I am a friend or relative of a current/past resident

Cottonwood (formerly Grace Living Center) is older looking. It just was not very modern, and they have not really done much to modernize it. The nurses were understaffed. They just had too many patients. They were helpful if asked, but my dad was in urine or poop every day. I walked in, and he's naked most of the time. Even though they are helpful when I came in, I don't feel that they were helpful to him as an individual without me being there. I would never recommend them. I never saw any activities. I saw the PT person once, and all they did was move him from the bed to the chair. I never saw them do anything else. He didn't go to the dining area. He was delivered food in a Styrofoam container. They should have more staff, particularly CNA's. They could check on their patients a lot better that way. Also, maybe increase the cleaning staff because their cleaning staff were not doing an adequate job. They should also bathe and dress the patients more frequently. Based on my observations, they just weren't cleaned and dressed properly (not even a gown).

I am a friend or relative of a current/past resident

My husband was recently a resident in the Skilled Nursing wing. Even though the facility is very clean, I was very dissatisfied with the physical therapy he received while there. Getting appropriate physical therapy after having hip replacement was why he went to skilled nursing. I don't feel that sufficient time was spent in teaching him how to regain his independence. He received p.t. twice a day, with one of those daily sessions being solely devoted to speech therapy as if he were recovering from a stroke. In the sessions devoted to actual physical therapy, he was walked the majority of the time with a few upper body exercises. I found out the day before he was released to go home that the regular physical therapist was on vacation and fill-ins were being used in his absence. It was basically a wasted week for my husband and did little to help in his recovery process.

I am a friend or relative of resident

Grace Living Center at Chickasha (GLC-C) was a true blessing to my mother. When I brought her to GLC-C from a place in Texas, she was malnourished and in great physical pain. She screamed whenever anyone touched her. She viewed touches as adding to her already immense pain. Grace Living Center at Chickasha, with gentle care and kindness, cured all her sores and ailments. My mother quit saying she hoped she would die in her sleep. She began to smile again, and she quit screaming in the night. The residents at GLC-C have smiles on their faces. Robust, skinny, and in between, they are all treated with respect and kindness. I will be forever grateful to the management and staff. Since a space became available at a Grace Living Center in the town where I live, my mother has recently moved.

I visited this facility

Everything in Grace Living Center seemed very clean. It smelled clean. The staff was very accommodating and helpful in answering questions. The rooms were also clean. The dining room was very nice. It had a nice fireplace and table. Everything was nice.

I am a friend or relative of a resident

My great grandmother was a resident in this assisted living center. This center was always clean and appropriate when i was visiting. This facility always smelled clean. My great grandmother loved this place and was very happy there. The staff were very friendly and took good care of the residents. They knew the residents by name and knew what they liked and disliked. This facility had good food for the residents and allowed family members to eat with the residents. The facility provided activities for the residents that allowed them entertainment and alloed them time to get out of their rooms and meet people. I have no complaints about this facility. The facility also took their residents on outings into the community so that they did not feel trapped in the center. The center had vending machines throuhought that allowed the residents to continue to use their money to buy things and feel independent.

I am a friend or relative of a resident

I was extremely reluctant to place my mother in any type of assisted living.The staff at Grace were concerned with my feelings and willing to work with me to alliviate any doubts I had about her safty and personal care. The staff has treated my mother with the respect she deserves and does their best to provide her with a safe and loving environment.I remember the very first question I was asked was what is your greatest concern about placing your mother in our facility. I truly appriciated this as it made me feel as though my feelings were worth hearing and that they would do their best to make sure that they would be addresed. My main comcern was for my mother falling and every effort to prevent future falls ius being looked after to the best of their ability. I am ever so thankful for the kindness of the staff and the care they are giving someone so special to me and my family.
