My mom came to Nazareth following a long hospital stay and hip surgery. This was my/our first interaction with any kind of skilled nursing facility and I was nervous about the kind of care and facilities where my mom would be placed. I have been so amazed by the graciousness and patience of all the staff I have interacted with. They show all the residents compassion, make them smile and laugh, and care for their needs. I feel very lucky that my mom ended up here.

I have been an inpatient twice this year due to surgery for a hip replacement. I found the staff very caring and supportive. They were knowledgeable and concerned about me. The therapist helped me go from unable to stand to walking with a walker. My case presented a difficult course and they worked hard to help me over come several complications. They made a difference in life.

I was a patient there for about two months. Staff was wonderful and nurses very caring. I ‘d send my loved ones to this facility. Food pretty good if sometimes cold. Work on telling a woman if she is sharing her bathroom with a man. Work on placing a loud patient on a more appropriate floor. Room small but recliner and bed comfortable. Excellent service with fresh ice water even at late night times.

My grandmother is currently in this facility and is receiving horrible care. When I was there to visit my grandmother was laying in urine, bed wet clothes soaked all the way through the mattress. I wonder if me and my mom wouldn’t have asked to have her moved up on the bed if they would have even checked on her. She is not being given the care she is needed and this will be reported. The room hadn’t been cleaned, the patient next to her also was laying in urine.. Unacceptable!!

Both my mom and dad were there for a very short few months and the caregivers were great with them. They didn't mind being there at all. It wasn't home, but they both knew they couldn't be anywhere else before there passing. They remembered me coming every day to see them and knew I knew the drill of what to do. They would chat and be very respectable to everyone that they had seen. Just giving the workers a big thumbs up because without them the people living there wouldn't have the care that they need. I know my mom played bingo, the room was nice and big for the both of them. They had a tv in their room and could bring in some furniture if they needed to.

I have made referrals to this facility and visited them as well. This Skilled Nursing home has an aggressive marketing and admission staff. They are quick to respond to patient needs for placement. The referral process is quick and easy. They are a for profit organization. It's an older building, with parts recently remodeled. The rooms are average sized. Halls are wide. The building itself is off the beat and path about 1/2 mile from the Main Street shops/Senior Center. The building is next door to St Ann's Catholic Church and was formerly operated by the nuns. The Administrator, Director of Nursing and Director of Admissions are all new to their roles in the past 2 years. This is a large part of the Leadership Team and that much change on the leadership team could be really positive or a mark of some problems. Rehab is a focus of this skilled home. They also have a floor that is not considered a dementia unit but operates similar to one in that they place their patients with memory and behavior issues on this floor. They offer a variety of activities that are at appropriate levels for patients. During their most recent State Health Inspection they received 13 deficiencies. The average number of deficiencies of a SNF in Wisconsin is 9.
