At Azura we work very hard to address any issues in a timely manner, but as stated in the review, this perception of Azura is more than 2 years old. Many things have changed during that time, as we are always looking for ways to improve and implementing these enhancements. In addition, HIPPA doesn’t always allow us to communicate issues to all involved in the love and care of an individual. However, whenever a family or anyone has a concern our Azura Team is always willing to listen, address the issue and move forward to ensure the care of our residents. In addition, we have an Azura Connections system that allows for anyone to call, email, fax or send in a comment of concern or praise. We wish this process had been used, instead of reporting it 2 years after the perceived issues occurred. Thank you.

We apologize for your frustration and understand your need to share with others. Please let us take a moment to try to explain our thoughts as well. Azura Memory Care is very committed to the overall wellbeing of our residents and specialize in helping those with memory loss and advancing dementia. While our current policy does allow supervised smoking in designated areas, we have found that smoking, even with assistance, is not a long-term possibility for those with memory loss as they begin to forget how to properly use the cigarette or other smoking material in a safe manner. Therefore, we have found great success in helping residents transition to an e-cigarette, which initially helps with their oral and nicotine needs. The beauty of an e-cigarette is that it slowly wears down the nicotine and weans the person off of that need and eventually their oral need dissipates as well. This is much less taxing on a person than asking them to stop, which could involve serious withdrawal effects often exacerbated by the dementia disease process. This is why we encourage families to help those with memory loss who are smoking switch to e-cigarette as soon as possible. In addition, a face-to-face assessment is always part of the Azura plan of care, which is why we ask for the ability to do an in-person assessment early in the conversation with the family. This assessment gives us a more clear picture of what is actually happening with the individual and if our care would fit their needs. It is rare, but there are times when the assessment finds that we would not be a good match for the individual. If that occurs we immediately notify the family and try to offer other options to them. Again we apologize that we weren’t able to help your loved one at this time.

What ongoing training does the staff receive in dementia care?
At Azura Memory Care we recognize that those within our care deserve the highest quality of service, which is why we have invested in creating an exclusive training and engagement program called MOSAIC. MOSAIC is used to ensure that every team member working at Azura understands what is dementia and the symptoms of the different types of this disease process such as Alzheimer’s disease, Lewy Body, Frontotemporal, and other related dementias. In addition, MOSAIC provides hands-on training that provides insight into the proper approach and communication techniques that will allow our Team to be most successful in engaging those within our family of care to continue living life to the fullest. In the first year our Team receives over 90 hours of training and has over 30 hours of ongoing education each year. Azura was also the first provider to require the Virtual Dementia Tour, a sensory experience that helps one walk in the shoes of those with memory loss.

We pride ourselves in offering a higher than average staffing ratio, which enables our Team to engage with residents and spend time making their personalized joys a reality. In addition, at Azura Memory Care we monitor the acuity of each resident and of the overall home to ensure that as the needs of our residents change we are equipping them with the supports they need to maintain quality of life and quality of care.
What programs or activities does Azura Memory Care of Kenosha North offer to keep residents engaged? How does the staff engage residents' long-term memories?
Thanks to Azura’s exclusive MOSAIC training and engagement program we offer an extensive array of activities to engage our residents socially, mentally, emotionally, physically and spiritually. Our goal is to help our residents maintain a high quality of life. One way we do this is by collecting an extensive Quality of Life Profile for each resident from their loved ones. This profile is then used to personalize their care plan and other tools that our Team uses to stimulate residents in conversations or activities that personally bring them joy. We also take our residents out into the community to local restaurants, concerts, farmers markets or any place that will make them smile! We also tap into their personal desires through our MOSAIC Dreams program, which asks what our residents best day would be and then works to make that day come true. This program has enabled residents to go on helicopter rides, fishing trips, ice skating, Brewers games and to even meet an elephant!
Who assesses residents' health and cognitive functioning? How often is that assessment repeated?
At Azura Memory Care we look at each resident as an individual and deserving of a very personalized approach, plan of care and daily smile. Thanks to the extensive knowledge of our three-pronged interdisplinary team, all assessments required are done by our team at Azura. These assessments are done per code, but also as needed to ensure that all aspects of our resident’s health are monitored in real-time and addressed in efficient and effective manner.
What techniques does Azura Memory Care of Kenosha North use to support memory care residents during mealtimes?
Our MOSAIC Dining experience ensures that those within our care are receiving delicious and nutritious food that has been selected with their needs specifically in mind. We first tap into the normal likes of our residents and then using a team of dietitians created a menu that is both dementia-specific and mouthwatering! In addition, we utilize a special approach to dining that includes our caregivers sitting down and eating with our residents. Thereby utilizing their expertise to help cue, monitor and inspire conversations and socialization around the dinner table. We have also utilized research on best practices in dementia that include proper table settings to ensure space and tool recognition, adjustable heights in tables and red plates to increase appetite and help with visualization of food.
