I recently visited my mom at ABQ Heights, Who had a wonderful experience while there. Every staff member has been friendly and extremely helpful. Their Kindness was much appreciated.

I am/was a resident of this facility

I was put in a room with a person that needed 24 hr help. Needless to say this kept me awake through out the nights. My reason for being there was I fell, stroke surviver, on my hip joint. The bed I had was sinking down to where I had a bar under the mattress on my joint. I tried 5 days to get me another mattress and they finaly did. One night the CNA took my Blood Pressure and said it was to high, 148/83, and she with held my blood pressure meds, she had no right! Having a room with two tv's 10 feet apart is dumb.

Provider Response

Thank you for bringing this matter to our attention. We will use this as an opportunity to improve. Thank you for taking the time to leave us feedback about your experience with us.

I'm at Albuquerque Heights Healthcare and Rehabilitation for rehab. I'm in long-term care just to get my strength back and then I'll be transferring out to the assisted living. I started in rehab and did my 90-day stay, and I graduated to long-term care only because I don't have a place to go in town right now until we can get me a little bit stronger. I am up on my own, and I'm taking high percent care of myself right now. The care has been excellent. The food is very good. The rehab staff is very attentive. They may seem harsh at first, but it's only because they need to get people up and moving and using limbs that have been weakened from surgery or lack of use. After a while, you appreciate them being as stern as they were to get us moving again. The arrangement of the room is good for the patient's privacy. There is enough storage for a small amount of clothing and personal effects. There is a shared bathroom with showers twice a week. It's very clean. The housekeeping staff is on top of everything. The nursing staff is good. I've had no complaints. The food here is on a very good level. There is a lot of variety in the meals. Oftentimes, there are some very good alternatives to the main offering for the meal. If you don't like what is being served, you can always ask for a chef's salad or other substitutions for lunch or evening. Eggs are made to order in the morning. No complaints.

Provider Response

James D., we're so glad that we were able to go above and beyond your expectations! Thank you, and have a fantastic day!

I am a friend or relative of a current/past resident

If you want to be in contact with your loved one, don’t put them here! My father’s room was inadequate, cramped and didn’t have a phone. Calls to the front desk were dropped or ignored. I asked attendants to ask my father once an hour if he needed to urinate. He is deaf and can’t understand buzzing nurse’s station. No one asked him so he basically held his urine for hours. When I visited several times I tried buzzing the nurses station dnd got no response. Nurses were glued to their phones at their stations; even when I went up to them I was ignored. I wouldn’t put my worst enemy in this place.

Poor communication from case manager throughout my mother's admission at this facility. Staff failed to provide any kind of bathing opportunity for the first 8 days of her stay, which led to a fungal infection and rash. On her 10th day at this facility, I received a phone call at 10:30 PM from someone on the nursing staff. She told me they had taken my mother to the hospital for an appointment at 2:30 that afternoon, and she had not arrived back yet (?!). They wanted to know if I had heard from her (??!!). My mother was unable to walk at this point, and they were saying they had lost track of her. I found her in the hospital ER where she had been awaiting admission all afternoon and evening. She won't be going back there.

My friend stayed at this facility after having had some serious surgeries. She was placed there to recover and receive therapy to get stronger to ultimately go back to living on her own and caring for herself. Although I wasn't able to visit her due to Covid restrictions I checked in with her 2-3 times per day over the phone. There were so many issues that should not even have been issues. The food was terrible, the scheduled personal hygiene care she should have received was cancelled or rescheduled several times and the discharge planning and preparation was the worse. When Medicare notified her that they would no longer cover the Rehabilitation services at this facility she was literally put out. Before release she was not allowed to walk on her own because she was at fall risk and her back was full of blisters from not having been out of her bed as much as she should have been. She went from release to the emergency room where she was re-admitted to the hospital and has been there since. It was shocking to see her released like this. Patient Advocacy was non existent and the people responsible for discharge planning certainly did not do their job. Most staff was great, Nurse T that took care of her wound vac as well as the Physical and Occupational therapists and the nurses and techs. She is getting ready to be released from the hospital and just found out she will be coming back to this facility. Given the situation I have mixed feelings about posting this review. I certainly don't want her to have repercussions, but I am hoping that her care can be better and she can achieve her goal of recovering to full independence.

