I visited this facility

Bridgecreek Memory Care was beautiful. They were in a great setting. There were lots of natural light and windows. The residents were freely roaming the entire interior of the premises. Jodie, the director, was very personable and informative. She answered lots of questions and knew every resident we passed by name. The facility was kept well. The nursing staff was friendly and welcoming. The one thing that sadly we did not get to do was go into the courtyard. We didn't tour the exterior of the facility and the yard that the residents have access to, because of the weather, but one thing that was a bit of a downfall for us was its proximity to the family's location. It was 45 miles from our home. The resident's rooms were shared rooms with Jack and Jill bathrooms. There were no private ones available at the time that we toured. They had two different open-style dining areas that the residents were able to choose from. There were CNAs and orderlies available during lunchtime to help the residents that were unable to freely feed themselves, and those that were independent enough to eat were allowed to do so at their own pace. They had an activity schedule posted with multiple activities available throughout the day. They had guests that frequented. There was animal therapy available and gardening activities. There were multiple areas throughout the facility where residents were able to congregate, socialize, and watch TV at their leisure. There was a communal kitchen area where residents were able to help themselves to snacks and have a sense of independence. The staff was friendly and engaging. Every staff member addressed each resident by name.

I am a friend or relative of a current/past resident

My wife moved into Bridgecreek Memory Care. Our only concern is that she's the last room, so sometimes the food is a little on the cool side, but otherwise she's doing fine there. The people are very helpful and they've been treating her really decently. I have no complaints about the staff. They're understaffed but they're doing what they can. A community like that, there have been a few outbreaks of COVID. And so, that even stretches them thinner trying to keep that under control. I've shared some of her food. She's on a lighter diet because she's not able to eat very well, but the food they've had there, it's been good. They've had anywhere from lasagna to chicken taco salads, and then they have a cart that comes through every day for snacks. They have a whirlpool bathtub and she was able to use it once. They have two different dining halls and they have two different seating areas for watching TV. We've played bingo, and then she's gone to exercise classes and watched the movies.

I am a friend or relative of a current/past resident

I'd like to thank Andrew and Casey for all their help getting my Mom in a room so fast. My Mom likes it and always has good things to say about the Staff, they are very helpful and nice to her. I would have liked her to have a private room but that is not an option with Medicaid. My Mom doesn't seem to mind much, she likes her roommate. She does need to share a jack-n-Jill bathroom with 3 others, not ideal. I feel they should have a better space for her tooth brush, towels and bath supplies. I found you really need to ask questions how things are done, like Med list, showers, teeth brushing and laundry, so to better understand how your loved one is getting care. It might not be as good as home, but the next best thing. Thank you, Maureen Kennel

I'd like to take this time to thank all of the staff in Lebanon for the great care and keeping me well informed with my moms condition. The new addition to the facility is great! I visited this facility

It is a nice facility. They just recently remodeled. They have three dining rooms where they can segregate people. The people like my wife, who is a little bit higher functioning, are in one dining room, and then a next dining room which is kind of middle, and the third is for people who are really getting toward the end of memory care. So instead of one large dining room, they got three small rooms. There are a few residents that are high-functioning that actually get together and do a little card playing, or play Solitaire. My wife’s only been there for a week. My interaction with the staff is absolutely 100% positive. They’re very caring and just right on top of things, and they keep me informed.
