I visited this facility

I liked that the rooms at Grande Village Suites were very big, clean, and roomy. They have a full kitchen, a refrigerator, microwave, stove, and dishwasher. The price was good. My mom will be paying half of the price of what she's paying now. They take Medicaid, too. Jessica was the one who toured us around and she was awesome. They have a lot of activities like outings and happy hour once a week where they have food and drinks if they want. Then they go to a lot of different places such as museums and shopping. They also bingo, cards, and puzzles together. It just seemed the right fit.

I am a friend or relative of a current/past resident

You want to know how much compassion and empathy these facilities have? My poor mother was in the care of 4 of their facilities over a 5 year span. After suffering a fall that resulted in a broken hip requiring surgery, I had her moved from their Lorain location to Grande Village. My mother contracted COVID TWICE in the span of 10 months, despite being fully vaccinated, while in their care. Once in the first facility and finally, at Grande Village. For some reason Grande Village decided they needed to quarantine her in a facility nearly an hour away and moved my bedridden, incredibly fragile and frail mother. Not even a week later, she died. Not a single person from Grande Village, where she was "cared for" for the past 6 months has contacted me to acknowledge her passing, let alone offer condolences. To add insult to injury, I received a call the day we said our final goodbyes to my mother from the quarantine facility, giving me an update on their Covid cases! I refrained from losing my mind on the woman and left her with, “you do not need to call me with your Covid cases updates. My mother passed away last week while in the care of your facility.” Today, I received a bill for salon services for her haircut. This haircut took place before she died and they have $160 in her personal fund that is supposed to be used for services like this. Do these people not communicate with each other? Are they that disorganized and incompetent? Caring and compassionate? Not so much. Understaffed with high turn over and day labor workers? You betcha. Before considering putting a loved one in a nursing home be sure to do thorough research. Investigate staff turnover, ask about day labor and temporary aides, do spot checks to see how clean the facility is, call them and see if they return your phone calls. Good luck trying to get someone to answer the phone on a weekend . I often had to place calls repeatedly to get a return call from front office staff. If you do end up using this facility check on your loved one constantly. The facility is all smoke and mirrors. It looks good on the outside until you look a little closer. I gave the facility 2 stars because of Sprenger hospice staff. They showed my mother care and compassion and I appreciated the updates from Chaplain Danny, who visited with my mother regularly. HE did call me when he learned of my mother’s passing. Shame, shame, shame on the rest of them.

I wish I could give this place 0 stars. My mother was grossly neglected. Because of their covid no visitation policy, I was unaware of how bad the neglect was. It was as though the staff did very little because they were not being held accountable since there was no visitation. My only contact with my mother was the telephone which she often forgot how to answer or she could not reach it. When I called the nursing station they rarely answered the phone. When they opened up for visitation and I saw how bad my mother looked, I immediately removed her from the facility and had her transferred to a hospital. My mother was gravely ill when she arrived at the hospital. She had deteriorated to the point where she did not survive. Do not put your loved one in this facility.

First of all, giving this place one star is being extremely generous, it would not let me continue without hitting one star. I am speechless that this place is still open. My grandma has been treated horribly in here & was not at all taken care of. The nursing staff would mess up and then lie and say it was not their fault. I requested a urine sample & a week later they said they still didn’t do it, they let her lay there & suffer & get “very ill” without calling and updating me. I would call multiple times a day & no one ever answered the phone or called me back. I called the administrator & left a message a week ago and never heard back. Today they dropped her & told me that she fell. She can’t fall because she can’t walk alone or get out of bed and then they called me tonight saying she is bleeding really bad internally but they didn’t call 911, they called their own transport service and they said it will take at least 90 minutes for them to arrive. Then they proceeded to act confused as to why i requested they call 911 as this is urgent. This nursing home needs to be shut down immediately!! I will be contacting the ombudsman for the Stare of Ohio and possibly an attorney based on the outcome of my grandmas health due to their negligence. I would urge anyone looking to send their family member here to look elsewhere because if you send them here, you probably won’t ever see them again.

I'm sure that there are plenty of worse places, but that says more about the sad state of nursing home quality in the country than anything else. My family member needed a fair amount of help and this was a horrible place for someone in that situation. It was chronically understaffed, from the front desk (usually empty) to the more vital nursing and aid staff. The staff who were there changed over constantly and weren't particularly friendly or helpful. Once when I tried to go into my mother's room when she was being changed, the aid slammed the door in my face and told me I wasn't allowed in. I got yelled at once for leaving my mother in the room in her wheel chair and going to get someone to help her use the bathroom. Another time, her eye was so coated in goo that she couldn't open it and it hadn't been noted or dealt with; I had to track down a nurse to help. And the staff's idea of entertainment was to stick her and several other residents in front of the TV set for hours, every single day. We were promised she would receive help eating and it was obviously not given since she dropped 8 pounds in her first 2 months there. The rooms looked nice enough, but who cares when the care is so poor? The food wasn't bad tasting, but it took them almost an hour to get a full meal out for about 20 people, and many items served weren't suitable for someone who had trouble eating. And communications were terrible; trying to contact anyone there or get questions answered was like pulling teeth.

