I am posting this to hopefully encourage this facility to make changes. My mom had a terrible experience at this care facility and was released yesterday. My mother has had spine, hip, and ankle surgery to recover from a car wreck from long ago. She's been in and out of hospitals and rehab off and on for years, and they sent her here recently since she had gotten weak following a stint in the hospital with infections. While she was there, they threatened to take her off of pain medicine, gave her antibiotics late every day after she begged them multiple times to give it to her at 5pm like the doctor ordered. They never gave it to her at the designated time. Instead, the nurses and technicians continued to go hours at a time without doing anything, my mother would call me crying saying that they ignored her. They have a fuction where a patient can only call once, the call button stops working after one click. The patient has to listen to it beep until they come to check on them. But, they'd take an hour just to see what my mom needed. What if she had fallen? My mom was supposed to get rehab here, but she came home weaker than she was when she was sentenced here. I'm sorry, I mean sent here. The whole time she was there, she got between 15 to 30 minutes of rehab per day. In two weeks, they gave her one shower. She was forced to urnitate on herself because they took too long to reapond to her asking for help. The other 23 hours of the day she was laying in bed, getting weaker. If I had an enemy, I would recommend this place.

I visited this facility

The tour I had at SouthPointe Healthcare Center was good. It's a newer building and larger than what I had seen before. I didn't have any interaction with the staff, only the admissions director who took me on the tour. That's a place I'd originally picked for my brother, but at the last minute when I brought him there, they said they didn't have a bed available when they told me they had. So, they shipped me off to their sister community. That was truly a horrible experience. The room she showed me was a nice large room. It's a single private room with its own roll-in bathroom with shower and toilet. The dining area looked pleasant. It had smaller tables set up.

I have had to place my mother in several facilitates for Rehab around Indiana. This company has taken long term healthcare to another level by providing residents with privacy. There are very few shared rooms, and even the shared rooms have a nice division wall unit. I am a very picky person when It comes to elder care, so far this place has my mother on a very happy path!

Rooms aren't bad and are kept clean. That being said food is not edible. Most times can't even figure out what it is. At one point there was no cook. My parent is paying out of pocket and the treatment is awful. She doesn't get all the treatments she is supposed to and sugar doesn't get taken a lot of times before meals. This place needs to be shut down until they fix things. They are always short staffed.

Some staff members important ones do not get back with you on pertinent information after you have called several times. They do not return calls. Patient got there in February didn't get a care plan meeting until May after asking several times.... That is unacceptable when you have a loved one in there. Had to call the state regulatory to get some action

Such a nice place! clean and friendly! I had to go there after having surgery on my ankle. I was there for 6 weeks and was able to go home and get back on my feet with my grandbabies. the staff was wonderful <3

Staff HARDLY EVER returned phone calls about my mom. They didn’t have us sign any admission papers, took dnr change over the phone (after crossing out her name that was incorrect on the original dnr and initialing it). We never got any copy of any changes. They drugged my mom up with way too many anti psychotic drugs without even talking to my sister or me beforehand, or even notifying us. She couldn’t reach her phone or call button, and was left to languish in bed. She developed worsening sores because she wasn’t moved or gotten up and they didn’t try to encourage her in therapy while she was suffering from covid or after she was moved back to the non covid unit. There was trash all over the grounds by the door, including masks and gloves. All in all it was a horrendous experience and I wouldn’t recommend this place to my worst enemy. I’d give minus stars if possible because of their lack of caring and compassion for an 88 year old who previously lived on her own at her home prior to a fall. HORRIBLE, SIMPLY HORRIBLE!!!

