My brother stayed at the nursing center. As his POC I dealt with many of the employees there. I had one issue that required assistance from the staff to help me resolve and the Director of Social Services at Fairfax Nursing helped me to get it resolved and also to put my mind at ease. I think Dir of Social Services does an incredible job for Fairfax Nursing Center.

I cannot even begin to describe this awful place. It is dirty, run down, STINKS, staff is rude, and there for the wrong reasons. Medication is delayed, I would not feed the food to my dog, PT is non-existent. Please. Do NOT bring your loved ones here. This is no place to rehab or place anyone for long term care.

I'm not even sure where to start. The care- what care? The nursing staff is minimal, overworked, all doing double shifts There are like 25 openings for the building if you look on Indeed. The staff seem tired, and grumpy. My mom's aid took care of 18 others, we never saw her. The building is old, dirty, the floors are not clean or shine The food- oh my gosh. When you complain that it's cold or tasteless, they do nothing. We had an awful experience and do not suggest anyone allow their loved one to go here. It used to have a A+ reputation, but the Nurse said it sold to a corporation and keeps going down hill.

My father was on the 2nd floor in this facility for 12 days of 'rehab' for multiple cracked ribs. Although a few staff members were pleasant and responsive, overall the care and service were abominable. Prior to rehab, my father was able to walk and fully continent. During the 12 days of 'rehab,' PT got him out of bed exactly ONCE to take 3 tiny steps. Otherwise, all PT/OT/ST was conducted in his bed. All toileting occurred IN BED. My father was dressed in a hospital gown and a diaper for 12 days, with one exception when an energetic CNA responded to my wish to get him up and dressed, in a wheelchair, and outside for one hour. Thank the Lord for that CNA. How is this approach to care rehabilitative for physical, social, and emotional functioning? Not at all!! My father developed a deep tissue injury and a testicular rash. Staff did not notice the rash - our outside hired help pointed it out. At that time, we were told that the Nurse Practitioner would look at it the next day. The next day?? Why not that day? It then took 2 more days for a treatment to be prescribed and applied. Either a family member or outside hired help was with Dad every day for at least 8 hours. We had to remind staff to apply this cream. Had we or our hired outside help not been there, I am not confident at all that the treatment would have been offered. Exactly NO PT/OT/ST was offered on the weekends. I believe Medicare requires these services to be offered 6 days/week. Not in this facility. In addition to this awful medical/rehabilitative care, the service generally was terrible, as other reviews have noted. A few staff members tried to be responsive, but many times our calls to staff went unanswered (phones were transferred to other lines, message boxes were full, or phones just stopped ringing and were disconnected). Our request for additional rehab devices to prevent falls (e.g., lowered bed, small guard rails at the top of the bed, etc.) was never addressed. We saw one physician on the last day. Otherwise, care was overseen by Nurse Practitioners who were not around very often. During the minimum 8 hours/day that we or outside help were at the facility, we never observed any staff member attempt to bathe our father, help him brush his teeth, or assist him in using a breathing treatment recommended for covid recovery. After multiple questions about how this treatment was to be administered, one nurse said that the apparatus should be left out on his tray so he would remember to use it. What? My father has memory issues and was recovering from multiple medical issues, lying in bed all day and night - how is he supposed to be responsible for administering his own treatment? We were told that his 'shower days' were Weds. and Sat. He arrived on a Weds., apparently too late for a shower. On Sat. at 9:00 AM, I asked who would be responsible for doing this - I did not get a clear answer. None of it had been done by 1:00 when I left for a lunch break - I reminded the nurse, but when I returned at 4:00, still nothing done. The CNA at that time was a kind person who did finally shave him after I asked again a few times. When food was delivered, the tray was generally set down and the aide left. No one helped open containers, position food so Dad could reach it, etc. We had to continually remind staff that our father had multiple cracked ribs. On many occasions, even after reminders, from the hallway we would hear him yelling in pain as he was being changed or otherwise attended to. Many aides were just not sensitive to the pain he experienced. When it was time to move him to another facility, phone calls and faxes went unanswered on a Friday - and by end of the day, we were told that no discharges could occur over the weekend - so the transfer happened on Monday. I write all of this not because I think it will do anything to change the care offered in this facility, but to help family members trying to decide on a facility for their loved one. This is NOT the place to send someone you love for rehab.

My Aunt didn't care for the food at all. She said she didn't see her nurse aides very often. The building was old. The staff were not happy to be there. A lot that could be improved- even to the non-medical visitor. Basic business practices!

