I am a friend or relative of a current/past resident

The first night my mother was in this facility I went in to check on her after visiting hours and found her laying on the floor in her bedroom. The nurses were all at the nurses station talking and chatting it up. No telling how long she had been laying on the floor. This happened two more times within the following weeks. All communication with staff and directors is minimal at best. They overcharged me the whole entire time my mother was in this facility. Then they reported me to Adult Protective Services after they evicted her from their care. I just recently received another bill for $6,000 plus. If you're looking for care for your family members please look elsewhere. This facility may look nice, but like they say don't judge a book by its cover. Beware!

I disliked Park Place Care Center. I would give them very bad ratings. The cleanliness was a zero. My friend stayed there for four months. He stayed there the longest. The whole people in the memory care got bedbugs. I told them they were bedbugs and they said, 'No, they're cleaning.' It always smelled, and I go there and there would be no sheets on the bed or things wouldn't be clean. And when you ask for them to do something in their room, nothing would be done. I saw that they did activities for the non-memory care people that were there. It was a very bad experience for memory care. I thought the staff that worked with my friend seemed to care and would go out of their way. But the administrative and the upper staff were not very good. I didn't feel like they did any activities. I didn't see them doing anything with them. For value for money, I thought they were too high for what they did. If you were paying cash and you were not on a program, I don't understand why anybody would take a loved one there.

My mother was in rehab at Park Place Care Center for just a few weeks. The staff was very nice and very helpful, and they worked hard to make her as comfortable as possible. You had to make reservations ahead of time to see your loved one, but that was no problem as long as you made reservations ahead of time. They were very nice and accommodating. This facility was decent. She did not like the food, but she was very picky, and they had problems with the staff in the kitchen. It's expensive.

This used to be a good facility but since being bought by Chosen Healthcare the quality of care has fallen drastically. The administrative turnover is ridiculous, with new managers never lasting more than 1 year. Its seems Chosen bought a well run and managed facility but it now squeezing profits from Park Place without little concern for the patients well or competent administration. I say stay away.

I am undecided at this point-relative has been there two months and the rate of staff turnover is not what I would have expected. Activities Director, Business Office manager, DON and Assistant DON have all turned over. And those are only the ones I know. Staff is rather stand offish, promised returned phone calls never happen.

My husband is in Park Place Care Center now. The staff is very attentive and caring. When I go visit, they talk to me about him. I'm happy with their services. Some days, he says the food is really good, and other days it's really bad. He's gone outside for a bible study and they have some other activities outside that he goes to. They have dominos and movies, and all kinds of things going on. He's kind of almost bedridden, so he doesn't get up for very many of them, but he has gone to some. It is homey and it's clean.

When one of your parents is in trouble and you quickly have to find a place for them to receive in-house rehab, physical and occupational therapy, you are naturally concerned you are choosing the right place for them. After a significant health scare last month, my Dad and I found ourselves in just that position. My Mom was hospitalized - then needed continued help, and fast. She was accepted to Park Place Care Center, and my Dad and I took her there ourselves. Having not been to the facility, my Dad and I were worried we had made the right choice. We could not have been more pleased. From the second we walked in the door, every single person who worked there smiled at us and said hello. They made us all feel welcome and comfortable with the situation. They immediately made my Mom comfortable in her room, and then asked all the right questions of us to allow us to be part of her care journey there. There was not a weak link in the chain of care at Park Place: from the quality nurses, to the nutritionist who took a great deal of time to find out what Mom would really like for meals, to the facility staff, to the amazing physical therapy team, we could not have been happier with her care. Because of the diligence of the entire team at Park Place, and the uplifting and supportive way they treated Mom, she was able to quickly get back on her feet, and was home in less than 2 weeks. They even inspired her to continue her physical therapy and rehab on her own, once she got home. And she is actually looking forward to it now! The facility is under new management, and although it appears that the old management team was lacking, the new Park Place team is certainly not. This new Park Place team is really great - and though the facility is not fancy or high end - it is super clean and full of people who truly care about their patients. THAT is what Dad and I wished for Mom - a safe, clean, caring place for Mom to regain her strength. Thanks to their hard work at Park Place, we got much more than we ever hoped.

I had my mother at Park Place for her rehab for two or three months, and it went pretty well. It was very good. They had very good exercises for her. I had tasted their food, and it was pretty good. Their dining area was really nice. They had a nice beauty shop facility, so my mom could have her hair done. The entrance area was very nice and big. A lot of the people sat around in the entrance area, so there was some communications going on between patients, and I liked that. The rooms, of course, were dated. It was an older facility, but it was clean. It didn't smell bad or anything like that. However, they needed more room for the patients so they don't have to share a room. The patient to nurse ratio was pretty good. There was always somebody around. I liked that they had a central nursing area that you could go to if you needed anything. They took good care of her there. The staff was wonderful and very caring. If they noticed anything, they would give her immediate care. I felt that they all seemed to be enjoying what they do. So, I felt that was a very good place for her, and I visited her almost every day. They had a big exercise room, and they had exercises classes every day. I thought it was actually pretty good. I felt pretty good about the security, that they were watching everybody and made sure that nobody was hurt.

My 37 year old son was admitted to Park Place due to a brain injury. He received the very best of care and has recovered fully due to the work the therapist and staff has done. He said that the staff really cared about all patients and treated them with dignity.

I am a friend or relative of a resident

My mom is in Park Place Care Center. At the moment, she is in their rehab facility, but she'll be going to their nursing home care eventually. We chose this one because it's in the same hometown and it is close to two of my siblings. Also my mom had been there before. It's a very clean nursing home. I like the personnel there. We were really pleased with them during our first interview. Right now, the staff has been very good to my mom. Also their rehab has been very good too. There are activities which are listed in their calendar. My mother's memory has prevented her from doing some, but we're working on with that. I have eaten with my mother before and the food is good. The only complaint I have is that it's a little noisy in their cafeteria when they're taking the dishes away. Their rooms are good and kept really clean. They have views outside their large windows.

We had our mother in Park Place two different times. The first time she was in Park Place, she was there for rehab and was not that happy because she was anxious to get back home. The final time we had mom there, she was declining rapidly and we knew she would not be going back home. The staff was very caring - especially one nurses aide who took special care to help mom with her hair and would apply her make-up. They worked very closely with the Hospice organization we had working with us and we appreciated how thoughtful they were during her last few days.
