I am/was a resident of this facility

The rooms are adequate. Plenty of closet, chest of drawers, bathrooms clean and spacious. Food is Institutional,so don't expect gourmet. It was OK. The nurse's, aids,and whole staff, for the most part,are wonderful and caring. I was there for rehab following knee replacement. The physical therapy department is outstanding. The director runs a tight ship, and takes pride in his therapists. After a few bumps in the road, I was well on my way to recovery

I have never done this and wish I had earlier, but this place is grossly understaffed and overcrowded. My family member was here after breaking a hip. The family member was just put on Parkinson’s medication and significantly better, but they had NO accommodations but to put him on the floor so he didn’t fall out of bed. That nurse and all staff were sooooooooooo busy, literally running around with a floor FULL of patients. The facility was not upkept. We had been there two prior times, quite some time prior, about 5 years prior, and it was fine. So I trusted. But do NOT trust. Please. If it looks over crowded…get your family member out and transferred elsewhere. I am so sorry I did not. Trust your gut. You don’t have to be “nice” and think positive like I did. That. Simply does not give your family member staying there anything. Because of the 5 -6 year prior stay experience, with very different staff…administration as well….I trusted it was just a bad first Impression. I was concerned and I was right. Don’t stay here if you can avoid it. Shame on the state for NOT regularly doing visits as family members to see the real environment. Announced visits are a joke. Places prepare for them and put on a good front. It was not good.

I am/was a resident of this facility

I was looking for a place to regain my strength after a prolonged respirtory illness and hospital stay. My wife and daughter visited Tall Wood first to "scout" it out. They came back convinced this would be a good place for me. The entire expierence was positive. The staff was knowledgable and always helpful. This along with a friendliness was just what I needed. Everything was done to make my stay comfortable and productive. I stayed about a month and the excellent phyical therapy program got me back on my feet. I would highly recommend this Care center.

I visited this facility

Had Mother on respite there. She passed the day before she was to come home Communications between hospice and Tallwoods wasnt timely. Her menu should have been pureed. She was brought chopped. Took 2 days for that to be changed . When called was told she was resting comfortably . When I went that wasnt what I saw. The nurses and aides were nice. But I would think understaffed. My mother was very fragile. Would have liked to have seen her handled more gentle. I just wasnt happy with what I saw. And she was only there 4 days when she passed.

I or a family member is at Tallwoods just about every day to visit our mother. We are extremely happy with the care of the staff, as well as the cleanliness of the facility. The physical therapy department has also been amazing.

The facility is really superb. Everything from the food to the activities and staff. They have been really good to my sister. She is always raving about the place.

My dad has been temporarily living at Tallwoods for rehab. Overall, his experience has not been great, however, tolerable. Today, leaving my dad in their care has me thinking twice and wanting him home immediately! When I got to my dad at 10am, he was upset, frustrated, demanding he come home. He was making me so upset, I was trying to explain his safety at home is priority, he needed to “suck it up” and allow the Time to get stronger and be in a position we can care for him at home. He was not hearing this and all he was saying was, take him home they don’t help here! The bell rings for over an hour for help, he’s dirty by then and his bottom hurts from sitting. Well...After we went to PT, came back to the floor, all he was saying and feeling, I witnessed. A simple request to have him changed, cleaned, and ready for dialysis turned into my worst nightmare. At 12pm, The CNA came to his room, after I went to the charge nurse seeking help. I went to the charge nurse because his nurse had no idea who his aid was. By this time, I took the lunch tray off the cart for him and his roommate, that were taken to the floor at 11:30am. Took the food to the nurses station asking to warm them up, cause they were ice cold. The CNA informed me and felt she cleaned him already and he was “done”. She felt, what she did at 7:30am for him was enough and there was no reason to change him, get him out of his pjs, and ready for his pick up. This meant, he was up in the wheelchair for about 5 hours, dirty, not dressed, would be leaving the facility this way for an additional 5 hrs, when he arrives back wait for pm care, then cleaned? What part of this is ok? I addressed this again with the charge nurse who said she would make sure this was done. I understand it’s a weekend, maybe short staffed, however, one nurse was sitting at the desk, one aid was walking around the hall, the other was standing by the nurse station leaning on the wall, and the other was giving meds. There was staff doing nothing. Knowing you are dependent on others to use the rest room, stand, get dressed, having no legs, puts you in this position is frustrating in its self, but when you have the nurses and aids not want to provide this assistance and put my dad in a position to say “don’t bother them” makes me sick! There is no reason for this to happen. My dad is in his right mind, can you imagine how those who don’t are treated? Tallwoods Care Center need to evaluate their mission and practices. My fight is valid, and I will advocate for my dad, remember there are patients who do not have someone to do this for them!

My mother was here last winter. She had been in and out of the hospital for respiratory issues for a few weeks and was deconditioned, she definitely needed therapy to be able to go home and be independent. The staff were so caring, They made her feel like they were her family. This in turn gave her confidence to complete her therapy to go home. I know I wanted her to come home earlier, but I have to say the extra week they kept her made a difference. She has not been rehospitilized since. Thank you! I recommend tallwoods as a place for your loved one. My family and I were very impressed, and do recommend it to others in need of therapy.

My dad is a resident here and i give them 5 stars. The nursing staff are amazing, they are extremely caring and treat him as if he is was their own father. Rehab is second to none. The overall staff are great, always friendly and very caring...

