My mother was in this facility in 2019 with positive results. She able to rehabilitate and return to her normal life. Something has drastically changed since then. After another brief hospitalization she returned to Throgs Neck Rehab in early January 2022 in the middle of the OmniCron variant wave. Initially her cell phone charger disappeared, and eventually her phone became disabled. It was nearly impossible to get through to my mother. I tried many, many times to call the nurses station at different hours only to have that phone ring 30-40 times with no answer. On the two occasions someone answered the phone they told me she was either sleeping or eating. Then I was told her phone was missing. I tried calling the Concierge, Social Worker and eventually the site Administrator. I kept being told they would look for the phone. I called several times for her medical records to be sent to her new residence. I found it impossible to get cooperation from the staff. The Administrator Mr. Caraballo apologized on behalf of his staff but could not get his staff to do their jobs. On the day before my mother was schedule to be discharged she asked a staff member to call me. I spoke to her for the last time. She complained she was not treated well but was anxious to leave. The next day I received a call from the on duty physician informing me my mother was not well. Her blood pressure and heart rate were gravely low. The doctor asked me, "what would you like us to do". I thought I would have been told she was on her way the Emergency Room instead. As it turned out my mother was in Septic Shock something a medical professional should have recognized. My mother never spoke another word or responded to any commands. She passed away 6 weeks after this episode. During her hospital stay I asked for her personal belongings. Cell phone, jewelry and clothes. I was given a box with her name on it. When I opened the box it was filled with other patients clothes. Several with different patient names. None of her clothes, cell phone or jewelry were returned. My recommendation is to stay as far away from this place as possible. The owners of this place need a full overhaul of their staff and it starts at the top.

This Facility is the best ive even experienced, im always able to reach someone in the facility (i usually speak to the concierge) the rooms are always kept tidy ( so is the rest of the building) the staff are always nice and accommodating, the building here has a team that makes sure each and every resident is not left behind and is cared for.

It is a great place for the person who needs to feel like the staff knows them and cares.

My mother was in a nursing home called Throgs Neck Extended Care Facility. She was there for four years, and I took her out during the pandemic. They just don't have enough people working there to care for these people, but they did the best they can. It's small. They had very little activities when I was there, but now they're probably changing. The food was just OK, but they had good medical care. They had good nurses and doctor that took care of the people. They were very good. We were very grateful for the ones we had. They kept the room very clean. We were able to pick the food that she liked. They just had rehab and meals. They did a lot of arts and crafts, bingo and stuff like that. They just need more staff. They don't have enough staff there. The bathing area was very bad. They only had one shower room on each side of the floor of 20 people. She had Medicaid.

All of my experiences at this facility was horrible in services for me. The security and staff are very rude and when I went in my relatives room there was dust everywhere. Now they make us visit outside so I wouldn’t know how the room looks now but it’s definitely a concern. I truly want the best for my beloved family member and all of the other members there. I’m willing to do whatever is needed even writing our Mayor for better results for our beloved family members if necessary.
