I visited this facility

We visited Harbor House while in the Kenosha region looking at assisted living facilities to consider for our uncle. The outside of the building and exterior lawns appeared very well maintained. We did not have an appointment, yet the staff was accommodating and took us on a tour anyway. They can accommodate a variety of ambulation levels and as needs change, the care team adjusts accordingly. This facility had many communal rooms for activities ranging from reading to card games, exercise and many other stimulating things to do. The exercise room included regular exercise classes and well maintained weight training, bike and treadmill equipment. Personal assistance is available any time needed. The menus had healthy food options for all kinds of diets. Healthy snacks are offered often and whenever residents request them. The dining room was very nice, with plenty of space. The food we saw looked and smelled delicious and was presented in an appetizing way. The private living spaces were nicely designed with beautiful decorations with lots of fresh air and light. The seniors may easily navigate nicely landscaped tree-lined walkways.There was a large auditorium for parties and group events. The outside activities feature many interesting cultural events in Kenosha and the surrounding areas, like art, films and many other interesting cultural gatherings. This living environment presents mentally and physically stimulating activities for seniors including everything they need, with competent and caring staff.

This is a rather unique assisted living facility that is actually composed of three separate buildings, all caring for individuals with different stages/manifestations of dementia. It is located off of the main road on the outskirts of the Wheeling and Buffalo Grove business districts, and backs up to a quiet, safe residential area. There's plenty of parking and it is a short walk to any of the three small "houses" that each contain assisted living rooms, all of which are the size of small studio apartments. Each house is configured in a square/rectangle of rooms around an open center, with a dining and activity area at one end of the building, and a small enclosed outdoor patio in the middle. Additionally there is the real bonus of having a huge "yard" that is the backyard of all three houses, which is fully fenced in. This allows residents to go outside and safely walk a decent distance, without risk of eloping from the property, unless of course they climbed the high wooden fence. The common areas inside each house are not enormous, but they are sufficient to meet the needs of the residents. Families have the choice of visiting in the common living room, which is less private, or in the resident room, or on good weather days, outdoors. The food is average to above average and most residents and visitors have no complains. The staffing, meaning number of staff available, is average with a lower than average rate of turnover. All staff are kind and caring, and because the facility is small, able to provide a family-like feeling to the care they provide. Harbor House mission is to provide care for "atypical dementia" better than other facilities can. Often they succeed. The private pay rate is reasonable and well within industry standards.

Harbor House and it's staff are the life line I needed. I used them for respite for my mother and now she is there as a full time resident. The smaller size maks it more home like and the staff takes care of each resident as if they were there own family.

What ongoing training does the staff receive in dementia care?

At Azura Memory Care we recognize that those within our care deserve the highest quality of service, which is why we have invested in creating an exclusive training and engagement program called MOSAIC. MOSAIC is used to ensure that every team member working at Azura understands what is dementia and the symptoms of the different types of this disease process such as Alzheimer’s disease, Lewy Body, Frontotemporal, and other related dementias. In addition, MOSAIC provides hands-on training that provides insight into the proper approach and communication techniques that will allow our Team to be most successful in engaging those within our family of care to continue living life to the fullest. In the first year our Team receives over 90 hours of training and has over 30 hours of ongoing education each year. Azura was also the first provider to require the Virtual Dementia Tour, a sensory experience that helps one walk in the shoes of those with memory loss.

How many staffers are on duty in the daytime and overnight?

We pride ourselves in offering a higher than average staffing ratio, which enables our Team to engage with residents and spend time making their personalized joys a reality. In addition, at Azura Memory Care we monitor the acuity of each resident and of the overall home to ensure that as the needs of our residents change we are equipping them with the supports they need to maintain quality of life and quality of care.

What programs or activities does Azura Memory Care of Kenosha South offer to keep residents engaged? How does the staff engage residents' long-term memories?

Thanks to Azura’s exclusive MOSAIC training and engagement program we offer an extensive array of activities to engage our residents socially, mentally, emotionally, physically and spiritually. Our goal is to help our residents maintain a high quality of life. One way we do this is by collecting an extensive Quality of Life Profile for each resident from their loved ones. This profile is then used to personalize their care plan and other tools that our Team uses to stimulate residents in conversations or activities that personally bring them joy. We also take our residents out into the community to local restaurants, concerts, farmers markets or any place that will make them smile! We also tap into their personal desires through our MOSAIC Dreams program, which asks what our residents best day would be and then works to make that day come true. This program has enabled residents to go on helicopter rides, fishing trips, ice skating, Brewers games and to even meet an elephant!

Who assesses residents' health and cognitive functioning? How often is that assessment repeated?

At Azura Memory Care we look at each resident as an individual and deserving of a very personalized approach, plan of care and daily smile. Thanks to the extensive knowledge of our three-pronged interdisplinary team, all assessments required are done by our team at Azura. These assessments are done per code, but also as needed to ensure that all aspects of our resident’s health are monitored in real-time and addressed in efficient and effective manner.

What techniques does Azura Memory Care of Kenosha South use to support memory care residents during mealtimes?

Our MOSAIC Dining experience ensures that those within our care are receiving delicious and nutritious food that has been selected with their needs specifically in mind. We first tap into the normal likes of our residents and then using a team of dietitians created a menu that is both dementia-specific and mouthwatering! In addition, we utilize a special approach to dining that includes our caregivers sitting down and eating with our residents. Thereby utilizing their expertise to help cue, monitor and inspire conversations and socialization around the dinner table. We have also utilized research on best practices in dementia that include proper table settings to ensure space and tool recognition, adjustable heights in tables and red plates to increase appetite and help with visualization of food.
