Don’t send your loved one’s at this place! You will regret it ! It’s full of Covid patients dying every day.

My dad is already at Five Oaks Manor. It's not the greatest place, there's not enough staff but I'm sure all of them are that way. The staff members that they have are overworked, and some of them are not friendly. The place is a little run down and the facility itself needs some attention. The food is not good at all, they need something done to their food. The whole place just needs cleaning up and more help. The ones that are there are working hard, doing the best they can.

My family member has been a resident here for 3+ yrs. I'm thankful there is a place for resident to be with all of the health compromises pt. has. Most of staff are kind + caring. Others, like everywhere, are not. I accept this fact. What is difficult is lack of COMMUNICATION between staff, resident and family member. I don't call staff a lot because of this and now there are NO phone numbers at present due to NO staff phones ordered but not delivered. I have yet to receive "call backs" fr. past 3 call "messages" I left. If you call on weekends forget making contact....sometimes someone will answer phone, transfer you, 9/10 times to wrong ext. As of late my family member had Dr. appt. I left "message" at least 2 weeks before appt. Had to cancel appt on day of appt due to lack of communication. Dr. office has not chgd me yet for lack of 24 hrs notice and I hope they don't. New administrator did not call back on the two messages left for him since he began. From what I understand does not make his presence known to long-term pt. on bottom floors...upper floors are much nicer accommodations for "rehab" pts. Since this administrator took over a high volume of staff turn over and jobs deleted. There's room for improvement in every organization. This one definitely.

My family is so grateful to Five Oaks Manor for the excellent care they gave my sister. We were very elated to know that 5 Oaks has such a large staff of professionals .Their medical staff is outstanding and the therapists are excellent. What we noticed the most was my sister was so at ease and comfortable there because everyone who helped her in any area showed care and concern for her. She really liked her room and always enjoyed the food- they even made her a tomato sandwich with mayonnaise, which she loved .She has done very well after coming home.

I was a resident for 6 weeks at another facility, and if I ever have to do it again, Five Oaks is the facility I would choose! Staff from the administration on down is caring and responsive to both the needs and emotional wellbeing of the patients and families. Patient rooms and common areas are open and clean for easy access and mobility for walkers and wheelchairs and are welcoming and bright. While there is "no place like home," Five Oaks, in my opinion, is the next best thing.

I had a friend who was admitted after hip surgery. From Admissions all the way to daily care, she was treated with dignity and respect as they would with their own family member. Got her up and running again in no time! Clean and home like facility.

I visited this facility

My 86 year old Father-in-Law was a patient at the upper level Rehab part of Five Oaks Manor for almost 3 months. He arrived with pneumonia, aortic stenosis, heart failure and dementia. Our goal was to get him back to his assisted living facility, which seemed like an unlikely possibility. From the beginning I noticed the staff and nurses were concerned for the patient and family. The competence of the care was excellent and the Gerontologist and staff were available for questions and concerns. The facility was very clean and pleasant. To my surprise they were able to get his health to a point that he was able to return to his assisted living facility. I will forever be grateful for the compassion, competence, and care shown to our loved one and family.

My 89-year-old mother was admitted to rehab from the hospital because she was no longer able to walk or eat solid foods. The rehab team worked with her but were unable to make any real progress. Part of the problem was that their psychiatric provider did not see her for the first 10 days she was there & made no med changes that would have allowed her her to really wake up & function again. They also did not have the staff or make the effort to get her up to the bathroom (2-person assist), so she was incontinent by necessity. The professional staff, general physician, and administrative staff were very kind and helpful, but the aides have too much to do to be really good caregivers.

My mother was recently in skilled nursing following a hip replacement. She was there 45 days. She arrived with a UTI, and anemia that handicapped her initial therapies. The Five Oaks staff did an outstanding job caring for her and getting her to a point where she could function again. I have nothing but praise for them. A friend whose father fractured his hip a few weeks ago was able to have his dad admitted. He is making very good progress at this time and the family is pleased with his Five Oaks experience.

The staff at Five Oaks Manor was nice. They're really nice people. They showed me around. The rooms were fine. The place was clean.

We had to face putting my mother in a nursing home at her final stages of life. The staff at Five Oaks were very kind and they even went out of their way to accommodate a private room so that we could visit alone with mom. I would recommend them again.

My Mom was admitted to Five Oaks in December. Dr. Keipper added a new medicine that has helped. The therapists are exceptionally kind and competent and Mom's strength has improved. My doctor's nurse raves about how good the driver of the van is with the patients that she brings for visits. The hair stylist is a full time employee that is good with the patients and helps with other activities. There is no extra charge to have patients' hair styled. The nurses and CNAs are so kind and caring. It has been so hard to place my Mom in a nursing home; but the staff at Five Oaks has made it easier. The office staff has been helpful and the director has been caring. My Mom's condition has improved and I thank God for the caring staff that helps her. The housekeeping staff keeps up with the cleaning.

My grandmother was placed in this facility about 4 weeks ago. We are in the process of trying to move her to a new nursing home. Her health has declined tremendously in just 4 weeks. She has been dehydrated since she moved in and has not been cared for properly. They are very understaffed so when she calls for help using the bathroom, they tell her to eliminate in her undergarments. It takes them hours to finally get around to changing her soiled undergarments. At this point, she is being hospitalized for a bladder infection (which I believe is due to not being helped using the bathroom or changing soiled undergarments), dehydration and bed sores. The hospital said they have 5 other patients from this facility that have been admitted for similar issues just this week alone. She went in to this place 4 weeks ago, able to use a walker independently and now can't even sit up by herself. Do NOT send a loved one to 5 Oaks! I would rate it Zero stars if I could.

I would not recommend this place to anyone at all!! My mother has been there for a year and her health has gone down tremendously..This place is horrible!!! Understaffed, uncaring people and a administrator that only cares about money!! I am in the process now of getting her moved out..She has fallen numerous times because of lack of care, last time she fell, she broke her hip and shoulder, which was twice for her shoulder...she has been hospitalized 4 times or more for dehydration..Very very poor care..if you go on medicare.gov you can see the poor ratings..I wish more people would post on this!
