I am a past client of this provider

We hired Transitions LifeCare for my sister. They were absolutely fabulous. A nurse came in and assessed my sister. We met with a social worker and someone from a non-denominational religious service. They provided (what's included in the hospice care) an aide for an hour twice a week. The person that they sent was excellent. Everyone knew exactly what they were doing. Everyone was very caring and respectful. Obviously as a hospice, they are still in touch with me offering care and any kind of support that we need as a family. They are a non-profit, so I haven't received any bills. I don't believe I am going to receive any bills. Everything was fine with scheduling. If I needed them to come due to an emergency situation, over the weekend, or if I was concerned about something, someone was here within an hour. They were very responsive.

I am a current client of this provider

My dad is basically nonresponsive at this point. He can talk a little bit, but he can't communicate. He can't tell you if he wants something to eat or if he's going to the bathroom. He's in the end stages of vascular dementia, so he basically sleeps most of the day. He does get up in his chair for a little while, but he pretty much just nods off in his chair. He's starting to have sundowner's syndrome at night, so we're just trying to regulate his medication to keep him comfortable. He can't walk without assistance anymore. Transitions is changing some of his medications, because he can no longer swallow his pills. He's doing what they call "pocketing," so they're changing some of his medications into a powder or liquid form. They come in and bathe him a couple of times a week. They brought in a hospital bed, a wheelchair, and a walker to provide help with my mother. We have a nurse who comes once a week, a CNA who comes twice a week, a nurse practitioner we can call, and a doctor on call, so we don't have to take him out of the house. Transitions is absolutely amazing. They're wonderful, wonderful people. They have been so kind and willing to help in any way. Everything they've done has been beyond perfect. We're very satisfied.

My father was at Transitions LifeCare before he passed. He was there for four days. It was the best place; we can't say enough of good things about them. They were a great facility. They kept him comfortable, and they kept him clean. They were great to the family, especially to my mother. He wasn't in any pain during his stay, and it was just a great experience for us, having to go through all that with them.

We had Transitions LifeCare for my mother and they were absolutely first-class. The people there arranged and did everything for us from the transportation from the hospital to the facility. I think they took very good care of her, I stayed with her 24/7 as well. The people we worked with were consistent, so we had the same nurses. They knew my mother's condition and my condition, which was what I was most impressed with. The facilities was remarkable. It had very large rooms, had very comfortable sofas that folds out into beds, comfortable chairs and plenty of room for the family members. We had about 12 family members in the room all the time and they were very accommodating of that. Plus they had all the amenities outside for families that are from out of town including a kitchen, lounge areas, work areas, WiFi and everything. It was basically more exceptional than I had expected. We were all very impressed and very agreeable that it was the best option for mom at the time. I had no issues with the staff, they were very kind and gentle and very sympathetic to my mother and to the family. I could tell that they are highly-trained. The place is all in one floor, there's plenty of parking and the outskirts was a very pastoral setting, so it was a very nice and beautiful setting. Medicare paid for all of mom's expenses.

I am a past client of this provider

Transitions LifeCare was wonderful because they knew what they were doing. They came in and helped my mother who needed help (mom has passed away since). The rest of us although we loved her to death didn’t necessarily know what to do to make her more comfortable. They comforted her, maneuvered her into bed, and took care of some sores that had developed. They did what they had to do to make her feel comfortable. I would highly recommend Transitions LifeCare to anyone who's got a loved one in that condition.
