I am/was a resident of this facility

I was a patient here twice. The first time was fine. But this last time was a horrible! One of the nurses tried to give me medication that was not for me. I reported this and spoke with the unit manager. She was made aware of what happened. There were two roaches in my room, this was also reported. Some of the staff was on point. But some acted as if they were doing you a favor by coming to work. When I was discharged I was told I would get care at home from an RN, PT and OT. So far no one has contacted me. I was told by PT they had sent in for me for me to get a motorize scooter and a walker, so far my doctor has not received any of the forms! I was discharged almost two weeks ago and nothing! I would NOT advise anyone to go to St, Catherines unless there has been a lot of improvement from the nursing staff to patient care. Also social services need to be more helpful and not trying to get business for their friends!

I am/was a resident of this facility

Place is beautiful from the outside looking in, however once inside, as a short term resident I was counting down the days until I was discharged. They give you a menu to pick the meals you want but you never get what's on the menu because they never have it. What food they do give you is always served cold. Never no hot meals. With the exception of two aides that we're very caring and pleasant, I found the staff to be very lazy in performing their duties. I would not recommend this place at all

Brother (in last couple weeks) received Unsatisfactory care except for PT…..need more adequate staffing. Room adequate. Needs to be better response to patient pushing button for help for incontinence, bedside water, etc. Patient had to return to Hospital/ER for dehydration, UTI, extreme constipation. Communication between providers & with patient/family could be improved. Family continued to ask for conversation with facility doctor. Patient felt needs not addressed. Family must visit a lot! Requests for help succeeded when family present. Doctors orders sometimes not available or accurate as had been told to family at doctors visit.

Provider Response

We appreciate the review and we take your feedback extremely seriously. We would like to listen to the details around your concern. Would it be possible for you to reach out to our Executive Director? Thanks!

To start the 1 star is only because it would not allow to go further without a rating...that being said my father-in-law was placed in St Catherine's by St Vincent's hospital after a 2 week stay in the hospital to help with his mobility in which from day one in the rehab center was a nightmare for my wife and her mother....during his 3 days in this facility my wife who is very involved with the care of her mom and dad was on the phone if not at the facility itself reminding them about medications that her dad MUST have due to his heart condition only for it to fall on deaf ears, there are 5 or 6 meds that her dad must take everyday for the rest of his life that he was not receiving on a regular schedule, her dad has now gotten worse in a 3 days rather than getting better and has been put back in the hospital because of their lack of care for him...in just 3 short days our view of this facilty is it needs to be shut down and we WILL do our part to see that actions are taken against this facility.Their actions or lack of will be reported on a State level ,as for my opening comment maybe if y'all showed more concern for the individuals that are placed in y'alls care by family members than a rating maybe this would be a better place...also take notice of their survey it's all about them and not about the welfare of the individuals..If you love your family members do not send them to this facility unless you are ready to have some long restless days and night

WORST FACILITY EVER

My aunt went into this facility walking and needing rehab to get her stronger for a speedy recovery after a dog bite she came home in a vegetable state very weak not even knowing when she had to use the restroom also she had severe pressure wounds and bed soars on her butt and legs. she has since had to have both legs amputated. I was currently looking at long term living facilities and came across the reviews i wish i would have done this prior to her stay she would have been better off. So i along with my family agree that we would not let St Catherine care for a dead dog. I pray my family story saves someone else.

WARNING! Sent my father in walking and he came out a vegetable. I believe he laid in bed for three weeks and lost so much weight and muscle strength that he could not walk at all when he came home. He contracted pneumonia and no one told me for eight days. (They were allowing no visitation from family because of the Covid so I could not even check on him.) They didn’t even test him for Covid at the time they diagnosed pneumonia. If you want your loved one to die quickly by all means send them to this place. It has taken weeks of physical therapy in my house to get my father strong enough to even use his walker again. They were supposed to be the ones getting him strong and they sent him out mentally ready to die… Incompetent staff and people who don’t care!!!!

There are always one or two outstanding, caring ones per shift but the overall care for avoiding disasters small or large are not in place! You will need to sub care and advocate for your loved one constantly! I had hoped for more based on other Med ratings but this has been a battle!! I am sick today and cannot go in and am calling every few hours to make sure he is ok! Have to get better and get back tomorrow! If her needs help again from being assisted to eat to changing to other emergency needs I have no confidence! Yes, alarms go off and are not answered, lights flash and do no good! People scream for help and no one comes!!!

