I am/was a resident of this facility

I have been here almost 3 years and I was only supposed to be here a few months. After some PT, I was told "insurance " was not going to pay for PT anymore. So I got stuck in bed and ended up with 2 bedsores. I never went anywhere except to change rooms umpteen times. They never put me on my side to get me off my butt which is where 1 bedsore is. In just the past few months I've been lying on my sides. A lot of aides seem like they are only here for the money because they are in and out of the room with hardly any sympathetic care. This facility is always running out of supplies. Most of the time from what I've seen its wipes and briefs. The food is horrible. It's been really spicy the past several months. They don't have any type of extracurricular activity for bedbound residents. No more bingo on the TV, no coloring or word searches. When they have parties, we don’t get anything, not even cake. Getting a hold of an office member is laughable. Example: I've left messages for the social worker and she never gets back with me. I continually have to keep calling to talk to her. I hear a lot of promises but no follow through.

My husband had to go to rehab after being in the hospital for just under a month. He was very deconditioned from recovering from his surgery and he had pre-existing mobility issues. From the beginning, I was greeted with kindness and attention from Liz, who was always a friendly face and there for a kind word to get us through this difficult time. Day one the team (included PT, SW, financial, ect.) met with us and came up with a plan for rehab and going home. The nurses were exceptional especially Kimla and Cheryl Lin. (Sorry if I spelled their names wrong) My husband adored them especially with his complex wound issues. The physical and occupational therapist were amazing and customized a regimen that met my husband where he was physically. I just want to give a special shot out to Megan and Robert with PT/OT. You guys rock! If you are looking for equipment (like a special bed, ask for Tiffany. She was on top of it once she knew he needed one. The DON ( knew me by name and was instrumental in fixing things when issues came up. A special thanks to his nurse aid who was whiling to go the extra mile. Thank you Priscilla! I also want the thank Efran (the facility director) he took this time to know my name and met with my husband personally to make sure he had everything he needed and he help us along with Kimla (his special nurse) get safely into our car on day of discharge. We are not anyone special but we were treated like VIP’s.

I visited this facility

My dad is in this facility. Like anywhere else you visit, there are good and bad employees that should be in a different line of work. Overall, I am pleased with the staff, especially the Social Worker and the nurses. They are very compassionate and listen to my concerns. Some of the CNA's are really good, some others, not so much. The food is just okay. You have options though if you don't like what is being served. They will make you a sandwich. I would like to see more interaction like the dining open so the residents can see other people. COVID has really taken a toll on what the residents can do. Overall, I am pleased with the facility.

My friend was a resident of Cary Health and Rehabilitation for almost three years beginning in 2020. She recently passed away. For the benefit of those who might be considering placing a loved one here, I want to share our experience. Staffing: During her stay, Cary Rehab had 3 different directors, at least 2 nursing directors, 2 activity directors and COVID both within and without the facility more than once. Certainly, there were some responsible and compassionate caregivers, but those people came and went. There was a lot of turnover and indifference. Care: My friend had multiple falls while here: from bed, from her wheelchair. “Bed baths” seemed to be the standard. We were told staff can’t force anyone to have showers. On one occasion, when my friend had to be admitted to a hospital, the facility director at the time tried to refuse to accept her return to the facility, a practice known as “dumping.” Belongings: Many things disappeared never to be seen again: dentures, eye glasses, clothing, shoes and even her wedding rings. We were routinely asked if we were sure we had not taken these things home. She was often dressed in clothing bearing someone else’s name. We would replace and label new items with her name, and they too would go missing. We were reimbursed for one item, a pair of shoes. Sanitary conditions: I personally found dirty clothing and trash mixed together in a hamper in her room. I witnessed staff tossing soiled disposable undergarments and bed coverings directly on the floor while changing her. Why didn’t we move her to another facility? 1) It was nigh impossible to move patients between facilities during COVID’s height. 2) Moving is very disruptive to people with dementia. 3) This facility was within driving distance of the person who loved her most and visited often. Decisions about long-term patient care are based on many factors: finances, beds available, proximity to keep an eye on things. Every family must make its own choice. Do not bring your loved ones here! Rooms are old, creepy, and musty; there are no amenities - besides a few dollar store children's puzzles; the communication is horrible; everyone says "It's not my job" and has no idea who is responsible for what; there are no doctors working there - only one or two PAs who visit patients maybe once or week or in a dire emergency which takes forever; the Nurse Aide's sit around doing nothing and then when you ask a question or help for a patient, they look at you like you have 5 heads and they have no desire to work or do anything! There is one RN for 15 - 20 patients; you may get one hot meal out of 10 if you are luckily; meals are NEVER on time; forget about getting water - they patient can be dehydrated, while the Nurse states out loud "oh you are dehydrated, we need to get you water " and then doesn't for another 24 hours; they let their patients sit in their urine and feces because again no one wants to work!

