I am a friend or relative of a current/past resident

I have been a caregiver for my mother for about 20 yrs and she has been in and out of facilities for rehab, respite, skilled nursing, etc AND my mother was a resident of this facility b4 when it was brand new. I say all of that to to say this, I have NEVER seen a facility ran this way before in all of my years. From the staff's lack of rapport with its residents, the RN's being rude when trying to share or obtain info, to them not contacting me when my loved one arrived from the hospital, to them using a litmus test of 12 questions to determine whether or not my mother can be deemed as cognizant enough to make health care decisions for herself when she cant tell you the current day, month or year. I had the chance to meet w/the Manager on Duty and the DON to try and iron out these concerns and although their words seemed to convey concern, it felt as if they were looking at me as if I were the foreign entity in the conversation. I am in such a precarious position because my mother is on dialysis and most facilities dont have in house transportation to meet that need, I thought that Evergreen having dialysis w/in the facility would be a blessing. But at this point, I am really just thinking I need to move her to another facility because it's like Im waiting for something serious to happen when all of these red flags should be enough...even with me going there several times a day to make sure that she is eating and dressed, I just dont know it that will be enough.

My husband is in Evergreen Crossings and the Lofts. It's very nice. I like the attention that they give the patients trying to make sure they're comfortable. My husband has a lot of problems, so it takes a lot of care. The food is not bad. The staff is just very friendly. The place itself is very calm but inviting. You can sit out and chat with other members there. It's very inviting. They need to improve on their physical appearance because it's changed quite a bit in the last two to two and a half years. You don't get that glamorous feeling that you used to get that was there before. They have a beauty-barber salon, and they have an exercise room or therapy room, which is very good. I'm satisfied with them at this time.

I want to know how the number of residents in your care are not receiving proper meals and care. My brother in law and I have had to come there to bring my sister food because she is not receiving breakfast NOR lunch. When she brings it to someone's attention you take her a brown bag. That is not good customer service. She has been there for 2 weeks and is constantly nit being fed nor cared for. How is she suppose to recuperate properly without her nourishment and proper care. if your staff does not want to do their job, they should be replaced.

Was great when opened, security 24/7, live activity,great food from chefs, interaction with older residents,beauty shop,gave prizes for bingo games-had more aides to care for residents day and nite, now all of that is gone-some nites ,only-1aide per shift.my mother was there for 3 years-if it wasn’t for a nite nurse( rose) who noticed my mother’s breathing wasn’t right,she would’ve died in her room there. My mother had some rude and mean nurses there -I wouldn’t refer this place to a -killer☹️

I visited this facility

I was there for physical therapy, and found they might need much more professionalism to compete or match with other similar places, such as process procedure, organization, documentation and effectiveness.

My mother-in-law was at the skilled nursing of Evergreen Crossing and the Lofts for some time. I think this place was fabulous. It's clean, beautiful, it's very modern and the staff was friendly. My mother-in-law has great success with the therapy. And she likes some of the food there but not all. They exceeded my expectations overall.

Lack of organization, poor to no communication, right hand didn't know what the left hand was doing, it took days to never to for my mother to receive TED hose and or medication. The food was delivered to the rooms cool to cold. There are more things I could say, however I will never refer anyone there nor will I allow my mother to go back there.

My husband was at Evergreen Crossings & the Lofts. It was a very fancy place, but I don’t think they were very good organizationally. They weren’t very helpful. The rehab didn’t go very well at all, and the things they told me were very confusing and didn’t help at all. We would ask them to provide us with something, and we would have to ask 3 different times before it didn't get done, if it got done at all. I was not convinced that his health got better, and I would not want to take him back there. I think they need to be more organized, and the communication between different groups needs to be improved. I believe the staff needs more training on skill and on commutation. At the end of the day I wasn't totally convince that my husband was okay. When it was time for my husband to get discharge the thing I needed to have at home wasn't available us.So sad!!!!!! I am giving them the 2nd star only because they have private rooms and they look fancy

I am a friend or relative of a resident

It’s been going okay, and my mother has been living more comfortably at Evergreen. She likes the activities and everything. They have bingo, and different groups come in like musicians, ministers for Bible study, and she enjoys it. I have eaten there, but she doesn’t care for the food because it’s not her cooking.