My dad has been admitted about 4 times in the past 6 years, twice being in the past 6 months. The majority of the aides were great and very attentive to my dad-especially Tanya. The nurses also were good. The room was big and he didn’t have to share, which was very nice. It seemed cleaner than it had been in the past. Our biggest issue was with the social care worker-Tammy. She was basically useless. She told me to have my mom call her to make an appointment, Instead of calling her as soon as my dad was admitted. Any questions you asked she never had an answer for or she took forever to call back . She gave my mother no heads up on my dads discharge, then set no home care or PT up for my dad like he needed. She didn’t order his wheelchair until the week he left, sending him home without one when he could hardly walk. It’s been 3 weeks and she just checked in bc our doctor called looking for info on aftercare for my dad, so she just happened to call my mom after to check in on my dad. It’s upsetting that they would just send my dad home not making sure our home was prepared for him. They need new administration.

The staffing is terrible and constant management changes. They have had numerous DONS and administrators and directors. This place is a disaster and very very dangerous. They have nurses working double assignments and working two different buildings at the same time. Scariest set up I have ever seen. Patients wait hours for pain meds and many things fall threw the cracks. I can’t believe ODH has not shut Grande Village down.

I visited this facility

My top choice right now is Grande Village Suites. The visit itself wasn't that memorable, but the place sold itself with the size of the room and the amenities that were available. They offer three meals a day and they have a washer and dryer in the individual rooms so they don't have to send out the laundry or walk down the hall. Another thing I liked is that they have underground indoor parking so my car can be warm and dry in the winter. The place was clean.

DO NOT SEND YOUR LOVED ONES HERE!! If I could give this facility a 0/5 rating I would. My grandfather was sent here after a hospitalization for rehab so he could get stronger and return home. My grandfather was placed in room very far away from the nurses station. He was made to wear Depends so the staff wouldn't have to be bothered (he was not incontinent and was able to ambulate to the bathroom). He was a very clean man and was very unkempt and was beginning to grow a beard. Upon each visit he would have more and more bruises and one day was found to have a large laceration to his forehead that was just bandaged up when it should have been sutured. A healthy elderly man is now deceased after 1 week of being at this facility. I don't think this is a coincidence.... BEWARE

My father-in-law was recently admitted there for rehab because of dehydration and weakness. I honestly believe they caused his death. He deteriorated quickly while there and looked like he had been in an accident. Big gash on his head that looked like it should have had stiches and bruises all over. They claim it was from falls. My sister in law got a call at 2:30 in the morning saying he was unresponsive and they were calling the squad and transporting him to the hospital. They couldn't tell us what caused his death, but he passed about 12 hours later. I think they sedated him with something and it ended up killing him. The facility looked very understaffed or the staff were all hiding somewhere. Please don't take your loved ones there. Even the coroner was concerned about the condition of the body. He wanted to know where all the bruises happened and what nursing facility it was. When the son and the daughter went to get his belongings from the nursing home, nobody even talked to them or asked why they were here or anything. Guilty consciences???

Everything at Grande Village Suites was excellent. Dad's room was a two-bedroom 1,000-sq ft independent apartment. He was living in the independent part but he had all the meals brought to his room, and that was really good. They can't provide extended care there, so we had to transfer Dad into a nursing home.

Grande Village Suites was very clean. I knew a lot of the staff because I'm from Twinsburg. They were friendly, and very welcoming. The apartments were neat, and clean. They invited me to have lunch, and it was good. They had a menu that's adaptable to whatever you need because I'm a diabetic. I've been there for Bingo before.

I am a friend or relative of resident

I have a relative in the Memory Care Unit at the Grande Village Twinsburg location. I have noticed that if I ask a question about different ideas to make my loved one more comfortable, they feel challenged and the answer is "no" right off the bat. I've been labeled a problem relative. On 4 different occasions I have seen staff members sleeping. I have seen 3 different aides sleeping in a corner thinking people won't see them. On another occasion the aide that was supposed to be watching all the people parked in front of the nurses station in the Memory Care Unit sleeping. I feel that because my loved one is in a late stage level of dementia and can't express himself that he's invisible, I should just let him be left in the wheelchair all day with no alternative options. Just because his last days will be soon, doesn't mean that he should be ignored. These people can't say what they need. To care for these people properly they need to be transferred from chair to another type of sitting situation at least a couple times of day. I have come in to find his mouth and lips cracked from dehydration. It's not acceptable! He needs to be given water at least every 3 hours. Would that be too much to ask? These pore people are at the mercy of someone else every day for the rest of their lives. They need more staff! This is not ok. This facility needs a wake up call. I have tried to give this facility the benefit of the doubt over and over but I don't see any improvement. Nobody seems to care about these basic human needs. These reviews are to give an opinion about the care given. This is my opinion. Thumbs down. There are basic human needs that need to be addressed here.