Most of the nurses are on agency contracts. My weekend nurses would come in my room talking on their bluetooth and completely disregarded any of my requests, as if the patient was not there. One night I was bleeding for 4 hours and also begging for a nurse to attend to my surgical wound, as requested by my surgeon, but I was told there was only 1 nurse for the whole facility. 1 nurse for 50 rooms. I fell asleep bleeding and no wound care. I guess its was not their ankles they were worried about getting infected. The wound nurse started an argument with me when she did come see me and raised her voice instead of just attending to my surgical wound. I was prescribed wound care daily but I only received it every other day and I had to beg for it. Towels are gold. If you get one, hold on to it. I tried to use the nu-step which was an excellent rehab exercise but was asked to leave the therapy room numerous times although I was paying $362 a night. Yes, a little insulting. The shower room was completely unkempt. It was obvious based on the resources provided that profit was of the highest concern, hence one RN for 50 rooms. Although they participate with my insurance, they brought in an out-of-network medical staff who charge 3 times the standard/usual insurance reimbursement rate. Naturally, they don't tell you the provider is out-of-network, even if it means a financial burden to the patient. It is a buyer-beware medical practice. When I emailed their parent management regarding my concerns they never contacted me in return. I guess they are not interested in the feedback of their patients. Its a shame, these concerns could have been handled privately. So this is what I'm left with.

dangerously understaffed, horrific communication, impossible to get a call back or update on pt status, do not allow family members in the building, mistruths to cover up poor care, several confirmed allegations by the state, 1 STAR rating...my sister was there for physical therapy and ended up with a grade 4 wound with sepsis...sent out to ER and is now in hospice. The admin lacks professionalism and does not manage this facility properly. #truthwithnoexaggeration

PLease try to find another place other than this center. It is a place that is not well maintained - broken toilets and equipment. Employees do not wear name tags; will tell you someone will help you and as many as 3 hours later, you may be still waiting for help - And urinals, bedpans,personal items are moved by staff out of reach of patients. ..if you have to void in the meantime, you can stay wet for hours before someone changes the sheets!! This place needs to be closed -greatly reduced staff on weekends and during covid when no one can visit and see how bad things are, conditions have been even worse.

I couldn't be more pleased with the wonderful care that my dear friend has had during the 22 months he has been with this care team at Cary Health and Rehab. My weekly visits in his support have given me the opportunity to meet with the members of the care team. From the moment I walk in I am welcomed by name by Classie Anne and often by the Executive Director Doreen. As I head down the hallways, each person shares smiles and ask if they may help. As I approach my buddy's room, nursing, CNA's, housekeeping, dietary and maintenance folks share smiles and engage appropriately. We laugh together and sing a song together and help each other lighten the load of the day. It's as if I am an important part of the team. Then I reach my friend's room and he is smiling as we greet. Often, he is having lunch while I roll out his weekly supplies, placing them in his designated places. Social services, accounting, housekeeping, dietary, activities, maintenance and others greet me and ask if there is anything they can help with . It's like family. That's it! It's like family!! My buddy has no family that visits with him. At least not in the 4-5 years since my wife and I jumped in to care for him. Don't get me wrong, this isn't about me or my wife. It’s about our buddy and friend. You see, we've had him in 3 different care facilities during this time. I've had the experience to assess best care practices and Cary Health and Rehab is the best of the best. We've had opportunities to move our friend closer to our home to lessen the 70 plus miles round trip; but will not take the chance of having him in a care center that may risk his quality of care. The recent change in executive management and leadership integrates the best of executive leadership with compassionate care. My buddy and I could not be more pleased. Yes, my wife agrees also. Ben