My mom was here twice 1st time was great changed hands it was a nightmare. They were told several times she could not tolerate pain meds they ignored it kept her sedated she could not do rehab I would visit room dirty she sat in urine for hours I ask that the md call never did. She also has a rare bowel disorder they did not make sure she took her meds so when I brought her home she was blocked. I only received a call from the business office they wanted to keep her in longterm care i again asked for an explanation never got a call. So on March bbn 8th 2020 my son brought her home got her clear of the pain meds bowels cleared home healthcare came she is now walking with a walker eating on her own she had an outstanding n.v balance she had several financial issues at one time and a was unable to start paying eff xplained that to them and they bvb treated me like a thief I dont control her finances she does all they wanted was to have her sell here property and leave her in this nightmare currently paying them a monthly mlm payment of400 month dont mind paying since it was from her 1st stay which was good they did try to over charge got that fixed. It is a beUtiful place but looks are definitely deceiving

From the moment I entered this facility to get rehab from surgeries there really wasn’t any Wound care, personal Care Or assistance. Pulling chain in bathroom for assistance Went Unanswered, left on bedpan for hours. Staff acted like it was a burden to assist me. One aid told me HE didn’t like my attitude. It was Difficult to get anyone to do what they agreed to do. One of the worst experiences of my life!

NEVER EVER EVER send anyone you love or hate to this facility. 4 days in here for rehab and a loved one had to be sent to the ER , dehydrated, unable to speak, bed sores, malnourished, frightened & hurt, the staff was not doing one thing they said they were doing or being paid to do all the way down to giving her meds. She was near death when she arrived back at St Francis ER and the staff that saw her just 4 days earlier laughing and telling jokes were now in utter shock of how she looked. The whole facility needs to be shut down, nursing and physicians licenses need to be taken. The truly do not stand by their oath. DON'T EVEN SEND YOUR DOG OR WORST ENEMY THERE CONSIDER YOURSELF WARNED!!!

Released from the hospital on May 31, 2020. When released, from the hospital, she was talking and on her way back to good health. She went to the Nursing home to regain strength in her ability to walk, after a month long illness. Released by the hospital to a rehabilitation/nursing home. June 5, 2020, sent by ambulance to the hospital. Once in the ER, she was found to have bed sores, dehydration, severe mucus build up in her mouth, so much so that her tongue was swollen to the point of blocking her mouth. Severely malnourished. The ER staff could not believe how she looked after only 4 days in a nursing home. Today, she has still not recovered. She lays in ICU, barely clinging to life.

My mother had rehab at Southpointe Healthcare Center for three weeks the first time. She's currently in rehab again for about one week. It's not very welcoming when you first step in. The receptionist is very nice, but our first visit was rather traumatic. We got there at 7:00 pm, and that was the first day of our very first admission. There was no one there to meet or greet us. I found a nurse down two hallways, and I told the executive director they could do some signage out there to tell people what to do if they bring somebody in for the first time after hours. That's been six weeks ago and nothing has been done. It was rather traumatic for the family especially my mom. They have been called ahead that she needed oxygen, but the bed was not ready, the bed wasn't made, and there was no oxygen in the room. It was a very poor start. They seem short staffed. The nursing staff or the help staff aren't very quick to respond to patients' needs. There's no way a patient can verbally speak to someone in the nurses' station. If they press the button, a light lights up in the nurses' room, but it doesn't make any noise. I was expecting the patient to at least have an option to be able to speak to someone at the nurses' station if they had something serious, and that's not an option at this location. They have scheduled activities like movies and Bingo. The therapists are a lot more together than the nursing and the staff that does the changing of the bed.

I would strongly encourage you NOT to bring any of your loved ones to this facility. The care was below average and the employees are rude and have zero time to help in any way. The only positive comment I have was the Physical Therapy department and that was only 30 min a day while my mother was there. There are too many things that can go wrong when your not looking or able to be there so please look at other facilities before deciding on this one and do your homework. Take your time and research research research and ask around.

Horrible, bad from the management on down. Food, if you can call it that sucks, long call light times even when on the toilet. Short staffed in all departments many employees are over whelmed with way too many patients per caregiver ratio. I have dialysis 3 days a week, my second week here they dropped the ball and on Saturday I had no transportation to my treatment which I must have in order to live since I have end stage kidney failure. Medications are always messed up. It being Saturday no one in management answers their phones and no one will come in to get me to and from my treatments. Why the hell am I even here.

