Caregivers can play card games, read novels or magazines, listen to music, or just chat with seniors to brighten their days. They can also provide transportation to activities, appointments, grocery shopping and other outings.  Transitions of Care When you or a loved one is released from a hospital, rehabilitation center, surgical center or nursing home, ComForCare can help you transition back home and support you while you heal. Agencies and their teams complete DementiaWise video training modules to learn about Alzheimer’s disease, vascular dementia, Lewy body dementia, frontotemporal dementia, and other forms of dementia. Gaitway TM Program ComForCare provides care to individuals of all ages, however, the primary client typically has a ‘homebound’ status and over the age of 65 that presents greater risk for falls. This can be life altering in many ways. In response to this, ComForCare has developed the Gaitway TM   Fall Risk Management program.