I visited this facility

I have been a patient here for over a year and the receptionist has always been terribly rude. Today my appointment was at 11 a.m. I have never been told to arrive early except for my first appointment. Today I arrived at exactly 11 a.m. and she was with another patient bantering with him in Spanish. So I sat down and waited until she finished. I walk to the desk and she told me I was late that it was 11:03. She would not allow me to see the provider because I was late even though I arrive at my schedule time. I became visibly upset and started crying and told her this was ridiculous and that I wanted to speak to the provider. A nurse came and offered me water and calms me down and gave me a number to call to file a complaint that does not work. The receptionist even called the cops on me. When the cops arrive I explained the situation and they actually almost laughed about it saying they have had problems here in the past. I would not advise going to this doctor unless you want to end up wasting your time and efforts making your appointment on time to be told you cannot be seen. If your scheduled appointment is at 11 a.m. and you arrive at 11 a.m. they tell you you cannot be seen. If they want you there at 10:50 then they said schedule your appointment at 10:50 am. This receptionist is so rude everyone always has a problem with her I don't know why she is still employed there I guess it's because it is hard to find bilingual help I don't know but I will not be returning. I do not advise anyone go see this doctor regardless of the sliding scale it is not worth it

I stayed at Cary Health And Rehabilitation. I was very happy there. The staff there and people were very nice, and my room was a good size. They also have activities for the residents and everything was good, including the food. They have physical and mental therapies, and very nice therapists. I wish I could have stayed there forever. They have amenities like a gym, exercising, and so on.

I visited this facility

My daughter was there in September on hall 4 and has been there at least 12 times for Respite Care. Management has recently changed with most, if not all rooms now being single occupancy. this is a major improvement. The staff seemed less "hurried" and the check in went well - the staff was observant and we felt comfortable with her being there. One thing I would change is having to see both Admission and billing persons separately because they were not both available at the same time (billing was in a meeting BUT Admission did get it all done. Previously you only saw Admissions. We will continue to use the facillity .

I visited this facility

I am a friend of a family member. this place is HORRIBLE! I wouldn't let my dog go there! The staff is certainly not qualified to take care of sick people. I never saw an RN or a PA, only LPN's. Which in my opinion is a glorified Medical Assistant. When my friend arrived, the room that she was put in, had a disgusting bathroom it was extremely dirty and there were a number of flies. So her son requested that she be moved to another room. Which supposedly was to be a newer room. The only thing different was that the bathroom was cleaner. Last night her son went to visit her and she was having chills and her temperature was over a 100 degrees. Her blood pressure had dropped to under a 100/60. Her son was outraged and told the staff to call an ambulance immediately and he wanted her transported to a new by hospital. If you don't have an advocate to watch your loved ones, your loved one could die in this place!.

While most places will have its pros and cons, this facility is filed with more cons so if you care about your loved one don't bring them here!!! The nursing assistants have 12-16patients to care for each shift which explains why my father soaking wet and the call bell ringing with staff walking by as if they didn't hear it!!! I'm not sure how many patients the nurses have. I have yet to see an RN mostly LPN. It's like pulling teeth to have conversation with them to find out status of your loved one! Unfriendly staff. Dad needs set up to eat, tray was sat down in front of him and staff just left it!! Sitting there when I arrived so based on time I was told trays arrived and his guess , the tray was at least sitting there 10-15 minutes without him able to open the milk, remove the tray top, or take the top off the drinks to have lunch. Ohh the ice cream was now a milk shake!!! They do have new management, but staffing remains too short staffed for them to provide adequate care. Staff is always waiting for laundry and supplies. Overall it is clean, but old needing repairs. Showers In the room don't work!! Generally no urine odor! They really need to hire more RN's to professionally manage this place so there's better guidance.

We have been very involved with my dads care. Someone has been there everyday and still this happened. 1. My father has dementia he was left in the bed for a long period of time (who know how long) without being checked on. When we came in dried and fresh blood was all over him where he was against the wall and scrapped his knees on the rough wall texture. 2) He was taken to a Dr appointment (taken by the nursing home van) where my mother was waiting on him at the Dr office. He was taken to the wrong Dr office and just left without finding out if my dad had an app there and asking if a family member was there!!! My dad had a BM everywhere and was trying to clean it up!!! The staff at the office tried to help him and he was scared and would let them. 3) They let him go a long time without water and he got dehydrated, went to the hospital and almost died. 4) They had no hand soap in the bathroom for 3 weeks. We met with and called them out on each time. my dad went down fast after that and is now dying related to previous dehydrating.

Cary Health and Rehabilitation Center was clean. My brother seemed to like it. The people were very nice. The food was fine. He really did not have any complaints about the facility. It was very clean.

Worst Place Ever to Stay at. Food Cold, Aids are slow and had to wait one hour to go to bathroom. You have to ask for a pillow and blanket. They don't return you calls. Would not stay here again
