I am a friend or relative of resident

ad has been in a few different facilities so far since his illness and this was hands down the worst experience we have had with any of them. His clothing went missing regularly although they did replay him for new items after many weeks of complaints. There was some complication or another with his medication orders and supply daily and he had multiple incidents of medical issues (requiring emergency room stays) that, had the nursing staff been doing the required charting and well being checks would have been far less serious if caught earlier. The strict dietary restrictions he had weren't followed by the dietary staff (dessert being served to a diabetic, salt on the tray for a no sodium diet) and his blood sugar management was atrocious. He would tell the staff he was feeling like his blood sugars were low and it would take over 30 minutes to have his glucose levels checked. This happened at least weekly. By then he was so low it required emergency intervention that possibly could have been avoided had they responded in a timely manner. He was left in the shower (on strict orders for assistance when standing) for over an hour once and had to slam on the wall with a shampoo bottle until someone finally came. The room was not cleaned frequently and the staff regularly left messes and obstacles in the room which made moving around freely in his wheelchair impossible for him. What prompts this review though is his treatment upon his most recent week long stay in the hospital (which was caused by Avameres nursing staff not noticing a 40 pound weight gain in 35 days.) Upon being ready to return to Avamere his social worker at the hospital was informed that Avamere would not accept him back as a patient and would not give a reasonable reason as to why. Because of this he was forced to move to a facility over 30 minutes from us when Avamere was only 5 minutes away. A staff member "packed" (aka shoved everything he owned into garbage bags and a box) and locked it in the Director of Nursing office. There was no inventory completed nor was there a second staff present as a witness and we have not had a chance to go through them to check for missing items yet. We were unable to retrieve his belongings that weekend and had a terrible time trying to reach someone to set up a time to do so as the office was locked both times I was able to stop by. When we did get to speak with the DN he was extremely rude and disregarding to us, refusing to even look up from the papers in his hand while we were speaking and blowing us off stating we needed to talk to the administrator. After numerous complaints from dad they finally sent his belongings over with the maintenance guy almost two weeks later but it put a ton of unnecessary stress on him. The care received by a few of the staff was great but most were highly unprofessional and rudeness seems to be the norm. I regularly found the Nursing Assistants hanging out in a group by the front desk, eating and talking loudly about their patients or the management and their issues with them. I also can count on one hand the amount of times I witnessed a staff member wash their hands and had to ask that they did on numerous occassions. This was usually immediately after they had left another patients room and entered dads attempting to provide care. The absolute disregard for patient privacy was horrifying as I regularly overheard patients full names being used as well as detailed descriptions of their health and or behavioral issues (and this was by the Director of Nursing and management as frequently as the care staff!) Do yourself/family member a favor and skip Avamere. Unfortunately, good skilled nursing facilities are hard to find but I can wholeheartedly recommend you stay away from this one.

I visited this facility

My mother was at Avamere Rehabilitation. The food was excellent. The staff was extremely good. The rooms were the exact size and very clean. They had church services, music and bingo games. I would absolutely recommend them to other people. They're excellent.