I am a friend or relative of resident

I am a relative of a former patient recently discharged. This facility has a very good herapy department including physical, occupational and speech therapy which is why my relative and I chose this facility.HOWEVER, the situation with nursing was very, very different! Do NOT let the fact that they have "deficiency free survey" fool or influence you because unless quality care is actually being delivered currently, it won't matter how perfect the previous survey was!! The nursing care was truly pathetic! When I went in to see my loved one, I could count on even the most basic of tasks NOT having been done. In speaking to the nurses who were overwhelmed with their humanly-impossible assignments, I learned that the situation was only going to get worse because Grande Viillage was cutting the nursing staff even further! When I spoke with the Director of Nursing and/or the Assistant Director of Nursing, the results were always the same - they offered only temporary solutions to ongoing facility- wide problems I would bring to their attention with the hope that future patients and families would not have to go through what I and my loved one were going through. Pharmacy was poor: a new med would be ordered but (on multiple occasions), would not be delivered by pharmacy until two or three days later. Social worker-I felt we received very poor service from the social worker for too many reasons to go into here. After seeing one representative of the activities department upon admission, we never saw them again - they did not attempt to initiate any activities with or for my loved one. I was there for several hours every day so I know what was and was not being done. Housekeeping staff was great and very helpful. In talking with other families, it was apparent they were experiencing the same types of problems in the care of their lives ones. I hate to think of what conditions my loved ones would have had to experience/ensure had he not had a persistent advocate--and I am glad I was able to do that for my loved one but I shouldn't have had to address the same problems with staff day after day after day!!

I have a relative in the Memory Care Unit at Grand Village in Twinsburg. I'm not happy with the situation and I would not recommend this facility. There's not enough help. They are understaffed. There have been too many incidents when the patients have been left alone in the main area where they put the patients when they aren't in their rooms. Also in this area there are many patients that can not feed themselves and there are only a few that have help with that. My suggestion is to stagger the lunch and dinner times. So everyone can have that one on one time. (The food is bad ) The state dictates how many caregivers there are per patient. Just because the state has suggested minimums doesn't mean the it's the correct way to run this facility. The owners could use better judgement and hire more people to care for these helpless, scared lonely people. It's the end of these peoples lives. Why do they have to sit in dirty, food stained clothes or in there own urine. I didn't choose this facility but unfortunately I have to deal with it. I will pray that changes will be made because that is all I can do.

The facility always smelled of urine throughout the buildings all the time. The workers were too busy complaining about their schedules to answer patient calls for help/assistance. The workers are not a happy lot and rarely smile. The nurses, with one exception, Jennifer, did the minimum and didn't listen to my concerns. Concerns for patient health were directed to nurses and the house doctor seemed to be the Great Oz- impossible to see, but you hear about the guy. I removed my family member for safety and health reasons since concerns were being ignored, dismissed, or not adequately addressed.

My husband and her sister chose Manor of Grand Village for their father because it was close to home. The facility was clean and neat. It was new but sometimes it smelled of urine. They had a nice community room. The room was for two residents but he did not have a roommate. It was clean everytime I went there.

I visited this facility

Overall the visit was enjoyable but nothing above average. The area that stood out the most was the food that was available there. The food was lame and had no texture. On top of this the facility had an odor to it. The staff did try and provide activities for the residents but that was its high point. They even have a WII on site. Visiting hours were also a positive point with 24 hour visitation. Parking as well was plentiful which is always a nice over looked feature. Rooms were of spacious size. The other benefit was that they were all private rooms with private bathrooms. Staff was nice and didn't seem as though they were understaffed which was a concern. Outside the place had a nice area to encourage residents to get sunlight. From a gazebo to a sitting room there was plenty of space for everyone. The activities did not stop inside as bingo and entertainment also reached outside the walls to these areas. Overall the place has a great social life but needs work on its food to become above average.

The facility looked very clean and neat. The staff members seemed very friendly. Overall the whole atmosphere seemed nice. However, the members looked very bored, and a bit sad. They had plenty of activities such as music, board games, and a nice living hall. Also, the visit was during the holidays and the facility was very nicely decorated, with Christmas trees, a fireplace, and other various festive decorations.