Floors in room was filthy not sweep. Even in the patient's bathroom and toilet was not properly cleaned after the roommate was using. My mother's roommate was yelling at staff and at my mother even me. That roommate not understand that she is sharing same room with my mother with little privacy and that I am deaf and visiting with my mother. She still yelled gave the staffers bad names. My mother was given a crappy not good bed which she suffered from a cracked bone in her spine and three broken ribs. Nurse two of them forgot to give my mother her pain medicines few times. My mother is not happy for being neglected. She waited for assistance staffer came mother told her what need and she forgot already! Please clean up the shared bathroom and the mess there and on the floors. I am pretty disappointed at their services or caring level. My mother has not seen a doctor in 4 or 5 days yet. Not a good experience. :( No tour or staffers showing me and my mother around yet since she arrived last Thursday evening. My mother not participate in activities or no explaining of amenities what that nursing home offers yet. My mother's interactions with some staffers was mixed mostly not great especially one nurse was so weird and had to leave room immediately because of roommate's yelling or bad attitude. It is like walking on eggshells. Her space was very limited like a matchbox especially that she has her wheelchair and walker with her.

I am/was a resident of this facility

I had multiple bad experiences at this facility. I came in with a lot of pain, screaming in pain for medication. I was told once that I had already had it and then she walked out and closed the door. I was in agony for 17 hours. Then, we found out the 'nurse' had made an error and she could have given me pain medicine. Another time a 'nurse' came by to give me medication. I said I had not had that medicine before. She insisted I had. I argued with her. She then checked and determined that the medicine was for someone else. There was water on the floor several times but no one bothered to wipe it up until I insisted.

My mother had to go into Albuquerque Heights Healthcare and Rehabilitation. It’s been positive. The rooms are fine; the food is good. The staff is very helpful and caring. I think my mother is well taken care of there. We haven’t even come upon a month yet, but I’ve been pleased. It seems like everything is good.

I visited this facility

My father was only at Albuquerque Heights Healthcare and Rehabilitation for one night because it was so horrible. They needed more staff. There was one woman who asked us to wait; she was counting medicine and doing everything all by herself in an area away from the nicer rooms. They had these folks, probably eight to 10 different individuals, in wheelchairs who were just slumped over, drooling on themselves, spitting up on themselves, and moaning. We picked Albuquerque Heights because it was next to a hospital with an emergency room. We also chose them because they said they would give my father a room with a window. The room had a window, but it was nowhere where he could see it, and he was paralyzed, so he wasn’t able to get up. There were two people in this one room with three in most rooms. They put him in a room with a man that hollered all the time, and it scared my dad. The man would holler because he couldn’t speak. One nurse came in the room, said that it’s OK, told him to stop hollering, and then left. She didn’t do anything for him. When I went in the day before, I walked in like a stranger. I wandered all over the place before somebody asked me who I was and why I was there; I lied and still wandered all around the place and never did sign in. There were tissues on the floor, and the carpet needed to be put back in. The building was super old. The only positive thing I can say was there were other rooms that had really nice TVs and monitors and had been fixed up in the part that you walked through; those areas were nicer but when you walked deeper into the building and into the back, it just got worse.

I placed my mother with Albuquerque Heights Healthcare because we could not afford any other in June of 2014. My Mom passed away there in Feburary of 2015. Her stay was a long ordeal for the family as she had advanced dementia, wheelchair bound and other health issues. She was well taken care of, always clean. However, the facility seemed understaffed. The last days of her life they placed her in a nice private room which the family was eternally grateful. We were able to be with her 24-7 until she passed . . .

I am a friend or relative of resident

I'm a relative of a resident currently at this facility, while there is staff there that is competent friendly, knowledgeable and trustworthy, there is a lot of faults here. Under staffing could be one, attitude another. I have dealt with quite a few nurses, therapists and staff at this facility and all in all they helped my grandma, but they didn't not go above and beyond by any means. Upper management needs to start watching over this place more carefully, especially when it comes to safety of others, especially patients (having nobody stationed at the front desk and no security is a bit scary).

Would not recommend this place to anyone, some of the staff cares about the patients but that is few and far between, the administration staff is not responsive for medical records they are rude and very unprofessional.

My mom has been in Albuquerque Heights since August and I think she's happy there. The staff is wonderful. I think they have done a very good job with her. She is in a semi-private room with a bed, a bathroom, a clothes closet, a little dresser and I brought in a phone and a TV for her. However, She has several different roommates and it has been kind of tough for me. They have a few activities, but she hasn't really catch on to it. She didn't like the food. She said it's awful. I think it is a fairly nice place. However, it will be helpful if they have consistency in the staff and the food service

the facility is clean, it receives regular maintenance, and it's well laid out for emergencies. the staff seems nice, friendly, but I noticed them be inattentive sometimes. there was also one gentleman who worked there that my grandfather didn't like, he was rough with him and wouldn't listen to him. I understand that he doesn't work there anymore, but it took some time for him to be fired. other then that the staff is good. my grandfather says that the activity's are good, they try to keep them active, which is good, and they're always bringing in entertainment or instructors. last time i was there they were doing yoga in the main area.all in all its a good facility. and i would recommended it.