I visited this facility

We really liked Kindred Transitional Care and Rehabilitation. It was very nice and the staff was very nice and attentive. The rooms were pretty new, clean, updated, and nice-sized. I didn't try the food, but it looked good and fresh.

They admitted my dying mother in late September /early October 2016 knowing she was actively dying and was unable to do ANY rehabilitation. So many things happened that I described in detail on Google. Worth reading, I'll assure you. Mom was placed at the end of the hall and ignored. She was in pain, and the nurses were giving her tylenol behind my back. I do know cancer pain in dying patients, as I've been a nurse since 1992, with much experience in hospice. Until January 2015, I taught nursing. 3 of my previous nursing students work at this facility One took care of Mom and was very kind and attentive. Another one had been recently appointed as unit manager on Mom's wing, and did some very unethical things. I am so glad that Mom spent her last night at a hospice house prior to passing, and not in that horrible place. I gave official kudos to all but 2 if Mom's nurses, not knowing about the tylenol incident. (Mom had stage IV lung cancer with metastasis to her brain, liver. And spleen.) She also had 3 pressure ulcers on her bottom and had a history of degeneration in her spine so bad that she had injections for years. But, according to many of her nurses, she didn't have pain, right? She was grimacing during her entire time there, as well as thrashing in the bed and moaning. But the way less educated and experienced nurses know more than me? I spent every possible moment with her, even sleeping in her room except for brief moments when I'd eat &shower. I returned once and her bed was sitting straight up in the dark. She had drool and boost dripping from her mouth. She was the dying woman at the end of the hall.

I am at Kindred Southpointe for more rehab and therapy. I was here once before in 2015 and I cannot say enough good things about this place! I did not dread coming because it is such a good place. You are treated with the upmost respect and not an old crank. The food is excellent. The nursing staff are all very friendly and gelpful. They treat me like their own family. I just love the therapy department they make it fun, they are just jewels! There are just people in life you meet in life that you have a special lifelong bond with, those people are at Southpointe for me. Everyone here is so professional and educated. I know my needs and my wants will be met here no questions asked. They've got a good thing going here! I seriously have NO complaints!!!

This facility is clean and nice. The staff is friendly and knowledgeable. My loved one is not always the most compliant but the staff are very patient with her from the housekeeping, dietary, nursing to the Administrator. I recommend this facility to everyone I know. The Administrator takes time out of her day to help my loved one with whatever she needs, she is a true Angel. Thanks for all you do.

A new nice facility should be secondary to staffing. I have seen caring hard working individuals, but weekends are either very short staffed or the help is less responsive. Four weekends in a row of very frustrating lack of care has had determined hard working friends feeling so helpless. She is wheelchair bound and using the knee bending machine. She has been left on toilets and knee bending machines far past time required with no one responding to calls for 45 min and more. At one point, she had to have family call the facility to get them to respond. She had had her foot stuck in knee bending machine almost an hour past time to come out. I would recommend checking reviews about staffing and not just looking at appearance of building before choosing facilities.

I visited this facility

My mother was in Southpointe for 8 week after having had knee replacement surgery. My mother has many health issues including Parkinson making her very unsteady on her feet. Before having her knee replacement she had fell many time with injuries every time. Our family was very concerned about her having a knee replacement, concerned her falling would get worse. I can honestly say Southpointe nursing and therapy staff has changed my mothers life for the better. Not only did they help her through the pain and regaining her balance but they also gave her tips on how to steady her gait while walking, how to balance when picking things up and even suggested medical equipment at home to make life easier to manage. My mother has not fallen and gets around better now than she has in more than 10 years. Thanks to Southpointe my family has a happier and healthier mom.

My mom is at Kindred SouthPointe. Their nursing and rehab staff have been good. Their dining room is clean. The rooms are very nice and very clean. My mom does bingo, which they have about twice a week. She also does wheel-of-fortune, which they have once a week. Then she gets her hair and nails done also once a week. The dining room is clean and nice, but I think they need more variety of food. Mom complains sometimes about that.