Crowded, dirty, overworked AND burned out staff. My friend was discharged to here from an HCA facility - Reston Hospital. The discharge planner promoted this place as "five star". Indeed there is a banner in front of the facility which brags about its Newsweek high rating. I looked up that rating and discovered it is rated "average" in the most important care areas. The food was poor, the lack of professional and assistive staff was obvious; people calling "nurse, nurse" from their rooms, incontinence because there wasn't enough staff to respond in a timely fashion to requests for toileting. 4 men sharing one bathroom. Inadequate shower facilities. My friend reported very little rehabilitation evaluation and treatment. In some small rooms - 4 residents crammed in - are these medicaid patients? I presume this facilty - owned by Innovative Healthcare of Richmond, VA - is mainly interested in billing Medicare and Medicaid for marginal services. STAY WAY FROM THIS FACILITY.

My step mom was here. Awful! Not nearly enough caregivers or nurses. It's like a ghost town when you visit. Nothing can be said to make one think it's a positive experience here- Go elsewhere if you can.

My step mother was here in early 2022. Care was lacking, she rarely saw her caregiver. She was left in soaked diapers. This place needs help!

I visited this facility

I found the staff to be lacking, with many of them saying since the facility sold, the staffing has decreased drastically. The few staff I spoke with complained about many things including quality of supplies, quality of food and staff numbers. I went to the CMS website at the direction of the medication nurse and found the facility had a 5 star staffing rating, but is now a 2 Star Lots of interesting info on that website Do your research!

I visited this facility

Fairfax Nursing Center looked pretty nice. The people seemed happy and nice. It's clean and orderly. We only met with an admissions person who's actually new to the job, but helpful and friendly. With COVID restrictions, they weren't having any group meals. It looked like they had a nice gym area. The reviews and the government statistics show that they provide a pretty good level of care. There's a low rate of people with bedsores, so they're attentive to that, and the ratio of staff to patients is pretty good. The patients and visitors that we saw looked happy and they were enjoying themselves, so that's a good sign.

I visited this facility

Not enough staff for the number of patients on second floor My sister is a Hospice patient there receiving palliative care. Her needs are pain medication, bedridden so changing and these needs were not met many times. Talked to supervisors regularly who promise to remedy it but waiting for too long to receive prescribed medication and diaper changing continued. Ring the call button and the aide comes, turns off the call light and either forgets to tell nurse or nurse is told and is doing paperwork so she forgets. Highly recommend you do not use this facility for a Hospice patient

Fairfax Nursing Center is not the worst, but it's got a lot of problems. Ever since they went from a family-owned, kind of privately owned, to selling out to the big people, it's a problem. It's an old building in Fairfax. What they offer and what they do are kind of two different things. They claim they have all the things that you need, but then they tell you to bring this or bring that or they don't have some of the supplies. You have to continually stay on top of them or whatever. You have a social worker that doesn't do anything, he has no purpose. He's supposed to advocate for my mother but he does not. My mom's been there since July and they're on their third administrator. It's just wrong. We're trying to give them a chance to correct their poor behavior. There is one person there and only one person there, and that is the hospital liaison, that I trust. She has done what they said they're going to do, got back to me with the information, and has done things that they didn't even have to do. She did all that and my sister and my mom are very appreciative of her. There's a couple of staff members there too, a couple of CNAs and one or two nurses, that we are very appreciative of, as well. There are just so many other things that are a problem and they need to get themselves together, plain and simple. They're short-staffed, which is not my mother's problem or fault. Covid got rid of a lot of people and I understand that. I've seen a meal, my sister has probably seen more meals, and sometimes their dinner looks like it's a small child's lunch and not so much an actual meal for a grown adult. My mom is at the end of the hallway and a lot of times, hers and her roommate's food is not hot by the time it reaches them. Medicare is paying and I have no physical proof or evidence, but I'm sure that they're billing Medicare for things that are not needed or that they're not doing. I wouldn't send anybody there. I wouldn't recommend anybody to go there.

I visited this facility

I would give Fairfax Nursing Home a high rating and that would be my first choice for my mother. I just noticed everyone seemed happier there at the time of my visit. They had a little breakfast room that had nice and comfortable furniture. Their rooms seemed to be spacious. We spoke to the person who gave us the tour and she was very good. What I like best about this place was that it did not smell.

I am the husband of a patient with late stage Alzheimer's. I am satified with the care she receives which includes careful attention to what she eats, periodic reviews of her medications, and appropriate physical therapy. I am personally made to feel welcome when I visit.