This facility used to be great, but my mother is in for another rehab and our experience has been horrible!! I am looking to find another rehab to transfer her to, because she has gotten worse instead of better since being there. She has been left several times for hours with her bed soiled. There is no "call button" for a nurse, simply a light over there door that the patients can activate that is completely ignored. My mother's colostomy bag is rarely changed and on several occasions has over flowed to the point that her surgery incision is now infected from this neglect. Though it is know that she can not walk on her own, there is no safety features that alert the nurses of she gets out of bed. Last night she attempted to get herself to the bathroom after waiting for hours for someone to come help her with no response. She fell out of her wheelchair while attempting this and I was not notified until the day after the incident. When I try to confront any nurse or CNA about the issues, they all just complain about being over worked and under staffed. Every nurse we have had has stated the same issues. They just complain about how many patients they have to take care of. They are stressed and unable to care for any of the patients they way that they should be cared for. One nurse confessed to me that there were changes in the administration, or that perhaps they place was under new ownership and that conditions now were deplorable. This nurse came to St. Catherine's because of it's great reputation, which is sadly no longer the case. My mother was here a few months ago and it was a wonderful experience then. My grandmother also resided at St. Catherine's until her death, and we were more than pleased with the care that she received. Please go back to your old ways! I will be filing a complaint with the Department of Health.

My dad is a permanent resident here. You have a few CNAs and nurses that are excellent but I have some over all concerns. the minute you step off the elevator onto the second floor the entire place smells of urine. It is so overwhelming it takes your breath away. There was a recent medical issue with my dad and he has YET to be seen by the in house Dr. even after some test were run. I went to find a nurse to talk to her about my dad and the CNA told me she was with a patient but would be down to see me shortly. She never showed up. I went and hunted her down after waiting an hour and she was at the nurses station doing paper work. I was never acknowledged by her and a CNA interrupted her and told her I needed to speak with her. The nurse acted as if she was too busy to speak with me about my fathers health and NEVER answered my direct questions. It has been a week since my fathers medical concerns and even my mother is frustrated because she is not getting answers either. I am now stepping in on her behalf and will go to the acting administrator to see if I can get some satisfactory answers.

I would not recommend this facility to anyone. My mother has been in the dementia unit at St. Catherine's for several years. CNAs take care of 60 patients in the ward and they don't have enough of them to provide quality care. The nurses won't do anything for the patients other then give them medicine - they won't even help feed those who can't eat by themselves. Over one holiday weekend, my mother's prescription ran out and no one on the nursing staff was concerned enough to even notice my mom was suffering from severe withdrawal symptoms for three days! They recently changed the CNA shift schedule from three 8 hour shifts to two 12-hour shifts and the day-to-day care has significantly deteriorated. Often times I visit my mother and find her with soiled clothing because she hasn't been toileted. Even though I take home her laundry twice a week (which always smells like urine), her clothes disappear from her closet all the time and I constantly have to buy her new ones. The hallways smell and there is no visiting space within the ward for families except for my mom's room, which she shares with another patient. We used to go to the main dining hall on the first floor to visit (which they used for activities when its not mealtime), but now they have turned it into a rehabilitation space, so it is no longer available. The facility is old and outdated with alot of old/broken furniture and wheelchairs, and a heating and cooling system in the rooms that doesn't seem to work well. The dementia ward activity room is small and can't fit more than 20-24 people (many in wheelchairs), so many of the other 40 patients sit in wheelchairs or loungers in the hallways, staring into space or in my mother's case, at the fluorescent lights in the ceiling. Imagine doing that all day, every day. While visiting my mom one day, one patient was calling out over and over again, so I finally went up to her and asked if she needed anything. Her reply: "I need someone to be kind to me." I know dementia care is difficult and exhausting (which is why no one on that ward should have to work a 12 hour shift), but this is really unacceptable and something needs to be done. I have tried to transfer my mom to other facilities but there are long waiting lists for those on Medicaid, so I haven't been able to find someplace else. It is so very sad for my mom and all the other residents at St. Catherine's.

My mother was at St Vincent's first then sent to St Catherine's until she was strong enough to go home. My mother was in good condition when she arrived, Within 24 hours she was incoherent, did not recognize me, was twitching uncontrollably, was running a fever and had been left to soil herself in a wheel chair. My mother's balance is terrible and she should have NEVER been left unattended in a wheelchair. She is supposed to be on oxygen at all times, it wasn't hooked up to her. When I asked how high was her fever, they didn't even know she was feverish. She had not her any of her medicine for the day and she is a diabetic. When I left the room to place an emergency phone call, I came back to find her naked in the wheel chair while they found a gown for her!! This is deplorable and inexcusable! Because of this neglect, she had to be transferred back to St Vincent's with a fever over 103. It is amazing that this is the level of care that is considered excellent!

