Oak Meadows felt that my mom needed a little more care than what they had. The rooms were big. They were regular apartment building-type things, so it wasn’t what I was looking for.

What ongoing training does the staff receive in dementia care?

We are required in Minnesota to do yearly dementia training.

Describe the overall population of assisted living residents at Oak Meadows. What is their average age? What are their favorite things to do?

Average age is 85. We have classes, bus trips, a Red Hat Club, a full-time chaplain, theme parties.

What programs or activities does Oak Meadows offer to enrich residents' well-being?

We participate with McPhail Music Center for the Arts, Compass Arts, Musicians and our chaplain provides weekly services, Bible Study, Devotions and hymn sings

What places are near Oak Meadows or within walking distance for assisted living residents to enjoy?

Olive Garden, Burlington Coat Factory, Home Goods, Best Buy, 5 Guys Hamburger, Nails, Caribou Coffee, Buffalo Wild Wings.

What transportation services does Oak Meadows offer to assisted living residents?

We have our own bus for scheduled transportation. We do not go to medical facilities. We do, however, have two physician/nurse practitioner teams that are here monthly.-Allina and HealthPartners

What makes the chef or dining program at Oak Meadows exceptional? What kind of menu can assisted living residents expect?

We have our own staff in the dining room and kitchen. They know each resident by name, and many of their family members. Breakfast is a special entree with made-to-order items. Lunch is made to order. Dinner is two special entree's and a limited made to order. We do encourage residents to submit a favorite recipe, and our staff will make it.

What safety features or security measures does Oak Meadows offer to assisted living residents?

We are a secure building. We also have a 24/7 receptionist and home care team. Nurses are here 7 days a week during the day.

How does Oak Meadows keep residents' families informed about their loved ones?

We have a monthly newsletter as well as updates on our website, Facebook, and Linked In. We also have info in elevators and on our house TV's.

What is the care staff-to-resident ratio at Oak Meadows during daytime and nighttime hours?

It depends. There will always be three overnight.

If Oak Meadows accepts dogs or cats, what indoor/outdoor areas can they enjoy?

We only accept animals on a case by case basis. We have an outdoor area by our library, that has doggy bags as well.

What ongoing senior care training does the staff receive?

We are required in Minnesota to do yearly dementia training. Our home care staff also have ongoing training. As do the dining and kitchen staff.

How many staffers are on duty in the daytime and overnight?

It varies. During the day there is always a nurse, home health assistants and a receptionist. Evening and overnight there are 2 home health assistants and a receptionist.

What safety features or security measures does Oak Meadows provide to protect residents who wander?

If they wander, they would need to move to memory care. The doors are locked.

What programs or activities does Oak Meadows offer to keep residents engaged? How does the staff engage residents' long-term memories?

We do sing-a-longs which are very popular. Exercise classes, Music for Life, Church services and hymn sings. In the summer we are outside most every day it doesn't rain. We have craft session, visiting dogs, visits from local school children, parachute, flower arranging, concerts, Mind Aerobics, Chair yoga and reminisce sessions

Who assesses residents' health and cognitive functioning? How often is that assessment repeated?

Our nurses assess first two weeks, then 90 days, then and ongoing as needed.

What techniques does Oak Meadows use to support memory care residents during mealtimes?

Encouraging them to eat and drink. Cut their food if they need it.

How does Oak Meadows keep residents' families informed about their loved ones' health?

They will be called by our nurse. They are also welcome anytime to ask any of the memory care staff if there is a concern.

Describe the neighborhood around Oak Meadows. Is it located in a residential neighborhood, a business district, a rural setting, or other type of locale?

We have trees all around us, and a pond in our back yard. There are walking paths. We have lots of green space. And a golf course across the street.

Is Oak Meadows affiliated with a hospital if more care is needed?

No, but we are fortunate to have 7 hospitals in the area!