My mom has been a resident here for 1 1/2 months and the quality of care is poor. Joe in PT has been great and a couple Nurses, but beyond that it has not been good. They missed two of her medical appointments including the most important first post surgical follow up appointment. They took her to another one hour late. PT wanted her to use walker to gain strength and Nurses/Aids yelled at her for getting out of bed. PT said she can go to bathroom herself and Nurses/Aids on overnight shift said to use bedpan. She would often ring for help and no one came. I witnessed this one night for over an hour and a half. They were supposed to do a BP test by having her walk to Nurses station and then take her BP, but when I checked on it the next day they had no idea what I was talking about and it wasn't communicated to next shifts. Receptionist rarely looks up unless you say hi first and Social Services still haven't called back in days. Don't send your loved ones here!!!

I am the daughter of a former patient who was there and I have to say my mom was treated with the utmost care. I have since lost momma but if she were here today she would have nothing bad to say about this facility. I was there every day to visit with her and I did not see anything as written by others. The staff were very kind and caring.

I found this place to be terrible. I visited my grandfather here this weekend and I would not chose to board my dog there, let alone choose it for a beloved family member. The lobby and lower level are in good shape and provide a great fooling facade. However, travel to the second floor to Pine or Cedar unit and see what is really being offered. Patients rolling around in wheelchairs with vomit covered wheels. Feces smeared on walls of elevator. Ignoring patients. I watched an aide witness a patient spill a drink all over her lap in her wheelchair and ignore it. The patient sat in the wet cold puddle and fell asleep. She remained there my whole hour long visit. If I had custody of my grandfather he would be moved ASAP.

My brother in law is at Tallwoods with Parkinson's Disease. The rooms are large and comfortable. The staff has been absolutely wonderful. I get a call for anything that could be a concern and many things that are really not necessary to advise me of. My brother in law who was very concerned that he was being admitted to die is happy, smiling and talking to everyone and truly feels this is his home. Everyone from the administration to aides have been very attentive to my wishes and have made this potentially horrible situation a great alternative for someone I care about and love.

It was between Tallwoods and two or three others, and it was the location. In case my father ever needed to go on peritoneal dialysis, this establishment is trained in that. These were the key factors, and it was also very clean. The nurses are the ones that I definitely get along with. I have actually a few of not my favorite people working in the finance department, and those are the only ones that I have a few problems with, but otherwise, the whole staff is fabulous. He has his own room, the rooms are nice to look out onto the woods, which is where they based their name from, and he has his own bathroom. But even if he didn't, the rooms are big enough and very clean. It is close, and he gets to see his family everyday, so that is really his key factor. It would be just people returning some calls or getting in touch with you when you leave messages and stuff in terms of like the finance department. Everyone else is fabulous; there were times my father had to go to the hospital, and they always called me and let me know what was going on. The nurses are fine, but when it comes to the financial aspect, that's where they need work definitely.

Place is horrible. My father escaped from the facility under their care and when the nursing unit was questioned about it took no responsibility and said that we should just "pick him up and take him home." Definitely do not recommend to anyone to put their loved ones in this place. Rude staff, bad management.

I visited this facility

AWFUL! My grandmother was here and I can tell you that the care was atrocious! We dealt with everything from the staff not calling us for emergencies, being misinformed or uniformed about injuries or health issues, to her having her wedding ring stolen, being told "You don't need this anymore." This place needs to be shut down, it's a nightmare.

This facility at one time had been thought to be one of the better facility's within the area. Unfortunatly it appears as though it is has been mismanaged and let go. It is now under staffed and nurses and aids are unable to provide the quality of care that you would want for your loved one. Staff appears unhappy and have poor bedside manner which is in return making a terrible atmosphere for residents. When speaking with management regarding these issues you would expect change. I don't know whether it is due to lack of knowledge or just poor laziness that requests never are met and things are not taking care of. Beware, Do not be fooled by this facility, the pretty atmosphere does mean great care. My family member will no longer be here.

From the minute I walked into Tallwoods Care Center I got an immediate feeling of a peaceful atmosphere. There were beautiful pictures on the walls and the facility was well decorated. The visiting area was also has a very relaxing feel to it. The receptionist and nursing staff always seemed ready to help especially when I visited my grandmother. The quality of the food was also very good and provided a well rounded meal that is not repetitious for the residents. Activities are another aspect of Tallwoods that they excel at. The staff provides many different activities in order to keep residents happy. One thing in particular that I disliked about Tallwoods is that some staff seemed to take very long smoking breaks during their shifts. Additionally, on some visits to Tallwoods a few nurses seemed to be having non-work related conversations at the nursing station while call lights were lit above rooms. Overall, Tallwoods is a great facility and I would recommend it to other friends and family.

At Tallwoods Care Center my grandmother really enjoys having company- people around her age who she could relate to and remember the good old days. She really enjoys the activities and always says how nice the activities volunteers are to her, and how they remind her of her young grand children. There are a few nurses she does not particularly like, I think they are more pushy and she wants to be assisted, not told what to do. She still has plenty of spunk left and I am sure the staff sees that!Her favorite time is spent with her friends in the dining facility, she has raves about the food (not really sure why, I imagined it to be hospital food). From what I have personally seen, her room is kept up well and she really enjoys living there. Her quality of life is where it should be and I have to say the only negative I see is that some of the staff is not so pleasant.
