Woburn Rehabilitation and Nursing Center looks nice and homey instead of institutional, which is nice. The rooms have the feel of a home bedroom, which makes it comfortable. Some of the rooms are either semi-private or private. We moved my father from a semi-private to a private room and I don't know if all the rooms have their own bath and shower, but in the private room he is in, they have a private bath and shower, which is very nice. Some of the staff, especially the case workers or physical therapists, seem very good and some of the nursing staff. I've not met a doctor yet. Some of the nurses and aides can be a little bit rough though. They have a community room where people can gather and I think they have a TV and tables. So, it's a gathering place, but I haven't seen many activities yet.

They robbed us of the opportunity to say goodbye to my dad. It took them nearly 4 hours to notify us of his passing. He passed at about 8:00AM and they called me at noon. I requested a call from the administrator and it took a day and half for them to call me back. I was told it was a “miss communication, and the staff was overwhelmed “for not contacting me when my dad started to decline. I could of been there with him!(we only live 8 minutes from the facility. Please don’t put your loved ones in this facility.

Nursing staff were compassionate, conscientious and extremely attentive to my brother's needs. During my visits, my brother's door was open and I observed that they were equally kind and understanding with other patients as well The special Veteran's Day activity I observed got the residents involved and was very cheering, uplifting and moving. This is a good place!!

My mom was at Woburn Nursing Ctr. for 3 years. For the last 2 I had her on waiting lists for other facilities as she could not stand it there. I was not going to leave a review as not to be negative, but in lieu of a new 5 star rating by the federal gov't, I wanted to voice an opinion myself. I will say the asst. director of nursing was wonderful. That is the only positive thing I can say. As for the negative. My mother, although in a wheelchair, was much higher functioning than many patients in the facility. Because of that, her needs were quite often ignored. State mandates require a call button to be answered within 5 minutes. Time and time again, my mom would call me and say that no one had answered her call...20 min later. Several times this call came from the floor where she had fallen. This was documented in Woburn's system...not just my mom's report. I had to get the obudsman involved. One instance was particularly disconcerting. My mom called one morning to tell me that she had chest pain and pain down her arm. I told her to press her button and I would call the desk and tell them to call 911. 20 min later, I called the desk to find out what hospital she had been sent to. I was told by the nurse that they had not called yet because, "It is not an emergency"!! It was to me and my mom! My mother told me on numerous occasions that she was depressed. I had this information relayed to the psychiatrist (on staff at the home). He responded that since she was eating and interacting with others, she was not depressed. I told the nurse that he does not speak with her several times a day as I do. It took months to get her anti depressants adjusted. My mother deals with severe, daily pain. Once I asked how many pills she had left as I would need to get a refill myself as it was a compounded medication. In a month's supply, there were 17 pills left. She had gotten her "daily" med 13 out of the 30 days she should have gotten it. Every time I got off the elevator and saw the banner mentioning their "Award for Excellence" , I got very upset. Maybe the families should be the ones to confer the awards...not the government officials who do not deal with these facilities on a daily basis. As luck would have it, a room has finally come up at a different facility. She just told me she is happier than she has been in over 20 years. Now I am happy.

My father-in-law is now at Woburn Nursing Center. The location is very nice, and it is very close to us. However, there really isn't much in the way of grounds at the parking lot all around the building. The rooms are very nice though. They have recently updated all of their rooms. I do have one suggestion for them. They need to have the television speakers come through a remote on the bed so that there are no blaring, competing TVs in the same room. Most of the staff is wonderful, but there are a few bad apples. There's a 10% nighttime crew that perhaps aren't as gentle and not as attentive as they should be. For instance, my father-in-law is on oxygen and it is imperative that he have that because of his COPD. However, he was left for approximately six hours without oxygen. His oxygen level went so low that he ended up going to the hospital; that was strictly due to their negligence.

This place was so chaotic. My mom arrived on a Friday before a long weekend and it took 10 days to meet with the intake person and sign the paperwork and receive the orientation packet, including the list for the tv channels and information on the laundry services. Rules were loosely enforced. I had been taking my mom to the bathroom several days before someone told me I couldn't because I wasn't "approved." Meanwhile, they put my mom in diapers because they never responded when she pushed the call button so she would have an accident. One time I did ask someone for help and she disappeared. I had hoped to put my mom in one of the Winchester Salter facilities but was pushed to this one, which seems to be the most run down. And there isn't enough parking. And there really isn't much space outside to enjoy. I would never let my mom go back here.

My mother went to Woburn Nursing Center for a short-term rehabilitation stay. The facility itself was clean and the services provided by Woburn Nursing Center were great. My mother loved the food and the food service department was excellent. There is a continuity of staff members there, which was very important for my mothers rehab. I am happy with the nursing staff, the CNA's and therapists. I would have my mother return to this facility as well as recommend Woburn Nursing Center to others.

I am a friend or relative of resident

My parent was a resident of the Woburn Rehab for two months. It was a very clean place and the administrative and social activities staff was extremely helpful and nice. However, other than just 2 very exceptional CNA's, I found the other CNA's to be inattentive, lazy, impatient and often made nasty comments to the residents. Often when feeding some of the residents, they would rush the patients and talk down to them. Patients who needs to use the bathroom were often told to wait over and over again until they could no longer wait another minute. They often congregated in the living room instead of attending to the patients. The nursing staff made no efforts to make the families feel welcomed or to let them know they were there if you or the patient/resident needed anything. We always felt like we were bothering them. The nurses also questioned the doctor's order and wanted to follow their own agenda. We had to stand firm on this as my parent's doctor knew his patient better than they did. The food was also horrible. Given the cost of $450+ a day, the food should consist of more than a sandwich and chips at dinner time. I'm sure there are NH's worse than this one, but given the choice, I would choose a different facility.