The physical structure of St Catherine's is appealing and attracts people. The staff however is totally different. Patients that are not mobile are not toileted on a regular basis and patients with dementia are left unattended. Nurses force patients with swallowing problems to take medicine while partially asleep which could cause choking. They don't pay attention to patient's menu's that have restrictions. All of the staff is not the same. There are actually a couple of staff members that should be there. Everybody is not meant to work with the geriatric population.

My mother spent 8 wonderful years at St. CATHERINE'S WHICH SHE ALWAYS IDENTIFIED AS "home".the last 1.5 years were in the memory unit and while my mother had a very social personality, that helped her enjoy beauty shop, meals ,others visiting. I told the nurses and staff that my mother lived as long as she did (89)because of their attentiveness, hugs, care, and love. It may not be newest in physical space, but having moved my mother from GA where the laws for care are horrific ,to here and then 3 times to get to St, Catherine's , I thank God and the Angels and bless friends who helped get her there . Dr.Narvel who cared for her many of those years And all those who adored her made my life easier since I had to continue working, snd couldnt take over the 24 hr care suddenly needed following a stroke. When mother passed they annouced "she wouLd be leving at 10 am" Over the p a, and people formed along the hall paths all the way to take my motherr out the front door,to the portico, the intent being she would go out the door ,the same door she came in,with the hall lined with staff, housekeeper, nurses, admin. Patients, visitors all to bid her farewell and for me --one of the most loving snd breautiful mome nts ,healing and showing the love and care for our family and my beautiful mother. No other facility does this and is just part of the extraordinary loving thoughtfulness that is coupled eith fine medical care. I visited each day and where any of my concerns were always heard, reported back to me as needed, and where they kept my mother so happy And as healthy as possible. I was there every day,so It hurts me to see negative reviews since I know what we went through before St. Catherine's and I am forever appreciative and grateful for it. They are all so remarkable and work hard to make all outstanding and I am forever grateful to them.

My father had heart surgery at St Vincent's. His surgeons were great. We were lied to about a bed available at a facility close to our home and that he would have to go to St. Catherine's. He was to be on a cardiac & diabetic diet. They fed him everything he was not to have. When told he didn't receive his meds they said the pharmacy was closed. I had him transfered the next day. Even that was a difficult time with them. Would not recommend at all.

This is the ONLY facility my Elder will stay in, when they have to re-cooperate and restart physical and occupational therapy rehabilitation. We have seen the facility as described below, but we have also watched as the facility has listened to these postings and made vast changes. This past stay at St. Catherine's was phenomenal with all levels of staff listening, caring, paying attention and in some instances would anticipate a need just before it would happen. Even the food quality, taste and texture has improved. Amazing. Thank you, to the Staff of St. Catherine's for all you have done for our family and for successfully returning to us a stronger, healthier Elder.

St. Catherine Laboure Manor was a wonderful place. The nurses were great and they had a lot of activities for the residents. The rooms were clean and were kept clean, it did not stink at all. I would recommend this facility to other people.

Our situation was that my relative was sent to St. Catherine Laboure Manor for a rehab. This facility is associated with the hospital that she was in. However, her rehab was not successful enough for her to be able to leave that place. Also it is easier for us not to move her to another facility, so she just stays there. The people working there are fine. She has been there for several months now.

St. Catherine Laboure Manor is part of St. Vincent Hospital. Most of the patients when they had surgery and they need rehabilitation, St. Vincent will send them to St. Catherine as long as there are available beds and there happened to be one available bed for my mother when she got there. She stayed there for three months. The staff was phenomenal, very attentive. She had wonderful CNAs, two good nurses, and her managed care counselor was really good at communicating. My mom did fine in therapy. It's an older facility but it was still very clean.

This is the worst place for anyone that can't speak for them self's. This place is all about getting a pay check not the care of the people they are to take care of. My Dad is in the lock down area with Alzheimer's and they can't seem to get rid of the SCABIES that he's had now for about a year. This place need's to be closed down. They are all a bunch of hypocrites.

My Grandmother is in the Memory Ward at SCLM. The condition of the facility is appalling. The Staff is neither resourceful or professional. The Memory Ward is plagued with c diff colitis and universal precautions are NOT being practiced. There is a lack of compassion coupled with a lackadaisical sense of existence-it feels as though because of the Dementia, Alzheimer's or other neurological disorders that these wonderful human beings are being dishonored in their disabled state. Many articles have been stolen from my Grandmother's room. The atmosphere is not safe-patients have fallen, vacant rooms are used for sexual relations by residents, etc. We are currently weighing if we should keep here at St. Catherine's. We are present each day but some things that I will not mention have occurred which has resulted in a great breech of confidence. The nurse manager has no idea what is happening in her area. I often think would she turn a deaf ear to the cries of her mother as she does these noble people whom she has been charged to for and protect...She is failing and our loved ones are suffering!
