I had a family member in this facility for some time. The last time I visited, my family member had what I assumed was food stuck in her mouth and was basically gagging on it. I went to the nurse desk and she said she tried to do the mouth care earlier in the day and the patient closed her mouth, so she couldn't do it. A couple of hours later, she came by and tried again, successfully this time. Something I considered urgent wasn't taken seriously, and essentially ignored for a couple of hours. I was left with a strong impression that she didn't care.

I was in this rehab. center and was put in quarantine for two weeks due to COVID 19. I had been here at a previous time but was not as bad. I can not walk by myself, the help would stay out in the hallway talking and would not help the patients. The night shift was the worst. I would lay in a wet bed with urine and no one would help me. My wife talked to the office but nothing happened to correct the problem. Being in quarantine, they would not let my wife in to help me. My false teeth were on my food tray and was thrown away and they refuse to pay for them. A very neglectful place and feel sorry for others in that situation. Don’t put anyone there.

Please do not send anyone you love here. Staff is rude and lost at all times. Place seems dirty Show yourself and family members more respect. Send somewhere else.

My Mother has been a patient at Sandy Lake Rehab for some time. I have always enjoyed the activities that they provide the residents.. Barbra was the activity director for several years; however, she is no longer there and they replaced her with this new activity director called Denise . I was in the dinning room and I noticed she was talking to a resident in a sarcastic and degrading manner. That resident has such severe memory problems and was having a hard time getting up to the table for Bingo. She laughed at the resident saying " This Must Be Just Hard For You". I was shocked when I saw the evil smile and her tone of voice. After that I went to the Administrator John Wilson and he brushed me off. Volks; when an Activity Director acts in such rude and uncaring manner towards the residents, I do not want to know what the CNAs do when I am not there. I do not feel confident in this place and Looking actively to transfer my mother to another place who is caring and the administrator actually does help to resolve issues.

For the amount that this facility charges for a small semi-private room, $180 per day, Oh, by the way 2 rate increases in 5 months. You should be able to expect better service. We have met with and expressed concerns about some services that my disabled sister needs, that are not being done consistently, are always told they would "get right on that". However, things never change, same old hit and miss. Excuses that the helper was new ( and obviously not trained well) or it was a weekend person are NOT acceptable. They still charge the same rate. Our family and paid outside caregivers are there 4-5 times a week. This may be a nice looking place but the service is below par. Keep looking until you find one that is much more attentive to their residents. Wherever you go remember that you ALWAYS have to be checking on your loved one, no matter where they are.

Sandy Lake is a great place, my father in law was there and he loved it, the staff is great and very caring. Everyone there is always smiling and always trying to be helpful to residents and family members, I would recommend this facility to everyone!

If you're looking for an in patient rehab look no further. Prior to my surgery I've never had a need for in patient therapy and wasn't sure what to expect. Thanks to the outstanding therapy staff I made it home just in time to celebrate Christmas with my family. Thank you Sandy Lake for the great care.

Check QRS(space)DADS.com to check ratings on any place you are considering. This place only had a 63 out of 100 but we didn't know about the site beforehand. There were medication problems twice that we caught. Housekeeping was poor with sticky floors and sticky rolling trays. One day I came in at lunch and found my mother-in-law's upper plate out of her mouth with adhesive still attached and dropped in a dirty glass on the (sticky) rolling tray. No aide or nurse to be found so I cleaned it myself and put in a cup with cleaner. Before she died, my MIL ended up with incontinence sores and the CNO reasoned that she came into the facility with a pink spot and wasn't eating well so the skin quality suffers. She made it sound like it was the resident's fault she got skin sores. Her glasses were always dirty and they lost one of her hearing aids. We were there every single day and her care was still not enough. The fact that this place has been through two bankruptcies has left staff thinking that subpar work is acceptable and allowed. The few that actually care just can't keep up with the work required. Do yourself a favor and go elsewhere.

Sandy Lake has come a long way. My father has been there since they opened 4 years ago. As of July they have new ownership and management. The new management has taken the issues on with gusto. The Administrator and DON work well as a team and have worked very hard to make my father's stay pleasant. There are always a few bad staff members but the Administrator has worked to weed them out.

Sandy Lake was good as well. There are difficulties given the circumstance, but overall it was a good experience. The staff who accommodated me partly were good, and part of it were not. My mom had really good care and some of the nurses were not as good. It was a newer facility, and the rooms were bigger, but to be honest I think she got the other place. It was bright, new, and it was cheerful.

This place is straight out of a hideous horror story. The full story is so gruesome that we will let you imagine the worst possible scenario. We will tell you that when our loved one passed away, they did not notify the family. The funeral home called to inform us that they now had our loved one and to offer their condolences. We were traumatized, especially after weeks of similar abuse. If you value your loved ones, DO NOT put them in Sandy Lake Rehabilitation and Care Center!!!

I have one thing to say about this place.....RUN THE OTHER WAY!! This place should be closed down! I cannt say anything good about Sandy Lake except the building looks good from the street!! The care there is below standard and staff had no clue what the next person was doing just cared about the money they were getting from the state for rehab!

Sandy Lake Rehab was always clean and the furnishings were very nice. The food was adequate. The staff was courteous and knowledgeable. My Dad has been in several rehab facilities and with this one he made the most progress. I highly recommend their rehab services. However, they seem to be understaffed in the nursing department. The length of response time to a call for help was sometimes unacceptable. Overall, we were pleased with this facility and plan to return there when my Dad is well enough.
