I am a friend or relative of a current/past resident
Mom went in with a broken arm for therapy starting 4-26-17 as she could not use her walker due to a broken arm. In the hospital she was able to walk with one assistant to the bathroom. Within two days at Augustana Chapel Hill in Hopkins she could no longer walk, was shaking severely, crying, dehydrated, blood pressure to low, wanting to die. My sister and I kept asking why all this was happening with only a broken arm?? No one listened, told them this is not her, not normal, they looked at us like we did not know what we were talking about! Dr did not listen, nurse staff different every shift with one exception (there was one nurse who actually cared but unfortunately Mom did not have her very much) They had a meeting on her progress and health that we never received the information on, we had questions, no one to ask, they could not even find her file one day. Aids to help when called could not speak proper english,(even a nurse admitted they needed to work on their english skills) Over 10 minutes to respond to take her to the restroom and when someone showed up they had no personality, just came dragging in did the job and left. One time they left her in her wheel chair by the bathroom door with the brakes on so she could not even move to reach the call button until someone stopped by again. Mom could barely eat with all the shaking and they would drop off her food in her room and leave (really? how can a 93 yr old person with one arm shaking so bad cut her food???) Horrible place, she got so weak she could not do her therapy so her insurance called it no progress and cancelled coverage. Midterm of her stay we tried unsuccessfully to move her to a different place. After two weeks wasted time with no one listening in we finally found out it was the pain medication causing all her problems, we really thought she was going to die. When Humana stopped the coverage we did move her to a new place. It also took 24 hours to discharge her from Augustana Chapel Hill of Hopkins which was ridiculous, I guess they wanted another paid date. At the new place It took another 5 days for her to completely withdraw from the pain meds that we finally found out was Oxycodone. (this drug was not discussed with us) The shaking and all other symptoms have now stopped and today 5-30-17 she walked almost 300 feet with a single handed walker. She is now paying out of pocket due to the lack of care received at Augustana. One star is to high of a rating but the activity person was great as well as the person in therapy who saw the problems and ordered a complete blood panel right before we got her moved. DO NOT PICK THIS PLACE FOR YOUR LOVED ONE'S AS YOU CANNOT STAY WITH THEM 24/7 TO MAKE SURE THEY DON'T MESS THEM UP. My sister also wrote a review so these are two separate honest reviews.

Actually the "problems" of zero follow up and concerns my Mother received is not even a one star! Beware, she went in with a fractured arm, but very mobile otherwise. She was there for only 11 days, and every day she was there a new symptom appeared, from skin sensitivity, hands shacking so bad she could hardly eat, to delirium, unable to really even stand up and of course because of all these things going on she could not do her therapy enough for her main insurance coverage, so they refused to cover her anymore, again 10 days! We had to be there every day to ask now what, drug side effects what? We decided we had to move her for all the blunders, (plus Humana was going to drop her coverage) and they even dated her discharged papers for 24 hours after her new facility approved her arrival! We were told it would take that long to get her papers 100% ready! I arrived the morning to question this 24 hour paper wait, to find out, they took her off a sleeping pill and extra pain pill, and excercised her legs for the first time, when I walked into her room, she was like a different person, no shaking hands, hardly zero sensitivity, she was laughing and chatting compared to crying and praying! So please really question yourself before making a "rash hurried hospital spur of the moment Humana covered facility" we will never go back nor refer this place to anyone!

I visited this facility
Augustana Chapel View was very nice and very clean. The meals were well prepared and seemed like they had a lot to offer. The staff was very nice. The gentleman that I met with was very nice, very receptive, and very willing to help. The rooms were all double rooms. Some people were sitting in the central area where they were having a news program and different readings. They had activities, an exercise room, occupational and physical therapy, and speech therapy.

I am a friend or relative of a resident
My mother was a patient there. The staff was compassionate and caring. The leadership was attentive to suggestions for improvement. The rooms are small and the amount of natural light was limited. The food was good.
