doctor ordered medication for it and that he had taken care of the wound on her heel. During a Care Meeting with the Caseworker Lee, my sister, my aunt and I attended, we were told that once our mother was released from skilled nursing that she would be allowed to stay at Sienna. Even if she didn't participate in rehab. According to them, "there are ways around that." On Monday, October 24th, we were advised that our mom would be discharged from Skilled Nursing and we had to find some other place for her to go. The excuse was that "they are trying to get more patients that are better able to take care of themselves." While visiting our mother after be told we had to find another place for her, we were chased down the hall by the Caseworker Lee trying to get us to sign discharge papers. Please do not put a loved one in this facility.

I visited this facility

Sienna Extended Care & Rehab is a nice place and close to my home. They are pretty heavy on the security, so, it was hard to get good information. I think they need to be more available to the people that are looking for information. Their manager is very difficult to talk to. It looked OK, so I would still recommend this place. I think they should have better management. My sister-in-law said the food is OK.

The people there were fantastic at Sienna Extended Care & Rehab, and the food was good. I don’t have complaints; I thought they were great. The rooms were spacious and nice.

We chose Sienna Extended Care as it was convenient, and it was the only one that took Medicare. The facility is very nice, and there is plenty of space. The staff was very nice and informative, and if I have questions they tried to find out for me if they didn’t know. The actual room is private, but the entrance to the room connects to some other else’s and they're separated by curtains. They have arts and crafts and bingo, and I don’t think he has had any complaints about the food. It is very nice inside, and it didn’t smell. The landscaping is nice; they have an area within the hallways of the facilities where they can go outside of the building. They are kind of like in the center, and it’s nice to have benches outside that they can sit on.

The staff of Sienna Extended Care & Rehab that took care of my father-in-law was very, very good and they took very good care of him. He passed away there, and on his last day one of the nurses even came in and gave us a break and sat with him and took the time to try to feed him and carry on a conversation, even though he wasn't talking back. They way they were set up was semi-private, but each one has his bed in a separate room, and then there was just one sitting area, so we were able to sit out there and deal with other family members, and we were really comfortable. It was a very, very clean facility. My mother-in-law and father-in-law both were very happy with the food over there, probably more so than the other place he was. They had more choices. I would recommend this place to anyone.

Siena is short staffed and does not meet the needs of their residents. My mom was there for a short time and we are very unpleased with the care she recieved. I wil not recommend this facility to anyone looking to place their love one into a LTC facility.

I visited this facility

The staff is great. My grandmother is very social and the staff is very social and friendly. The facility seems to be clean every time we go. The food is average by her standards but it is a nursing home. Overall it seems to be one of the nicer places around.
