We provide peace of mind by helping your loved one remain safely at home, be independent, maintain dignity, improve self- care, avoid hospitalizations, and live their best life!  What is Personal Care? Sometimes, older adults or people recovering from an illness or injury need extra support and assistance with everyday activities. ComForCare Home Care offers need-based caregiving options to help you and the person you love live a happier, more productive life at home, for as long as possible. For each and every client, we work hard to determine the best possible match from our certified caregivers.  At ComForCare, we understand different clients require different types of care solutions. With flexible scheduling, custom care, caregiver matching based on needs and personality, and a genuine dedication to providing compassionate companion and elder care, you'll have peace of mind knowing your family member or friend is in good hands and getting the best care possible.  Personal care services from ComForCare are available 24 hours a day, seven days a week, including holidays. Our services include: Assistance with activities of daily living (ADLs), including: bathing, dressing, and toileting, help with mobility, including standing, walking, and getting in and out of bed, light housekeeping, meal planning and preparation, medication reminders and transportation.  Companion Care  Our Companion Care services include: participating in meaningful activities that allow older adults to have human connections and stay mentally sharp. Caregivers can play card games, read novels or magazines, listen to music, or just chat with seniors to brighten their days. They can also provide transportation to activities, appointments, grocery shopping and other outings.  Transitions of Care When you or a loved one is released from a hospital, rehabilitation center, surgical center or nursing home, ComForCare can help you transition back home and support you while you heal. We offer:   Communication with your family, discharge planner or other health care professional  Transportation home with stops to pick up medications or groceries  A call to let your family know you are home safe  Medication reminders  Help with meal preparation, light housekeeping, and laundry  A fall risk and home safety check  Help with exercises and walking  Any additional services as needed Living in a Senior Community? ComForCare services are available wherever and whenever you need them.  We can provide our personal and companion services to your loved one if they need a little additional support while they live in their Independent Living, Assisted Living and Memory Care Community. We are available 24/7 for support.  Hospice Support ComForCare is available to provide hospice support.  We have a team of sensitive and experienced caregivers who can be there for you and your loved one. We can help with bathing and personal hygiene, prepare meals, do lighthousekeeping and provide care and companionship to your loved one while also supporting the family during this difficult time.  ComForCare is known for its award winning programs including DementiaWise®  and Gaitway Our DementiaWise® Program  DementiaWise® is an occupation-based, team-centered training program, which serves to help home care teams resolve dementia-related challenges collaboratively with clients and their families. Agencies and their teams complete DementiaWise video training modules to learn about Alzheimer’s disease, vascular dementia, Lewy body dementia, frontotemporal dementia, and other forms of dementia. With an integrated essentiaALZ® exam provided by the Alzheimer’s Association, these modules both enhance and review our franchise owners’ dementia care expertise. Gaitway TM Program ComForCare provides care to individuals of all ages, however, the primary client typically has a ‘homebound’ status and over the age of 65 that presents greater risk for falls.
