I am a friend or relative of a current/past resident

The rooms are mostly double rooms with shared bathroom. Their food is very good most days. The activity program is packed with things for them to do. I'm happy I picked this place for my family members. Thank you to the staff for keeping them busy and happy.

My mom is at this facility and the team is fantastic. When my my first got their she was bed bound , she had to transfer to her chair with a device that picks her up into the chair. It's been about 7 months and her wounds are healed and she is walking with a walker. The staff treats her like family and loves her as much as I do. I want to thank everyone that helps her and shows her love.Evergren woods definitely was the right choice and I'm humbled by how much she feel at home . Thank you, from the bottom of my heart.

My brother lived at Evergreen woods Nursing facility in Spring Hill Florida for over a year, and we were very happy with his stay. The administration staff was very easy to work with and very helpful through the setup and process. The Nursing staff seemed to be very professional and capable yet friendly and kind to us.I always felt as if there was any need for help someone was always avalible. The facility seemed very clean and tidy, I been in other places and this center is like a breath of fresh air. The food seems to be fine for a nursing facility, some meals are great others ok and some well not so ok, but a proper diet is alway provided. My brother was given re-hab for recovery, they worked with him with the goal of improvement, they were very patient with him. Sadly my brother has passed away reciently and I know the staff did all they can do for him. If I had to do it all again, I would have no problem going back to Evergreen woods. I visited this facility

Please do not send your loved one there. Our mother received very poor personal care while her stay at this facility. Infact, my sisters and I were appalled at the health treatment our mother received. The OT /PT personnel were very professional and really did a great job with our mother. However, the housekeeping, aides and nurses care our mother our received was questionable.

DO NOT SEND A FRIEND OR FAMILY MEMBER TO THIS FACILITY!!! Long story short, my mother was admitted there for rehab from a broken femur. She was hospitalized for a UTI and thrust that was not treated correctly at Evergreen. She was then placed Hospice and passed away. This all happen in less than 30 days!!! I could go on and on but WARNING, KEEP YOUR EYES OPEN AT ALL TIMES!!

My father has been in Evergreen Woods Rehab since early February. It is now March 31, 2021. I have not been able speak with him. I have called several times. He contracted Covid while at the facility and was transferred to isolation. Was I informed? NO! He survived. Was I informed? NO! There is no communication. The staff I have spoken with is rude and has not interest in helping. My father is basically deaf and has some form of dementia of Alzheimers. He may not be able to hear the phone or may not know how to answer it. I hav not been kept informed of his condition during this time.

During my stay the staff always took care of my grandma. They have a full staff of nurses, all patients have private rooms with internet, huge bathrooms, kitchen, and living room with fresh flowers delivered daily, culinary chefs, respiratory therapy, PT, OT, ST, field trips to the museum, pets can stay with you, families get free meals, and transportation is provided to all the shopping centers, malls, and grocery store.

My Mother in Law was sent to this facility from the hospital for rehab therapy. . My Mother in Law has end stage of COPD. This facility is NO WAY set up, equipped or educated on having severe COPD Patients in their Rehab facility. My mother in law did a decline while at this facility and had to be rushed back to the hospital for respiratory distress due to the staff not properly caring for her. If this facility is going to take in end stage COPD patients to do rehab they should have respiratory team. My mother in law was found laying IN THE BED, with a canular on that was attached to the oxygen tank that was on the BACK of a wheel chair. NOW, do oxygen tanks run out of oxygen?? YES they do, and guess what happened. WHY WHY WHY would they be so lazy to put her IN BED while still attached to a tank, that is on the back of the wheel chair instead of changing it to the proper oxygen machine. They also turned up her oxygen to number 5(FIVE) which is a BIG NO NO when you have COPD. Another time she was in the bathroom with the canular on, but the tank was never turned on. They would give her the nebulizer treatment but take her oxygen off, leave the room, and again have no oxygen on and not come back to see if the nebulizer was finished. Her oxygen is her life line. You do not take it off for the breathing treatment.. Even at the hospital it was ALWAYS left on. This facility should NOT take end stage COPD patients and if they continue to do so, they need to have someone come in and train their staff on how to care for a patient properly!! My mother in law is in ICU at this moment. Prior to her coming to this facility she was able to talk, stand, turn around, etc, and today due to them not knowing how to properly care for someone in her condition, she took a decline!

I was a patient in this rehab several times and have been treated with nothing but respect and loving care each time. I couldn't have asked for better care, they made me feel right at home. The staff would go out of their way to solve any problems I had and the food was good too!

My grandmother was a patient several times due to falls,They staff was beyond amazing and the building was always clean and did not have any unpleasant odors.She enjoyed the many fun activities and entertainment in between therapy of course! As her health declined, we as a family had to decide what was best for her,she made that eaxy.,without hesitation she chose .Evergreen to be her new forever home.She referred to the staff as her angels on earth.She spent her last 4 years of life there. She was treated with so much love,compassion ,and ,dignity .I will forever be grateful for the care my grandma received!

My father was admitted into Evergreen Woods, and he was there for a week. It was very nice, very large, and was in a nice setting. The staff was kind, courteous, were very organized. The room was nice. They attended to him very well. They gave him his meds. I did not have any complaints about it.

I visited this facility

My brother-in-law has rehab at Evergreen Woods, and I do not like it. The closets had his dirty, smelly diaper-type things in with his clean clothes or I'd find a diaper just laying somewhere. In fact, I had put my purse on a dirty diaper, and then I sat on a chair with it. I was so mad. But a rehab is a rehab. He liked his physical therapist and said they were nice to him.

My mom was in Evergreen Woods for a couple of months. The staff was good. Her room was very nice. It had a bed, a nightstand, a TV up on the wall, and any kind of medical equipment that you would need. They had bingo, arts & crafts, and rehabilitation. It was very nice and clean.

My husband was sent to Evergreen Woods after his hospitalization for MRSA. It was decent. Everybody seemed to be okay but we had issues with one of the admitting staff telling his boss something I never said. He stayed there for a month and he was returned to the hospital for a mental problem. The second time he was sent there from the hospital, they wouldn't keep him because of his mental status but they wouldn't find another place either. They sent him back to the hospital and his doctor admitted him until we could find another place.

Although the nursing staff is very good and attentive, we have serious concerns of the quality of medical care provided by the physician. There is also a serious lack of effective communication between the Medical Staff and the Social Services Team.
